Most cases of acne fit within one of three main categories, says Macrene Alexiades-Armenakas, MD, PhD, an assistant clinical professor of dermatology at Yale School of Medicine and a specialist in laser therapy for acne, who practices in New York City:

Isotretinoin has been shown to affect all four pathogenic processes involved in acne formation and results in sebaceous gland apoptosis (cell death) and altered gene expression.16–18 Isotretinoin is highly effective for clearing acne and reducing recurrence. A single course of isotretinoin will result in significant improvement or complete remission of acne in nearly all patients. Long-term efficacy depends on individual patient factors and the duration and dose of treatment.18 If isotretinoin is ineffective, investigation of a potential endocrine cause for the acne, such as polycystic ovary syndrome, should be considered.19
As far as pimples go, those deep, humungous cystic-acne ones are probably the most traumatic. Aside from how much surface area those suckers tend to cover, the pain they cause makes them impossible to ignore. 3. Drink more water. Maybe your grandma swears by drinking 2 liters of water a day, or maybe you’ve just heard those red-carpet interviews where celebrities discuss the importance of hydration in their skin care routine. Wherever you heard it, believe it. Skin cells are made primarily of water, and the better hydrated your body is, the healthier your skin will be. This, of course, is not a replacement for a topical moisturizer — water reaches our other organs first, therefore applying an effective moisturizer is the most direct way to keep your skin hydrated.

Renée mentions that many of her clients with acne-prone skin are paranoid about using moisturizer, thinking that it will immediately clog up their pores and not let the skin “breathe.” “For starters, wearing moisturizer does not cause breakouts,” Renée clarifies. “Acne happens when the cells that line the inner pores fail to fall off properly, clogging the pore. This happens whether you moisturize or not. Depending on acne severity, Benzoyl Peroxide is used in varied concentrations: 2.5%, 5%, and 10%. It’s not recommended that Benzoyl Peroxide be used in conjunction with acne medication to avoid excessive acne caused by dry skin.

Cortisone is a quick fix for acne emergencies. We mean got a big board meeting tomorrow kind of quick. Go into the dermatologist’s office for a shot of this corticosteroid, and acne will disappear in 24 to 48 hours. The treatment works to curb inflammation, which makes it best for cystic breakouts and can be really good at combatting hormonal flare-ups. If done incorrectly, a cortisone shot can leave a small depression in the skin that lasts about eight weeks. “It’s a rare side effect that happens if dosage of cortisone is too high,” explains Linkner. “You want to go to someone who knows what they’re doing.”
“Stress leads to increased cortisol levels,” says dermatologist Ava Shamban. Like testosterone, cortisol is a hormone that can increase oiliness as well as inflammation. Not a good combo when you want to avoid being red in the face. Once upon a time, our bodies produced cortisol only when we were in danger, but nowadays, it’s much more likely to flood your system in response to a looming deadline than in response to a predator in the underbrush. So when your acne starts acting up, take a minute to figure out whether there’s anything you’re freaking out over, then unload all of that skin-wrecking stress.

Antibiotics taken by mouth for relatively extended periods can be very effective in controlling acne. Although the development of resistant bacteria is a theoretical concern as a result of protracted courses of antibiotics, this does not seem to occur commonly in acne patients. Still, the goal of treatment regimens is to graduate patients of systemic antibiotics as soon as feasible. As with any systemic medication, they can be associated with more side effects than if applied to the skin and may interact with other drugs. Sensitivity to the sun can result in a “bad sunburn” in some people who take antibiotics in the tetracycline family.
Acne is the common cause of spots. Most people with acne are aged between 12 and 25 but some older and younger people are affected. Boys are more commonly affected than girls. Acne usually affects the face but may also affect the back, neck and chest. The severity can range from mild to severe. About 8 in 10 teenagers develop some degree of acne. Often it is mild. The most popular over-the-counter products, such as Proactiv, contain benzoyl peroxide but at lower concentrations than most prescription-strength products. Proactiv, a system of cleansing products in which benzoyl peroxide 2.5% is the active ingredient, is claimed to enhance compliance by providing a cosmetically elegant product that also minimizes irritation. The makers of Proactiv also market Gentle Formula, which replaces benzoyl peroxide with salicylic acid for people with allergy or intolerance. There have been few studies assessing the efficacy of the Proactiv system. In one open-label study of 23 patients with mild to moderate acne, inflammatory lesions were reduced by 39% in patients using a combination of butenifine (an allylamine) and benzoyl peroxide compared with 34% in those using Proactiv.18
