Patients with mild acne can be treated with topical therapies; however, those with moderate to severe acne will require systemic therapy. Oral antibiotic treatment, hormonal therapies and isotreretinoin are the mainstay systemic therapies for acne.
Grade II: This acne grade refers to moderate acne that features a higher saturation of blackheads and whiteheads along with a higher number of pimples. Breakouts are more frequent than Grade I acne, and there may be development of papules and pustules.

Many cases of inflammatory acne are “hormonal” in nature — that is, they occur in teenage girls and women, and are aggravated by hormonal fluctuations like those that occur during the menstrual cycle. For these women, dermatologists often choose to prescribe either oral contraceptive pills or another medication called spironolactone.
Topical treatments on their own may not be enough to give you clear skin, especially in those with complicated, inflammatory cystic acne. There are several acne medication options approved for use by the FDA, but which one is best for you is a question for your dermatologist and/or general practitioner. Baldwin says if you have insurance and you have acne, a prescription may be the best step because “it makes no sense to try to handle the condition yourself or to use over the counter products that are always less effective than prescriptions meds.” Here are a few of the acne medications you’ll want to ask about:
But why does isotretinoin work so well? “We don’t really know,” said Joslyn Kirby, professor of dermatology at Penn State College of Medicine. “That’s part of the research that people at our group at Penn State are doing is looking into some of the changes in the oil glands in the skin called the sebaceous glands, and what is happening to the cells that might explain … this sustained improvement.”
Some pores become blocked (plugged). This is due to the skin at the top of the pores becoming thicker, combined with dead skin cells that are shed into the pores. You can see the plugs that block the top of the pores as tiny spots known as blackheads and whiteheads (comedones). Note: the black of the blackheads is due to skin pigment and is not dirt as some people think. In many cases, acne does not progress beyond this mild stage.

For individuals suffering from the cystic form of acne, a controlled clinical trial has found that Guggul supplements (also known as guggulsterone) outperformed 500 milligrams of tetracycline by a small margin. (16) In the study, 25 milligrams of guggulsterone taken twice daily for three months resulted in the reduction of acne, but more importantly, 50 percent fewer participants had acne relapses. Researchers noted that patients with oily skin responded remarkably better to guggul than others in the study.
Clindamycin suppresses the formation of bacterial proteins, resulting in fewer acne-causing bacteria. Anyone with a history of ulcerative colitis or gastroenteritis should avoid using clindamycin orally. Side effects of clindamycin include peeling skin, rash, itching, abdominal pain, nausea, vomiting, diarrhea, constipation and inflammation of the esophagus.
Some hormonal therapies may be effective in the treatment of acne vulgaris. Estrogen can be used to decrease sebum production. Additionally, it reduces ovarian production of androgens by suppressing gonadotrophin release.  Oral contraceptives also increase hepatic synthesis of sex hormone–binding globulin, resulting in an overall decrease in circulating free testosterone. Combination birth control pills have shown efficacy in the treatment of acne vulgaris. 
What is it: Made under the brand name “Azelex.” Azelex is a cream containing 20% azelaic acid, a naturally occuring acid found in whole grain cereals and animal products. It is normally applied twice daily.1Learn more from the U.S. National Library of Medicine.
Use artificial tears. Reduce use of contact lenses if necessary. Use sunglasses where needed. If artificial tears fail to help or if discomfort is severe, temporary cessation or reduction in dose of isotretinoin may be required.
A female and, if of reproductive age, pregnancy has been excluded prior to initiation of isotretinoin and that the patient understands the risk of teratogenicity and understands that she must not become pregnant during the course of treatment or one month after, or;
Before prescribing isotretinoin, dermatologists ask questions about the patient’s physical and mental health. They ask what medicines, vitamins and herbal supplements the patient takes. If everything looks okay, the patient must get a few medical tests. Everyone needs a blood test. Women who can get pregnant must have pregnancy tests. Women need 2 negative pregnancy tests before isotretinoin can be prescribed.
Accutane is especially good for cystic acne in women and body acne in men. “Oral vitamin A basically shuts down your sebaceous glands. If you suppress [them] for a long enough period, you can cure someone of their acne, and about 50 percent do hit that cure rate,” says Linkner. A course of Accutane can take about six to nine months. Sometimes patients need to repeat the course at a higher dosage in order to truly eliminate acne.
Cysts or nodules, which are severe pimples are infected and painful. They can form within deeper layers of the skin, become very swollen or tender, and take longer to heal then papules and pustules.
