First-line therapy is chosen based on the site and extent. Benzoyl peroxide kills P. acnes and mildly comedolytic ointments prevent the formation of comedones and blemishes. Once the patient begins using benzoyl peroxide, he or she can expect to see results in 5 days and should use lower-strength and water-based formulations if the skin is sensitive.
Pustules or Nodules: Sometimes, the tissues around the clogged pores get infected and cause inflammation. This inflamed portion is filled with pus, and it is hard. These are nothing but nodules or pustules.

One of the most annoying and embarrassing blemishes everyone has to do deal with at some point in time is the pimple. Out of all of the drawbacks to the drug, the biggest hurdle for me was the monthly blood test. I have low blood pressure that causes me to routinely pass out after having my blood drawn, and just the sight of blood makes me queasy.
These side effects contribute to why isotretinoin is often a last resort for curing severe acne. Extremely dry skin is one of the most common effects of the drug, but since it became FDA-approved to treat acne in 1982 it has also been linked to inflammatory bowel disease, depression, and increased rates of suicide. While studies have not proven that isotretinoin causes these conditions, they remain among the risks of taking the medication. Other serious but rare side effects include joint and muscle pain, eyesight trouble, and liver damage. Due to the drug’s impact on the liver, patients are advised not to drink and their liver enzymes are monitored throughout treatment. Much to my high-school friends’ delight, this made me the designated driver the majority of our senior year. If I had much more than a beer, it would show in my blood work and I was admonished by my doctor and my mom—something no teen wants to experience.
Salicylic acid 2% wash is moderately effective but less potent than a topical retinoid in acne therapy. Although it has been used for many years, well-designed trials of its safety and efficacy are lacking. The evidence for the use of topical zinc, resorcinol, sulfur and aluminum chloride is also either limited or negative.
The recommended starting dose of isotretinoin is 0.5 mg/kg/day. Daily doses can be titrated up or down, between 0.1 and 1 mg/kg/day, depending on response to treatment and presence of adverse effects. The total dose of isotretinoin over the course of treatment should be between 120 – 150 mg/kg of body weight.14 The length of treatment is dictated by the daily dose. Courses usually last between three to four months but may continue for one year or longer. Total doses of >150 mg/kg are with increased adverse effects.16 The dose and duration of the course may be adjusted on an individual basis; some patients will wish to reduce the dose or stop isotretinoin when the active acne has settled.
Honestly, I’ve only had almost-perfect skin when I was using a harsh prescription cream (would never do that again), and at times when I have been overall healthy (mind, diet, lifestyle habits). Meaning, I have never noticed a significant difference related to products or food. I went on an elimination diet a few years ago, and was eating so cleanly– no processed foods whatsoever, no gluten, no refined sugars, no meat/dairy. This lasted 3 weeks, and I did not notice much of a difference with my face. Obviously everyone is different, and I’ve realized my main problem is stress. I mean, it could be a lot worse if I ate horribly (I am vegan– not that I noticed much of a difference when I changed my diet– not that it was that bad before), but unfortunately I am one of those people that can’t get an easy fix. Fixing your state of mind, and how you respond to stress, is probably the hardest thing to do. But I’m working on it.
