Honestly, I’ve only had almost-perfect skin when I was using a harsh prescription cream (would never do that again), and at times when I have been overall healthy (mind, diet, lifestyle habits). As isotretinoin waged a scorched-earth strategy on my face that left me feeling like my skin was practically falling off, I was also weirdly excited by the changes I saw taking place. For more severe forms of acne, you may need prescription medication. Grade II: This acne grade refers to moderate acne that features a higher saturation of blackheads and whiteheads along with a higher number of pimples. Breakouts are more frequent than Grade I acne, and there may be development of papules and pustules.

Although the late stages of pregnancy are associated with an increase in sebaceous gland activity in the skin, pregnancy has not been reliably associated with worsened acne severity.[97] In general, topically applied medications are considered the first-line approach to acne treatment during pregnancy, as they have little systemic absorption and are therefore unlikely to harm a developing fetus.[97] Highly recommended therapies include topically applied benzoyl peroxide (category C) and azelaic acid (category B).[97] Salicylic acid carries a category C safety rating due to higher systemic absorption (9–25%), and an association between the use of anti-inflammatory medications in the third trimester and adverse effects to the developing fetus including too little amniotic fluid in the uterus and early closure of the babies’ ductus arteriosus blood vessel.[46][97] Prolonged use of salicylic acid over significant areas of the skin or under occlusive dressings is not recommended as these methods increase systemic absorption and the potential for fetal harm.[97] Tretinoin (category C) and adapalene (category C) are very poorly absorbed, but certain studies have suggested teratogenic effects in the first trimester.[97] Due to persistent safety concerns, topical retinoids are not recommended for use during pregnancy.[98] In studies examining the effects of topical retinoids during pregnancy, fetal harm has not been seen in the second and third trimesters.[97] Retinoids contraindicated for use during pregnancy include the topical retinoid tazarotene, and oral retinoids isotretinoin and acitretin (all category X).[97] Spironolactone is relatively contraindicated for use during pregnancy due to its antiandrogen effects.[1] Finasteride is not recommended as it is highly teratogenic.[1]

A range of OTC preparations is available from pharmacies and drugstores, usually in the form of gels, pads, creams, lotions, and soaps. Most of these are topical treatments, for applying directly onto the skin.

Spironolactone . Spironolactone is an oral drug that can block the action of the body’s hormones on the skin’s oil glands. This medication is not FDA-approved for acne, but is especially helpful for women who have acne that worsens around the time of menstruation.

Getting acne conglobata under control usually requires a combination of steroid drugs and antibiotics. Repairing skin damage caused by acne conglobata is not something that can be done with moisturizers, exfoliants, and peels. Laser resurfacing may help. There is no clear evidence that acne vulgaris is related to poor hygiene or that frequent face washing lessens acne. Patients should be instructed to wash their face gently with warm water and mild soap twice daily; rough scrubbing can cause new lesions because of follicular rupture. In the skin follicle, benzoyl peroxide kills P. acnes by oxidizing its proteins through the formation of oxygen free radicals and benzoic acid. These free radicals are thought to interfere with the bacterium’s metabolism and ability to make proteins.[77][78] Additionally, benzoyl peroxide is mildly effective at breaking down comedones and inhibiting inflammation.[76][78] Benzoyl peroxide may be paired with a topical antibiotic or retinoid such as benzoyl peroxide/clindamycin and benzoyl peroxide/adapalene, respectively.[35]

The first part of any treatment plan is diagnosis. Understanding how to treat acne truly starts with understanding acne itself. Self-diagnosing your acne can be a good start, but with severe cases, you’ll want to schedule an appointment with a licensed dermatologist.

Adverse effects associated with tetracycline antibiotics include oesophageal irritation, photosensitivity, Candida albicans vulvovaginitis and nausea and vomiting. Oesophageal and gastrointestinal irritation can be reduced by taking doxycycline with a glass of water and food, and advising the patient to avoid lying down for one hour after the dose is taken.

Bacteria. People with acne skin tend to have an oily, shiny complexion. This provides an ideal environment for the usually harmless acne bacteria (Propionibacterium acnes) to multiply and cause irritation and inflammation to the enlarged glands.

For women taking birth control pills, many of those preparations have androgenic (male hormone-like) properties and can contribute to significant acne eruptions. Progestins in these medications, depot injections and intrauterine devices are the most likely to cause acne.

