For Women: Unless you have had your uterus and ovaries removed (total hysterectomy with oophorectomy) or have been in menopause for at least 12 months in a row, you are considered to be of child-bearing potential.
The emergence of antibiotic-resistant bacteria, other than C acnes (formerly P acnes), is a contentious debate. An early study by Miller et al found increased skin carriage of coagulase-negative staphylococci in not only acne patients with prolonged use of antibiotics, but also in their close contacts.  On the contrary, a study by Fanelli et al found that Staphylococcus aureus remained sensitive to tetracycline even after prolonged use of that antibiotic for acne.  This has significant ramifications when considering efforts to control the spread of methicillin-resistant S aureus (MRSA), because tetracycline group antibiotics are currently one of the primary options for outpatient treatment of MRSA infection.
Whiteheads are small, raised bumps on the surface of the skin, and they are another form of clogged pore (comedo) that can be a precursor to pimples. Similar to blackheads, whiteheads form when the hair follicle fills with sebum. Unlike blackheads, whiteheads haven’t pushed all the way through to the skin’s surface so they are considered closed off from air that turns the sebum dark or black.
For severe cystic acne, isotretinoin is the most effective therapy. This drug is the only drug that intervenes in all of the causes of acne. It can often even clear severe acne that hasn’t responded to other treatments. However, the product can have side effects. It can cause severe birth defects and must NEVER be taken by a woman who is pregnant or who is not using contraception. In addition, it shouldn’t be taken by a woman who is nursing. Some studies suggest its use has been associated with an increased risk of depression, suicide, and inflammatory bowel disease. Talk to your doctor about the potential risks of this drug.

Systemic antibiotics are a mainstay in the treatment of moderate-to-severe inflammatory acne vulgaris.  These agents have anti-inflammatory properties, and they are effective against C acnes (formerly P acnes). The tetracycline group of antibiotics is commonly prescribed for acne. The more lipophilic antibiotics, such as doxycycline and minocycline, are generally more effective than tetracycline. 
Sulfur is mildly antimicrobial antibicrobial, and it is used in acne products to prevent P. acnes growth. Sulfur also helps keep the surface of the skin dry of oils and actually helps peel off this layer of skin, which is already dead. Many acne-fighting medicines use sulfur, including Clearasil, the Proactiv Refining Mask, and Murad.
A tetracycline that might be worth looking at as a potentially effective oral antibiotic treatment for acne is doxycycline. Doxycycline is used in the treatment of inflammatory acne as well as other bacterial infections, which includes respiratory tract infections like pneumonia, dermatological and genito-urinary infections, anthrax exposure, Rocky Mountain spotted fever, typhus, cholera, brucellosis, and syphilis. The most commonly available topical retinoids are tretinoin, adapalene and tazarotene. A meta-analysis of five multicentre randomized investigator-blind trials involving 900 patients showed adapalene 0.1% gel to be as effective as, but less irritating than, tretinoin 0.025% gel.8 Different concentrations of retinoids affect tolerability. One commonly used approach is to start with the lowest concentration and increase as tolerated.

It is important to distinguish this form of shoulder acne from folliculitis, which occurs in hair follicles, especially after shaving the shoulders. It can also occur after use of a hot tub or by wearing a wetsuit. Folliculitis is more likely to be painful than acne and is caused by a different microorganism. Folliculitis causes raised swellings with a yellow center, and acne does not. Consulting a doctor is usually necessary for treatment of folliculitis.
One way to treat acne is to stop the proliferation of the bacteria. In this science fair project, you will test a variety of over-the-counter acne medications to determine which is the most effective at preventing bacterial proliferation. Although Propionibacterium acnes (P. acnes ) is the bacteria which most frequently contributes to acne, in this science fair project you’ll be using the bacterium Escherichia coli (E. coli). E. coli is safer and easier to work with. You’ll use the Kirby-Bauer disk-diffusion method to measure the effectiveness of each acne medication. With this technique, bacteria is uniformly spread across an agar plate, then sterile disks, each coated with a compound (in this case, medication) to be tested, are placed on the surface of the agar. The compounds diffuse out from the disk into the agar. The concentration of the compound is highest right next to the disk and gradually reduces as you move out in concentric circles from the disk. If a compound is effective against the bacteria at a certain concentration, no colonies will grow in the area of the plate where that compound is equal to or higher than the effective concentration. The areas without bacterial growth are called zones of inhibition and are characterized by a clear circle around the disk in an otherwise contiguous lawn of bacteria. The larger the zone of inhibition is, as measured by the diameter of the clear circle, the more effective the compound.
If you’re going to take matters into your own hands, at least pop responsibly. Wait until the pimple comes to the surface of skin (you’ll see the white, pus-filled head of the pimple). Popping a pimple when your skin is ready “will cause the most shallow trauma to skin and decrease the likelihood of permanent scarring,” says Dr. Lee. You’re also more likely to get everything out; if you don’t, the spot will just get inflamed again.
There are also several other ingredients you can use on skin to reduce inflammation, such as chamomile oil and aloe vera. These can especially be beneficial if your skin is irritated due to using products containing salicylic acid, benzoyl peroxide, or both.
Intralesional steroid injections have been found to be beneficial for large inflammatory lesions. Comedone removal does not affect the course of the disease, but it does improve the patient’s appearance.  Additionally, some patients may benefit from superficial peels that use glycolic or salicylic acid, although more research needs to be conducted to establish best practice for these modalities.
