P. acnes also provokes skin inflammation by altering the fatty composition of oily sebum. Oxidation of the lipid squalene by P. acnes is of particular importance. Squalene oxidation activates NF-κB (a protein complex) and consequently increases IL-1α levels. Additionally, squalene oxidation leads to increased activity of the 5-lipoxygenase enzyme responsible for conversion of arachidonic acid to leukotriene B4 (LTB4). LTB4 promotes skin inflammation by acting on the peroxisome proliferator-activated receptor alpha (PPARα) protein. PPARα increases activity of activator protein 1 (AP-1) and NF-κB, thereby leading to the recruitment of inflammatory T cells. The inflammatory properties of P. acnes can be further explained by the bacterium’s ability to convert sebum triglycerides to pro-inflammatory free fatty acids via secretion of the enzyme lipase. These free fatty acids spur production of cathelicidin, HBD1, and HBD2, thus leading to further inflammation.
“People tend to groom the lesions. They examine them very closely several times a day and start imagining that there is something they can stick in the lesion or extract from the lesion. So they pick and the lesion gets worse,” Reed says.
A dermatologist can prescribe oral antibiotics as a quick fix for acne, but it’s not typically the first choice. While you might see immediate results, as soon as you get off of the antibiotics the acne will return. And Linkner warns, “While you’re on them, [you can get] yeast infections, nausea, upset stomach.”
Not surprisingly, the answer to this question is yes. Vodka works as an astringent because of its alcohol content. When applied on the face, it cleanses it and kills the bacteria. It is also said to shrink pores. Unfortunately not many people have tried this remedy. However, there are a few who vouch for it. If you do plan to try this rather offbeat home remedy, be prepared as vodka might sting on the sensitive, inflamed skin.

To make your own exfoliate mix two tablespoons of the dry ingredient of choice with 1–2 tablespoons of the base of choice. Rub into skin in a circular motion. Start at the forehead and work your way down, paying particular attention to problem areas. Remove with a damp cloth, and rinse well.
You have a good shot at getting rid of milder, non-hormonal breakouts without a prescription. Apply a treatment with 2 percent salicylic acid, which gently exfoliates, such as Benzac Acne Solutions Intensive Spot Treatment, twice daily all over your face, or one with benzoyl peroxide, which fights the bacteria that cause zits. (We like Neutrogena Rapid Clear Stubborn Acne Spot Gel, which has the highest concentration available without a prescription.) You can also switch to a daily cleanser with at least 0.5 percent salicylic acid, such as Philosophy’s Clear Days Ahead Oil-Free Salicylic Acid Acne Treatment Cleanser. Then, every time you feel the first tingles of a new zit forming, hit the spot with a dab of antibacterial spot treatment (like that Benzac one we mentioned). And be patient: It can take six to eight weeks before you notice a real change in your skin.
Sugar: You may have already suspected that sugar is related to breakouts. Some studies now suggest that there may be a link. This doesn’t mean that if you eat a cookie you’re going to get a pimple. It comes down to how much sugar you’re eating in a day—particularly at any one time. If you consume a soda and a candy bar, for example, you’re likely spiking your blood sugar levels, and you could break out hours later. If you suspect sugar could be a culprit for you, try to cut back even by one sugary drink a day to notice a difference.

Glycolic acid is an alpha hydroxy acid that exfoliates the skin, helping to get rid of the dead skin cells that can clog pores. Just like salicylic acid, you can find glycolic in washes, peels, moisturizers, and serums at your local beauty store or drugstore.
In women, acne can be improved with the use of any combined birth control pill. These decrease the production of androgen hormones by the ovaries, resulting in lower skin production of sebum, and consequently reduce acne severity. Combinations containing third- or fourth-generation progestins such as desogestrel, drospirenone, or norgestimate are preferred for women with acne due to their stronger antiandrogenic effects. A 2014 review found that oral antibiotics appear to be somewhat more effective than birth control pills at decreasing the number of inflammatory acne lesions at three months. However, the two therapies are approximately equal in efficacy at six months for decreasing the number of inflammatory, non-inflammatory, and total acne lesions. The authors of the analysis suggested that birth control pills may be a preferred first-line acne treatment, over oral antibiotics, in certain women due to similar efficacy at six months and a lack of associated antibiotic resistance.

If you have body acne, taking a shower as soon as possible after working out is also key. It turns out that standing around in tight, sweaty workout clothes puts you at greatest risk for body acne and rashes. “The whole idea is that the bacteria that live on the skin can get trapped in the hair follicles and cause inflammation,” says dermatologist Dr. Elizabeth Hale. “The more you work out in the heat the more likely this is.” So, make sure to shower (or, at the very least, change out of your sweaty clothes) right after working out.
