While it’s important to understand what causes acne and worsens a current skin condition, it’s also important to understand the things that don’t affect your acne, no matter what you’ve been told. Luckily, there’s a dermatologist who’s garnered quite the acne-obsessed cult following: Sandra Lee, MD, a board-certified dermatologist in Southern California—aka Dr. Pimple Popper. Her YouTube channel is crazy popular, recently logging its billionth view. In her videos, not only does she educate people on what acne is, but she features footage of all sorts of skin issues erupting, including blackheads, pustules, cysts, and more. (There’s a “graphic and shocking” warning that precedes her videos—fair warning!)

People with darker skin often see post-inflammatory hyperpigmentation — marks that appear brown. Those with lighter skin often develop post-inflammatory erythema — which show up as purple or red marks.

Retinoids and retinoid-like drugs. These come as creams, gels and lotions. Retinoid drugs are derived from vitamin A and include tretinoin (Avita, Retin-A, others), adapalene (Differin) and tazarotene (Tazorac, Avage). You apply this medication in the evening, beginning with three times a week, then daily as your skin becomes used to it. It works by preventing plugging of the hair follicles.

The good point is that it is permanent, unlike the other dermal fillers, but it requires a more specialised technique that must be perfomed with care by a skilled professional. A mistake is harder to correct.

If you’re going to take matters into your own hands, at least pop responsibly. Wait until the pimple comes to the surface of skin (you’ll see the white, pus-filled head of the pimple). Popping a pimple when your skin is ready “will cause the most shallow trauma to skin and decrease the likelihood of permanent scarring,” says Dr. Lee. You’re also more likely to get everything out; if you don’t, the spot will just get inflamed again.

Shah often recommends over-the-counter retinols or prescription retinoids to her acne-prone patients. “I find that compared to other treatments they are beneficial for not just treating acne but also preventing new acne from forming as they help prevent that initial stage of the follicle getting clogged,” she says. “They can also help with some of the post acne [problems] such as hyperpigmentation.” But keep in mind if you have sensitive skin (or eczema or rosacea), a prescription retinoid might be too strong an option. However, your dermatologist can recommend an over-the-counter retinol with a low concentration (0.1 to 0.25 percent), which might be better tolerated. Retinol also isn’t a quick fix. It takes time to see results, and it’s something you’ll have to keep using to maintain its benefits. Shah also mentions that retinol plays well with other acne treatments on the list. “Retinol can be combined with other over-the-counter or prescription medications such as benzoyl peroxide, topical antibiotics, and oral medications. Most of these medications are prescribed by a dermatologist after a thorough evaluation of the skin, medical history, hormonal status, allergies and other factors that go into the decision about acne treatment choices. There are some over-the-counter options that can be tried first, like benzoyl peroxide washes or gels.

If you have been taking topical retinoids while you were pregnant do not immediately assume the worst. Findings from a study of a large number of pregnancies where the mother was exposed to topical retinoids, along with data from studies looking into the absorption of retinoids after use of topical cream on the face, suggests that you have not exposed your child to any extra risks. Even patients who are being treated for depression are not barred from taking isotretinoin, whose striking success often improves the mood and outlook of patients with severe disease.

Each inflamed spot will heal eventually. In some cases, the area of skin that was inflamed remains discoloured for several months after the inflammation has gone (post-inflammatory hyperpigmentation). This is often more noticeable in darker-skinned people. Also, a small pitted scar is commonly left on the skin where there was an inflamed spot. Hormonal agents provide effective second-line treatment in women with acne regardless of underlying hormonal abnormalities.30 It is not necessary to demonstrate androgen excess to achieve a benefit from antiandrogen therapy. Isotretinoin: Accutane was the original brand name; there are now several generic versions in common use, including Sotret, Claravis, and Amnesteem. Isotretinoin is an excellent treatment for severe, scarring, persistent acne and has been used on millions of patients since it was introduced in Europe in 1971 and in the U.S. in 1982. It should be used for people with severe acne, chiefly of the cystic variety, that has been unresponsive to conventional therapies like those listed above. If taken in sufficient dosage, it should eliminate the need to continue the use of prescription drugs in most patients. The drug has many potential serious side effects and requires a number of unique controls before it is prescribed. This means that isotretinoin is not a good choice for people whose acne is not that severe but who are frustrated and want “something that will knock acne out once and for all.” In order to use the drug, the prescribing physician, the patient, and the supplying pharmacy must be enrolled in the online “iPLEDGE PROGRAM.” Used properly, isotretinoin is safe and produces few side effects beyond dry lips and occasional muscle aches. This drug is prescribed for five to six months at a dosage that has a high likelihood of preventing the return of acne. Fasting blood tests are monitored monthly to check liver function and the level of triglycerides, substances related to cholesterol, which often rise a bit during treatment but rarely to the point at which treatment has to be modified or stopped.

Chemical peels: Whether the superficial peels (like glycolic acid) performed by estheticians or deeper ones performed in the doctor’s office, chemical peels are of modest, supportive benefit only, and in general, they do not substitute for regular therapy.

