What it is: Originally under the brand name “Accutane” but now available only in generic form, isotretinoin is a derivative of vitamin A taken in pill form for 15-20 weeks. Subsequent case-control and cohort studies had conflicting results, with some suggesting no association between isotretinoin and IBD and others suggesting an association between isotretinoin and ulcerative colitis but not Crohn disease [57, 58, 59, 60] Finally, a 2016 large meta-analysis, indexing more than 9 million cases to reduce effects of selection bias and confounding factors, showed isotretinoin is not associated with an increased risk of Crohn disease or ulcerative colitis. Although treatment with isotretinoin has numerous potential minor side effects in patients of all ages, an uncommon complication in young patients is premature epiphyseal closure.38 This generally occurs when isotretinoin is administered in high doses, thus limiting long-term therapy.

Comedonal acne. This is the kind of mild acne that involves blackheads and whiteheads. It forms because a component of skin oil called sebum, along with old skin cells, block the pores of the skin. Comedonal acne appears most often on the forehead, nose, and chin.

If this option is not successful, then the patient is started on isotretinoin (Claravis, Amnesteem, Absorica, Myorisan, Zenatane). Patients may benefit from a lower dosage initially, followed by an increase to the standard 1 mg/kg dose after about a month. Patients should remain on this dosage for at least six months to maintain a durable improvement. Rarely, a short course of oral steroids (prednisone) may be necessary to suppress the inflammation during the initiation of therapy.
What it does: It is thought to help unplug follicles and keep pores clear. It also increases the speed at which surface skin cells are replaced and helps reduce inflammation. A study done on Retin-A Micro, a particular brand of tretinoin, showed a 32-45% reduction in total “lesions.”2
Topical treatments on their own may not be enough to give you clear skin, especially in those with complicated, inflammatory cystic acne. The way tretinoin works is by locking into receptors on the surface of a special class of skin cells known as keratinocytes. These are the cells that create baby skin cells that mature as they travel from the basal layers of skin to the surface. Increasing the production of these cells causes the skin to stretch and grow around pores, opening them without any need of deep-cleaning cleansers or, in the case of nodules and cysts, lancing or surgery.
