Acne vulgaris is one of the most common skin disorders in the UK and accounts for more than 3.5 million visits to the GP every year. It affects 80% of people at some point in their lives and is most prevalent in adolescents and young adults between the ages 11-30 years. There is also evidence to suggest that the spectrum of onset of acne is changing, with cases emerging both earlier and later than what has been noted in the past. Acne can affect adults and may also develop for the first time after the age of 25 years.
Kids who have serious acne can get help from their doctor or a dermatologist (a doctor who treats skin problems). Doctors can prescribe stronger medicine than you can buy at the store. Acne prescriptions can include stronger creams that prevent pimples from forming or antibiotics that decrease swelling (puffiness) and kill bacteria that cause pimples.
Spironolactone may also be used in the treatment of acne vulgaris.  Spironolactone binds the androgen receptor and reduces androgen production. Adverse effects include dizziness, breast tenderness, and dysmenorrhea.  Dysmenorrhea may be lessened by coadministration with an oral contraceptive. In two 2017 retrospective studies, spironolactone has been shown to be effective in reducing inflammatory lesions in multiple areas of the body with minimal adverse effects. [46, 47] Currently, more high-powered randomized controlled trials are needed to assess the efficacy of spironolactone monotherapy in treating acne, but spironolactone should be considered in recalcitrant acne, in women who do not tolerate or have contraindications to oral contraceptives, and to prevent antibiotic resistance.  A 2015 large retrospective study of healthy women aged 18-45 years confirms potassium monitoring is unnecessary for these patients while taking spironolactone.  Pregnancy must be avoided while taking spironolactone because of the risk of feminization of the male fetus, and spironolactone is not recommended for males because of the potential for gynecomastia. [31, 46] While a black box warning regarding possible cancer risk was placed on spironolactone many years ago after rats fed high doses of the medication developed both benign and malignant tumors, several large retrospective and longitudinal studies have found no association with cancer. Honestly, I’ve only had almost-perfect skin when I was using a harsh prescription cream (would never do that again), and at times when I have been overall healthy (mind, diet, lifestyle habits). Meaning, I have never noticed a significant difference related to products or food. I went on an elimination diet a few years ago, and was eating so cleanly– no processed foods whatsoever, no gluten, no refined sugars, no meat/dairy. This lasted 3 weeks, and I did not notice much of a difference with my face. Obviously everyone is different, and I’ve realized my main problem is stress. I mean, it could be a lot worse if I ate horribly (I am vegan– not that I noticed much of a difference when I changed my diet– not that it was that bad before), but unfortunately I am one of those people that can’t get an easy fix. Fixing your state of mind, and how you respond to stress, is probably the hardest thing to do. But I’m working on it.