Buying a generic acne wash won’t necessarily improve your complexion; for a cleanser to be most effective, you have to pay attention to your skin’s needs and pick the ingredients accordingly. If your skin tends to be oily, choose products with salicylic acid, benzoyl peroxide, or glycolic acid. For sensitive skin, look for cleansers with lactic acid or hydrating ingredients like glycerin, which aren’t as drying as those made for oilier types.

When compared to ice pick scars that appear more column-like, box car scars look more angular with very sharp vertical edges that look, like a box (hence the name). The depression for this kind of scar is wider than the ice pick’s, giving the skin a pitted appearance.

Several factors contribute to the development of acne. The primary problem is change in the development of cells inside the hair follicle, leading to the formation of a plug or (comedo). The plug disrupts the normal movement of hair, skin cells, and grease (sebum), resulting in enlargement and eventually rupture of the hair follicle. Treatment should be directed toward the known pathogenic factors involved in acne. These include follicular hyperproliferation, excess sebum, Cutibacterium acnes (formerly Propionibacterium acnes) infection, and inflammation. The grade and severity of the acne help in determining which of the following treatments, alone or in combination, is most appropriate.

Similar to the above natural acne remedies, these acne treatments can be inexpensive and worth giving a try. Best of all, these use products you probably already have in your household, like apple cider vinegar and toothpaste. Use home remedies for acne with caution if you have cystic acne, open sores, or inflammatory acne, which are best treated with medical supervision.

Dermabrasion is used for acne scar removal and to treat skin imperfections like wrinkles. Dermabrasion can only be performed by a qualified medical professional, as it involves removing the top several layers on skin. New skin grows over a period of weeks to replace the damaged layers.

There’s always a time and place for everything, including this. When you finally spot that whitehead, “you can gently squeeze it out as long as it comes out in one or two tries,” Renée specifies. “Anything more than that means it wasn’t ready to come out and you will injure the skin resulting in a red or dark scar.” If you choose this path, make sure you wrap your fingers in tissues and prep the blemish with a warm, damp washcloth for two minutes can soften up the skin to make for an easier extraction. If these treatments are ineffective, it may be necessary to have a dermatologist give you corticosteroid injections. These injections tend to reduce inflammation in the nodules and allow the nodules to heal more quickly, lessening the chance of them leaving scars. If all else fails, a medication known as isotretinoin will often help. Isotretinoin is available by prescription only and its use needs to be carefully monitored since this medication has been known to produce severe and even potentially dangerous side effects on occasion. Dermatologists will normally refuse to prescribe isotretinoin for women who are pregnant, as it has been known to be a cause of birth defects. On the positive side, isotretinoin is quite effective in reducing sebum production, a major contributor to acne, in normalizing follicular desquamation or the thinning and peeling of the skin, in reducing bacterial activity, and in acting as an anti-inflammatory agent.

Make a natural exfoliating scrub. This will help remove dead skin cells that can clog pores and cause breakouts. Squeeze one grapefruit into a bowl with 1 1/2 cups of white sugar and 1/2 cup of coarse sea salt. Massage into the affected areas, then pat dry.

Silicone gel sheets work well with hypertrophic scars. They have been known to soften, flatten and improve the appearance of scars, but they won’t disappear totally. The most famous brand is this one.

When you do shower, be sure to rinse conditioner out of your hair. One potential cause of bacne is conditioner that hasn’t been properly washed from hair. Conditioner is great for your hair, but not so great for your back. There are several ways that you can help keep conditioner from invading your back and causing nasty little zits:

Almost all patients with this severe form of acne will require some form of systemic (oral) medication to control this condition. The goal of therapy is to minimize scarring. Benzoyl peroxide attacks the P. acnes bacteria. Once it’s on your face, benzoyl peroxide breaks into oxygen and benzoic acid, which rip through the bacteria’s membranes. However, one of its main side effects is dryness: If you’re going to use anything with benzoyl peroxide, make sure to moisturize afterwards. Sulfur and azelaic acid are less common and less severe alternatives to benzoyl peroxide, which can sometimes leave skin dry and irritated. “I use sulfur-based treatments a lot in adults,” says Dr. Peter Lio, assistant professor of clinical dermatology at Northwestern University. “It’s a good fit for patients who can’t tolerate the side effects of benzoyl peroxide.”

Different variants of acne exist, including acne conglobata, acne fulminans, acne mechanica, excoriated acne, chloracne, drug-induced acne (e.g., from anabolic steroids, corticosteroids, isoniazid, lithium, phenytoin), neonatal and infantile acne, and occupational acne. These variations have a similar clinical and histologic appearance to acne vulgaris, but they are distinguishable by clinical setting, severity and associated symptoms. Washing your face with regular soap is not enough to make acne better. The best face wash for acne is effective at removing oil and dirt, but still gentle enough to use regularly without overdrying your skin.