Some hormonal therapies may be effective in the treatment of acne vulgaris. Estrogen can be used to decrease sebum production. Additionally, it reduces ovarian production of androgens by suppressing gonadotrophin release. [31] Oral contraceptives also increase hepatic synthesis of sex hormone–binding globulin, resulting in an overall decrease in circulating free testosterone. Combination birth control pills have shown efficacy in the treatment of acne vulgaris. [44]

Antibacterial treatments such as benzoyl peroxide – which can be bought over the counter – will reduce the bacteria on your skin and may ease acne. However, there is no conclusive proof that they really do work. They may irritate your skin at first but this tends to settle down after a few usages. If you are going to see an improvement, it will take some weeks before it becomes noticeable. Take care not to get any on your clothing as it will make colours fade.

When your acid mantle is damaged, you’re actually more susceptible to breakouts, enlarged pores, and inflammation. To make matters worse, evaporating all the oil on your face can actually set your sebaceous glands into overdrive, leaving your skin oilier than ever. To understand acne, you have to start with human hair. Hair shafts cover the human body and are everywhere except the palms of the hands and the soles of the feet. Grade II: This acne grade refers to moderate acne that features a higher saturation of blackheads and whiteheads along with a higher number of pimples. Breakouts are more frequent than Grade I acne, and there may be development of papules and pustules.

If the skin on your butt is rough, don’t forget about exfoliating. Use body scrubs but remember to moisturize afterwards. Over-exfoliating can actually cause acne to get worse, which is the opposite of what you want (of course).

Dermatologists sometimes prescribe antibiotics to help reduce the amount of bacteria getting trapped inside pores. Examples of antibiotics prescribed to treat acne include clindamycin, doxycycline, erythromycin, and tetracycline. (8)

Treatment is normally no different than that for the milder acne forms. Over-the-counter products and even some home remedies can be effective, but since this type is relatively severe, it may be necessary to visit a dermatologist if the products or home remedies you are using are not effective. Many, though not all, over-the-counter products contain three important agents used in fighting acne:

Use a retinoid. As a Retinoid is a prescription medication in the U.S., be cautious of the side affects before usage. Retinoid cleansers contain high levels of vitamin A, which help to clean out clogged pores and dissolve away grime. You can get a prescription for one from your doctor. Trademarked OTC retinoid-like products are ineffective.

After eating the chocolate, the participants’ blood cells produced more interleukin-1b, which is a marker of inflammation, when exposed to Propionibacterium acnes. Eating chocolate also increased production of another immune system factor called interleukin 10 after exposure to Staphylococcus aureus. Interleukin 10 is thought to lower the body’s defenses against microorganisms, so higher levels could allow bacteria to infect pimples and worsen them.

Acne encompasses a range of facial spots beyond just your latest breakout situation: black and whiteheads, pimples, pustules, cysts, and nodules, says Dr. Lee. A black or whitehead occurs when a pore is clogged with debris and oil. When those blockages build up, bacteria, specifically the P. acnes variety, can get involved. That triggers inflammation and infection, which causes painful red pimples.

Acne removal: Your dermatologist may perform a procedure called “drainage and extraction” to remove a large acne cyst. This procedure helps when the cyst does not respond to medicine. It also helps ease the pain and the chance that the cyst will leave a scar. If you absolutely have to get rid of a cyst quickly, your dermatologist may inject the cyst with medicine.

“A true acne scar is when your body takes that healing process one step further and is not able to actually form the collagen the normal way. All the enzymes that are in the area of the acne lesion eat away at things like collagen and elastin,” explained Dr. Bowe. “In the case that the inflammation is so great, the collagen and elastic tissue is not able to regenerate completely, or it does in a sort of disorganized or pathologic and not healthy fashion, and you’re left with a scar.”

Use topical acne medications. There are many over-the-counter medicines that are effective for managing acne. Look for products that contain benzoyl peroxide, salicyclic acid, alpha hydroxy acids, or sulfur.[2] These ingredients are found in gels, creams, toners, and cleansers. If you do not know which ingredient works best for your skin, start with benzoyl peroxide.[3] Try different ingredients to find what works best for your skin.

If you cross-compare the active ingredients in each medication with the results of your experiment, do you see any trends? Are there certain ingredients that seem to contribute significantly to preventing E. coli growth? If so, consider designing an additional experiment to test your new hypothesis.

