Acne is a skin condition that occurs when your hair follicles become plugged with oil and dead skin cells. It often causes whiteheads, blackheads or pimples, and usually appears on the face, forehead, chest, upper back and shoulders. Acne is most common among teenagers, though it affects people of all ages.
Other concerns include inflammatory bowel disease and the risk of depression and suicide in patients taking isotretinoin. Recent evidence seems to indicate that these problems are exceedingly rare. Government oversight has resulted in a highly publicized and very burdensome national registration system for those taking the drug. This has reinforced concerns in many patients and their families have that isotretinoin is dangerous. In fact, large-scale studies so far have shown no convincing evidence of increased risk for those taking isotretinoin compared with the general population. It is important for those taking this drug to report changes in mood or bowel habits (or any other symptoms) to their doctors. Even patients who are being treated for depression are not barred from taking isotretinoin, whose striking success often improves the mood and outlook of patients with severe disease.
Avoid acne causing cosmetics. Any products that you use should be oil-free, water based, and non-comedogenic. These products will be less likely to cause acne. Also pay attention to the hairsprays, gels, and other hair styling products you are using. These products can clog your pores as well. Try to keep them away from your face when you use them.

Three years ago, overcome by the squalor of my home, I decided to hire a cleaner. I scanned Craigslist, feeling a prick of guilt; few things arouse class angst as reliably as the purchase of domestic help. Then I remembered another option. Near my Connecticut home was a refugee-resettlement center. On weekdays, dozens of recent arrivals loitered there, eager for work. This seemed to offer a solution to both my squalor and my angst. To pay a Craigslist gig worker felt a little icky. To pay a refugee—well, that felt magnanimous, almost patriotic.
The recommended starting dose of isotretinoin is 0.5 mg/kg/day. Daily doses can be titrated up or down, between 0.1 and 1 mg/kg/day, depending on response to treatment and presence of adverse effects. The total dose of isotretinoin over the course of treatment should be between 120 – 150 mg/kg of body weight.14 The length of treatment is dictated by the daily dose. Courses usually last between three to four months but may continue for one year or longer. Total doses of >150 mg/kg are associated with increased adverse effects.16 The dose and duration of the course may be adjusted on an individual basis; some patients will wish to reduce the dose or stop isotretinoin when the active acne has settled.
Acne that’s more inflammatory is denoted as moderate acne. This happens when there are occasional nodules and possibly mild scarring. Severe acne occurs when there are a lot of inflammatory lesions, nodules and possibly scarring. It’s also considered severe if the acne is still present after six months of treatment or if it causes serious psychological issues.
Don’t ignore side effects. If you’re experiencing side effects from prescribed medications, speak up to your dermatologist. Individuals react to drugs differently; your treatment plan must work for you.
Most patients with mild acne can be treated with topical treatment (gels, solutions and lotions) that can be obtained over-the-counter in New Zealand without prescription.
