Trapped sebum is ideal for a germ (bacterium) called Propionibacterium acnes to live and multiply. Small numbers of this bacterium normally live on the skin and do no harm. However, if a large number develop in the trapped sebum, the immune system may react and cause inflammation. If inflammation develops, it causes the surrounding skin to become red and the spots become larger and filled with pus (pustules). In some cases the pustules become even larger and form into small nodules and cysts.
The AAD believes that this medicine can be appropriate treatment for severe acne, as long as prescribing doctors educate their patients about the potential risks. Patient safety is dermatologists’ primary concern. Dermatologists monitor their patients for IBD, depression, and other possible side effects.
Topical clindamycin and erythromycin are antibiotics that are also anti-inflammatory drugs and are effective against a number of bacteria. They should always be combined with benzoyl peroxide or a topical retinoid and applied directly to the skin. Oral erythromycin is also available, but you may become resistant to its effects, limiting its usefulness.

In severe cases of acne, a visit to dermatologist or skin specialist is required to get rid of acne. Prescribed medication is needed for severe acne. Medication can be in the form of oral antibiotics, cream or lotion.
This type of oral medication is only useful and appropriate for acne cases in adult women. Spironolactone is not designed for acne treatment, but has proven to clear the skin after long-term use. This acne medicine is actually meant to treat conditions that result from too much aldosterone by preventing the body from absorbing too much salt and regulating potassium levels.

That’s good news because the British Medical Journal reports that acne affects more than 80 percent of teenagers and continues in adult life in 3 percent of men and 12 percent of women. In fact, it seems to be increasing, which could be due autoimmune disease, leaky gut syndrome or allergies, for example. Hormonal factors may play a role in breakouts as well. (1)

Contrary to popular belief, you don’t just get acne scars from picking at your pimples. According to Dr. Whitney Bowe, there are acne lesions that are prone to scarring — even if you leave them completely alone.
Limit washing your face (and body if you have body acne) to two times per day or after heavy sweating. Over-washing and over-drying your skin will actually stimulate sebaceous glands (it is a skin self-protection mechanism) to produce more oil.
The best way to get rid of acne is to find out what’s causing it and address that underlying issue. This is especially true for cystic acne sufferers, who may find that even the best acne products available over the counter aren’t enough on their own to pave the way for clear skin. While bacteria (P. acnes) and inflammation are the two main culprits, acne is also influenced by hormones, Dr. Bowe explains. “When a woman’s androgen receptors are particularly sensitive, these hormones can trigger excess oil production and cause skin cells to become sticky, leading to clogged pores and breakouts.”
Sulfur: This has been used for centuries to treat acne, psoriasis, and eczema. Sulfur helps by breaking down blackheads and whiteheads. It is unclear exactly how it works, but elemental sulfur slowly oxidizes to become sulfurous acid, which has antibacterial properties.
Some patients only require one course of oral isotretinoin for complete acne remission, while others require additional courses of isotretinoin therapy. A study found 38% of the patients had no acne during 3-year follow up, and, among the remaining patients, 17% were controlled with further topical therapy, 25% with topical and oral antibiotics, and 20% with second course of isotretinoin.  Relapse is more likely in younger or female patients. 
Do not donate blood while taking Accutane and for at least 30 days after you stop taking it. Donated blood that is later given to pregnant woman could lead to birth defects in her baby if the blood contains any level of Accutane.
If you have acne that’s not responding to self-care and over-the-counter treatments, make an appointment with your doctor. Early, effective treatment of acne reduces the risk of scarring and of lasting damage to your self-esteem. After an initial examination, your doctor may refer you to a specialist in the diagnosis and treatment of skin conditions (dermatologist).

Dermabrasion is an effective therapeutic procedure for reducing the appearance of superficial atrophic scars of the boxcar and rolling varieties. Ice-pick scars do not respond well to treatment with dermabrasion due to their depth. The procedure is painful and has many potential side effects such as skin sensitivity to sunlight, redness, and decreased pigmentation of the skin. Dermabrasion has fallen out of favor with the introduction of laser resurfacing. Unlike dermabrasion, there is no evidence that microdermabrasion is an effective treatment for acne.
But why does isotretinoin work so well? “We don’t really know,” said Joslyn Kirby, professor of dermatology at Penn State College of Medicine. “That’s part of the research that people at our group at Penn State are doing is looking into some of the changes in the oil glands in the skin called the sebaceous glands, and what is happening to the cells that might explain … this sustained improvement.”

Do not squeeze or prick the pimples or blackheads. This will increase sebum production and also let the bacteria spread on the surface of the skin, leading to more pimples. It can also lead to permanent pigmentation.
