Before seeing a physician, patients frequently use over-the-counter therapies for their acne. Such treatments may be more accessible, cosmetically elegant, less expensive and less irritating than prescription therapies.17 However, there is insufficient evidence to evaluate and compare the efficacy of over-the-counter formulations.2
Evening primrose oil maintains optimum levels of skin hydration, elasticity, and smoothness with its omega-6 fatty acids (15). It is also an antibacterial agent (16). This home remedy works well for treating adult, hormonal, and cystic acne.
Patients should be advised to wash their face gently with warm water and mild soap or cleanser, twice daily. An un-medicated face-wash is sufficient, although products containing benzoyl peroxide or salicylic acid can be effective. Rough scrubbing should be avoided as it causes follicular rupture, increasing the inflammatory response. Patients with sensitive skin, e.g. atopic dermatitis, should avoid soap, and anti-acne cleansers may cause irritation and contact dermatitis.
After eating the chocolate, the participants’ blood cells produced more interleukin-1b, which is a marker of inflammation, when exposed to Propionibacterium acnes. Eating chocolate also increased production of another immune system factor called interleukin 10 after exposure to Staphylococcus aureus. Interleukin 10 is thought to lower the body’s defenses against microorganisms, so higher levels could allow bacteria to infect pimples and worsen them.

The oral antiandrogen spironolactone can be added if oral contraceptives are not effective.3 Spironolactone is a 5α-reductase inhibitor when administered at higher doses.35 Spironolactone, used alone or as an adjunct at doses of 50–200 mg/d, has been shown to be effective in improving acne, but this is based on limited evidence.2 However, patients should be warned about possible side effects, including hyperkalemia, menstrual irregularities and feminization of a male fetus. Antiandrogen therapy alone may be successful, but in less than half of women;36 the acne may recur when it is discontinued. Combination therapy with topical agents or oral antibiotics provides substantially more benefit.37
Apply the appropriate amount of topical acne medications (enough for a very thin layer, generally a pea-sized amount for the face) to the skin. Using more medication than is recommended will not produce better results, but may cause more irritation or dryness.

Oral antibiotics are designed to kill the acne-causing bacteria found within your skin pores. The most common oral antibiotics prescribed for acne treatment are Tetracyclines, which include Minocycline and Doxycycline, and Erthromycin. Like their topical antibiotics counterparts, bacterial resistance may result in ineffectiveness of this treatment regimen so overuse is to be avoided.
Topical or systemic antibiotics should always be used in combination with benzoyl peroxide, a topical retinoid or azelaic acid. In women, they may also be used in combination with antiandrogen therapy or oral contraceptive pill.

Neem oil and neem powder are powerful natural treatments for pimples. They have powerful antibacterial properties. You can get them to buy online or at the local organic or natural food store in your area. Rinse your face with warm water to open up the pores of your skin. Most patients with mild acne can be treated with topical treatment (gels, solutions and lotions) that can be obtained over-the-counter in New Zealand without prescription. Hair follicles are the tiny structures that grow hair in the scalp. Sebaceous glands produce sebum. On areas where acne develops, sebaceous glands surround the hair follicles. The combination of the sebaceous glands and the hair follicles is the “pilosebaceous unit,” where acne pimples and cysts develop. Sebum moisturizes hair and skin. East Indian sandalwood oil is a naturally occurring anti-inflammatory agent, which is new to the skin care scene. It soothes the skin and fights off inflammation that often accompanies breakouts.”
I have literally tried every product out there to heal my skin. I get painful cysts that are impossible to cover or hide with makeup. People with nodules and cysts are usually considered to have more extreme forms of acne. These lesions are more common in men than in women. Cystic acne requires special treatment, and will be discussed later in this article.

Oral antibiotics have been used to treat acne for many years. Like topical antibiotics, oral antibiotics work by reducing Propionibacteria acnes (the bacteria responsible for acne breakouts). They also help decrease inflammation of the skin.
Isotretinoin affects all causative mechanisms of acne — it changes abnormal follicular keratinization, decreases sebum production by 70%, decreases P. acnes colonization and is anti-inflammatory.38 Indications for isotretinoin include scarring disease, severe nodulocystic acne and less than 50% improvement with oral antibiotics or hormonal therapies after four months.38 Isotretinoin therapy must be monitored carefully because adverse effects include potent teratogenicity, hypertriglyceridemia and pancreatitis, hepatoxicity, blood dyscrasias, hyperostosis, premature epiphyseal closure and night blindness. An association with severe skin reactions, such as erythema multiforme, Stevens–Johnson syndrome and toxic epidermal necrolysis, has been reported.39 Although a causal relationship has not been shown, patients must be warned about depression, suicidal thoughts and psychosis, and monitored closely.40
Acne can almost always be controlled with medication. However, results may not be seen for weeks or months. Most topical medicines work within four to eight weeks. Tretinoin may show peak results in three to six months.