Topical erythromycin and clindamycin are generally well-tolerated and have been shown to reduce inflammatory lesions by 46% to 70% in several randomized controlled trials.2 Monotherapy with topical antibiotics should not be used routinely beause P. acnes may become resistant within one month after daily treatment has begun.6 Some argue that this resistance is not relevant because the antibiotics (e.g., clindamycin, tetracyclines, erythromycin) also have intrinsic anti-inflammatory and antimicrobial effects.11 However, antibiotic-resistant Staphylococcus epidermidis and Staphylococcus aureus may also develop with monotherapy; resistance can be avoided when a topical antibiotic is combined with benzoyl peroxide.12

Various gels, lotions and creams are used to treat acne. Different preparations work in different ways. The following briefly describes the different types. However, always read the leaflet in the packet, because such things as how to apply it and precautions vary between different preparations. One general point is that you should apply topical treatments to all the affected area of skin and not just to each spot.

The relationship between diet and acne is unclear, as there is no high-quality evidence that establishes any definitive link between them.[51] High-glycemic-load diets have been found to have different degrees of effect on acne severity.[7][52][53] Multiple randomized controlled trials and nonrandomized studies have found a lower-glycemic-load diet to be effective in reducing acne.[52] There is weak observational evidence suggesting that dairy milk consumption is positively associated with a higher frequency and severity of acne.[50][51][52][54][55] Milk contains whey protein and hormones such as bovine IGF-1 and precursors of dihydrotestosterone.[52] These components are hypothesized to promote the effects of insulin and IGF-1 and thereby increase the production of androgen hormones, sebum, and promote the formation of comedones.[52] Effects from other potentially contributing dietary factors, such as consumption of chocolate or salt, are not supported by the evidence.[51][54] Chocolate does contain varying amounts of sugar, which can lead to a high glycemic load, and it can be made with or without milk. Benzoyl peroxide products are also effective against C acnes (formerly P acnes), and bacterial resistance to benzoyl peroxide has not been reported. [35] Benzoyl peroxide products are available over the counter and by prescription in a variety of topical forms, including soaps, washes, lotions, creams, and gels. Benzoyl peroxide products may be used once or twice a day. These agents may occasionally cause a true allergic contact dermatitis. More often, an irritant contact dermatitis develops, especially if used with tretinoin or when accompanied by aggressive washing methods. [35] If intensive erythema and pruritus develop, a patch test with benzoyl peroxide is indicated to rule out allergic contact dermatitis.

Pseudofolliculitis: This is sometimes called “razor bumps” or “razor rash.” When cut too close to the skin, growing hairs twist into the skin and produce tender bumps. This is a mechanical problem, and treatment involves shaving less (growing a beard, laser hair removal). Pseudofolliculitis can, of course, occur in patients who have acne, too.

Aspirin: crush one aspirin finely and make up to a smooth paste with water. Using a cotton swab, dab onto pimples and leave for thirty minutes. You will notice the mixture dries and cracks and is ready to remove. Wash off in warm water. Use this only once a day as it dries out your skin.

Be patient. It can take weeks to see results from any medication. Your acne may look worse before it gets better. Don’t be surprised if you get redness, burning, or dry skin from your acne medicines. If it’s serious, call your doctor.

Oral contraceptives are a very effective treatment for acne in many women, but you have to give them time to work, says Bethanee Schlosser, MD, assistant professor and director of the women’s skin health program in the department of dermatology at Northwestern University’s Feinberg School of Medicine. “I ask patients to give the pills at least three months of use before judging their impact. As common as acne is, it remains a skin disease that is imperfectly understood. Some forms are incurable, some can be cured, and most simply go away on their own. Exactly what can cause it to develop is often a mystery. What are known are those things that most often can contribute to an outbreak of the disease. Most explanations for the appearance of the disease center on clogging of the skin pores. The clogging of a pore usually involves dead skin cells. Normally, these dead cells will rise to the top of the pore where they are then shed. What can sometimes happen however is that sebum, the oil that lubricates the skin, can cause these dead skin cells to stick together and become trapped within the pore.

Ask your doctor before using other medication on the skin, especially anything containing sulfur, resorcinol, or salicylic acid, as the combination may be too irritating. Abrasive soaps and cleansers should be used with caution for the same reason.

Discuss the severity of the patient’s acne with them. Many people will perceive their acne to be worse than the clinical appearance suggests, and this should be taken into account in the treatment approach. The converse may also be true, e.g. a parent may perceive the acne to be worse than the patient believes or the physical findings suggest.

I certainly didn’t need oral antibiotics (which can cause long-term gut complications, immune problems, and yeast overgrowth), or Accutane (which can cause liver damage and increase the risk of depression and suicide), or oral contraceptives (which I would have been offered had I been a woman).

Thrush (Candida albicans infection) affects 5% of treated women and most often affects the vagina. Thrush can also affect the oral mucosa or body folds (intertrigo), particularly in diabetics or in obesity. Thrush is less likely with erythromycin than with tetracycline.
