The increase in melanin leads to some pigment transfer to the surrounding skin cells (also called keratinocytes which are the cells found in the epidermal layer of the skin and are responsible for making keratin.)

The diagnosis of acne vulgaris is primarily clinical.4 History and physical examination can help determine if there is an underlying cause of the acne, such as an exacerbating medication or endocrinologic abnormality causing hyperandrogenism (e.g., polycystic ovarian syndrome). Other dermatologic manifestations of androgen excess include seborrhea, hirsutism and androgenetic alopecia. Endocrinologic testing is not ordered routinely for women with regular menstrual cycles.2,3 Older women, especially those with new-onset acne and other signs of androgen excess (e.g., hirsutism, androgenic alopecia, menstrual irregularities, infertility), should be tested for androgen excess with measurements of total and free serum testosterone, dihydroepiandrosterone, and luteinizing and follicle-stimulating hormone levels.5 Pelvic ultrasonography may show the presence of polycystic ovaries.5 In prepubertal children with acne, signs of hyperandrogenism include early-onset accelerated growth, pubic or axillary hair, body odour, genital maturation and advanced bone age.
Recurrent acne may be treated with a further course of isotretinoin, and in some cases long-term, low-dose treatment is appropriate, however, discussion with a Dermatologist is recommended if this is being considered.18
While sunscreen won’t prevent the condition from forming, it can make a significant difference in preventing bumps from worsening and looking more obvious. The sun’s damaging rays are known to aggravate keratosis pilaris. Wear sunscreen of at least SPF 30 every day to encourage smoother skin.
If you love a good cheese plate, this one might come as bad news. Some studies show that cutting back on dairy can improve acne. “Milk naturally contains a great number of hormones, many of which are androgens — there is no such thing as hormone-free milk,” says William Danby, an assistant professor of dermatology at the Geisel School of Medicine at Dartmouth who has published many studies about diet and skin. Danby advises acne patients to cut down on dairy and eliminate straight-up milk altogether. You also might want to start avoiding processed foods. A recent study found a significant correlation between acne and foods like pasta, bread, and white rice. One reason for this might be that your body quickly converts these high-glycemic-index foods into simple sugars, which triggers a flood of insulin that can magnify the effect androgens have on acne.

Dermatologists take these reports seriously. That’s why dermatologists are conducting research studies to find out whether isotretinoin is directly responsible. Proving or disproving this is not as easy as it sounds. One reason is research shows that people who have severe acne often become depressed. This is true for people who have never taken isotretinoin.

Why does a pimple always seem to show up when your life already feels kinda crazy? “High stress levels lead to the release of a hormone known as cortisol,” says Dr. Goldenberg. “High levels of cortisol will produce more acne.”
There are several factors that lead to the development of acne. These include inflammation, too much production of a skin oil called sebum, presence of bacteria, and the clogging of skin pores (follicles) with debris.3

Both doctors recommend the use of retinoid skin products for both kinds of acne marks. These creams brighten and heal the skin. For an over-the-counter solution, Dr. Bowe suggests RoC Retinol Correxion Instant Facial Smoother, and for prescription remedies she usually puts her patients on Differin, Tazorac or Retin-A Micro. Dr. MacGregor advises patients to use Elure products, which utilize an enzyme called melanonzyme to even out your skin tone.
Sure, we’ve all heard that toothpaste or rubbing alcohol can help dry out a zit, but many DIY treatments aren’t solutions for how to treat acne. In fact, applying toothpaste or rubbing alcohol are more likely to cause irritation and dryness than treat the actual pimple. Instead, stick with topical over-the-counter and prescription spot treatments with salicylic acid or benzoyl peroxide that are specially made to target pimples, says Dr. Hammerman. For an all-natural spot treatment, she suggests dabbing tea tree oil on the area a few times a day with a cotton ball.
Even though isotretinoin does not remain in the body after therapy is stopped, improvement is often long-lasting. It is safe to take two or three courses of the drug if unresponsive acne makes a comeback. It is, however, best to wait at least several months and to try other methods before using isotretinoin again.

Blackheads are, essentially, open comedones. “Comedone refers to plugging of the follicular opening,” explains NYC dermatologist Elizabeth Hale, M.D., referring to hair follicles that technically cover your entire face and body (hi, peach fuzz). “Every hair follicle appears in a sebaceous gland.” So a blackhead is the mixture of dead cells, bacteria, and grime that builds up and hardens in the follicular opening—but it’s open to the world, which is why blackheads are so easy (read: tempting) to push out.
