Retinoids or vitamin A derivatives. These drugs are available as topical or oral drugs. Topical retinoids clear up moderate-to-severe acne by normalizing the way the skin grows and sheds. They can be used in combination with other acne products, such as benzoyl peroxide and oral antibiotics. Topical retinoids don’t have the severe side effects of oral retinoids; however, they aren’t recommended for pregnant or nursing women. The recommended starting dose of isotretinoin is 0.5 mg/kg/day. Daily doses can be titrated up or down, between 0.1 and 1 mg/kg/day, depending on response to treatment and presence of adverse effects. The total dose of isotretinoin over the course of treatment should be between 120 – 150 mg/kg of body weight.14 The length of treatment is dictated by the daily dose. Courses usually last between three to four months but may continue for one year or longer. Cystic pimples are inflammatory acne which involves more white blood cells rushing to the area and because there are more enzymes in that area, it leads to more destruction. The skin then tries to desperately heal itself which results in a scar.

Practitioners should discuss with the patient, and then provide in writing, the potential adverse effects, particularly teratogenicity in females. A suitable patient information brochure is provided with the medicine by the distributor. Baseline laboratory investigations should be requested. Female patients will require advice on contraceptive options, and where necessary, be prescribed contraceptives (some will already be using oral contraceptives for managing acne symptoms). Patients (and parents/caregivers for those under age 16 years) should be asked to sign a consent form, to indicate that they understand the adverse effects that are possible while taking isotretinoin and, for female patients, the importance of not becoming pregnant during the course of their treatment and a further month after it has been discontinued.

Wash your whole body with an oil-free, medicated body wash. You want one with an active ingredient of 2% salicylic acid. Neutrogena Body Clear Body Wash is a particularly good product with this. Concentrate on the areas with pimples and wait a minute or so before rinsing to get rid of all the oil. Let the medicine sink in and do its work.

Benzoyl peroxide is an antibacterial ingredient, and it’s very effective at killing the P. acnes bacteria that causes breakouts. But benzoyl isn’t without its downsides. The leave-on creams and cleansing treatments can dry out sensitive skin types and bleach clothing if you aren’t careful. Board-certified dermatologist Eric Meinhardt, M.D., previously told SELF that it’s best to stick to formulations that have no more than 2 percent of benzoyl peroxide listed on the active ingredients chart; stronger concentrations are harder on your skin without being any tougher on bacteria.

Lasers and other light systems using visible light wavengths, appear safe and helpful for acne. Treatment is often delivered twice weekly for four weeks. The effect may be enhanced by use of a photosensitising agent (photodynamic therapy).

Many skin conditions can mimic acne vulgaris and are collectively known as acneiform eruptions.[27] Such conditions include angiofibromas, epidermal cysts, flat warts, folliculitis, keratosis pilaris, milia, perioral dermatitis, and rosacea, among others.[19][70] Age is one factor which may help distinguish between these disorders. Skin disorders such as perioral dermatitis and keratosis pilaris can appear similar to acne but tend to occur more frequently in childhood, whereas rosacea tends to occur more frequently in older adults.[19] Facial redness triggered by heat or the consumption of alcohol or spicy food is suggestive of rosacea.[71] The presence of comedones helps health professionals differentiate acne from skin disorders that are similar in appearance.[8] Chloracne, due to exposure to certain chemicals, may look very similar to acne vulgaris.[72]

Herbal therapies such as tea tree oil, and topical and oral ayurvedic compounds seem to be well tolerated; however, there are limited data about their efficacy and safety in treating acne.3 One clinical trial showed that topical tea tree oil was effective but had a slower onset of action than traditional topical agents.43 The Cochrane Collaboration is undertaking a systematic review of the effects of treatments in the management of acne that are currently considered complementary or alternative.

Wash problem areas with a gentle cleanser. Twice a day, use your hands to wash your face with a mild soap and warm water. If you tend to develop acne around your hairline, shampoo your hair every day. And be gentle if you’re shaving affected skin.

Don’t overdo it. “Using any more than two acne products is just going to dry you out and make it worse,” says New York dermatologist Amy Wechsler says. Avoid skin care products with alcohol, which can irritate your skin, causing outbreaks. And never pick, scratch, pop, or squeeze blemishes. It can make acne worse, and lead to skin infections that may leave scars.
