Photodynamic therapy is a new treatment for acne. It begins with light microdermabrasion. This is used to remove dead skin cells on the face’s surface. Then, an acid is put on the skin for 30 to 60 minutes. After this, the acid is taken off. Lastly, the skin is treated with a laser. This treatment is still being researched, but seems to give positive long-term results.

Isotretinoin affects all causative mechanisms of acne — it changes abnormal follicular keratinization, decreases sebum production by 70%, decreases P. acnes colonization and is anti-inflammatory.38 Indications for isotretinoin include scarring disease, severe nodulocystic acne and less than 50% improvement with oral antibiotics or hormonal therapies after four months.38 Isotretinoin therapy must be monitored carefully because adverse effects include potent teratogenicity, hypertriglyceridemia and pancreatitis, hepatoxicity, blood dyscrasias, hyperostosis, premature epiphyseal closure and night blindness. An association with severe skin reactions, such as erythema multiforme, Stevens–Johnson syndrome and toxic epidermal necrolysis, has been reported.39 Although a causal relationship has not been shown, patients must be warned about depression, suicidal thoughts and psychosis, and monitored closely.40
Some hormonal therapies may be effective in the treatment of acne vulgaris. Estrogen can be used to decrease sebum production. Additionally, it reduces ovarian production of androgens by suppressing gonadotrophin release.  Oral contraceptives also increase hepatic synthesis of sex hormone–binding globulin, resulting in an overall decrease in circulating free testosterone. Combination birth control pills have shown efficacy in the treatment of acne vulgaris.

Adult acne is caused by sebum, an oily substance produced by the skin’s sebaceous glands. Sebum clogs pores which attracts bacteria and become inflamed. For some adults breakouts are as a result of hypersensitivity or overproduction of androgens, the male hormone.

I certainly didn’t need oral antibiotics (which can cause long-term gut complications, immune problems, and yeast overgrowth), or Accutane (which can cause liver damage and increase the risk of depression and suicide), or oral contraceptives (which I would have been offered had I been a woman).

The adverse effects of benzoyl peroxide include skin irritation, dryness and redness. Direct contact with eyes or other mucous membranes will cause severe irritation. Most of the adverse effects can be minimised by reducing the time the product is on the skin before being washed off, or by reducing the concentration used. Advise the patient that if their skin peels or becomes very dry, an oil-free moisturiser can be used. Benzoyl peroxide will bleach linen, clothing and towels.

Antiandrogens such as cyproterone acetate and spironolactone have been used successfully to treat acne, especially in women with signs of excessive androgen production such as increased hairiness or skin production of sebum, or baldness. Spironolactone is an effective treatment for acne in adult women, but unlike combination oral contraceptives, is not approved by the United States Food and Drug Administration for this purpose. The drug is primarily used as an aldosterone antagonist and is thought to be a useful acne treatment due to its ability to block the androgen receptor at higher doses. It may be used with or without an oral contraceptive. Hormonal therapies should not be used to treat acne during pregnancy or lactation as they have been associated with birth disorders such as hypospadias, and feminization of the male fetus or infant. Finasteride is likely an effective treatment for acne.
