This targeted treatment contains hydroquinone, which Dr. Bank calls, “the gold standard for fading hyperpigmentation.” The formula contains 2% HQ, the maximum for an OTC formulation (prescription formulations are 4% or higher).

After eating the chocolate, the participants’ blood cells produced more interleukin-1b, which is a marker of inflammation, when exposed to Propionibacterium acnes. Eating chocolate also increased production of another immune system factor called interleukin 10 after exposure to Staphylococcus aureus. Interleukin 10 is thought to lower the body’s defenses against microorganisms, so higher levels could allow bacteria to infect pimples and worsen them.

The earliest pathologic change is the formation of a plug (a microcomedone), which is driven primarily by excessive growth, reproduction, and accumulation of skin cells in the hair follicle. In normal skin, the skin cells that have died come up to the surface and exit the pore of the hair follicle. However, increased production of oily sebum in those with acne causes the dead skin cells to stick together. The accumulation of dead skin cell debris and oily sebum blocks the pore of the hair follicle, thus forming the microcomedone. This is further exacerbated by the biofilm created by P. acnes within the hair follicle. If the microcomedone is superficial within the hair follicle, the skin pigment melanin is exposed to air, resulting in its oxidation and dark appearance (known as a blackhead or open comedo). In contrast, if the microcomedone occurs deep within the hair follicle, this causes the formation of a whitehead (known as a closed comedo).
Years ago, teenagers with acne were told to cut out the potato chips and given a tube of Clearasil. Today, we know far more about the reasons why some people develop acne and how it can most effectively be treated.
Modern medicine considers mild to moderate acne an acceptable feature of growing up. If you go to your doctor for help with whiteheads, blackheads, or pimples, chances are you’ll be given a prescription for an antibiotic or benzoyl peroxide, or both, and sent on your way. If you have especially deep skin infection causing nodules or cysts, you may be put on Retin-A or Accutane, and if you have the more aggressive forms of acne that cause pimples to grow together all over the top half of the body (acne conglobata) or the kind of acne that results from the immune system’s attack on an ingrown hair (acne keloidalis nuchae), you will probably get a referral to a dermatologist for laser therapy or surgical reconstruction procedures.
Most patients with mild acne can be treated with topical treatment (gels, solutions and lotions) that can be obtained over-the-counter in New Zealand without prescription. Most people just use topical agents for facial skin as they can be difficult to apply to one’s back. Extra vitamins and minerals have not been proved to help.

“Pomade acne” is a breakout caused by hair-care products including conditioner, shampoo, gel, and hair spray. This form of acne occurs when oils from styling products seep into skin, usually around the hairline, and trap acne-causing bacteria in pores.
Oral contraceptives: Oral contraceptives, which are low in estrogen to promote safety, have little effect on acne one way or the other. Some contraceptive pills have been shown to have modest effectiveness in treating acne. Those that have been U.S. FDA approved for treating acne are Estrostep, Ortho Tri-Cyclen, and Yaz. Most dermatologists work together with primary-care physicians or gynecologists when recommending these medications.
Omega-3 fatty acids are a type of “good fat” found mainly in fatty fish like salmon, sardines and anchovies. Research shows these fats reduce inflammation in the body. Inflammation contributes to acne, and some experts believe that increasing dietary omega-3s may keep acne symptoms in check. Is there science behind it? One small study involving only 5 young people with acne showed a benefit, but larger studies are needed. Still, adding more omega-3s to your diet may be helpful. If you don’t enjoy the taste of fish, there are omega-3 supplements available at most health food stores – but talk to your doctor first.
Whiteheads are small, raised bumps on the surface of the skin, and they are another form of clogged pore (comedo) that can be a precursor to pimples. Similar to blackheads, whiteheads form when the hair follicle fills with sebum. Unlike blackheads, whiteheads haven’t pushed all the way through to the skin’s surface so they are considered closed off from air that turns the sebum dark or black.
Several scales exist to grade the severity of acne vulgaris, but no single technique has been universally accepted as the diagnostic standard. Cook’s acne grading scale uses photographs to grade severity from 0 to 8 (0 being the least severe and 8 being the most severe). This scale was the first to use a standardized photographic protocol to assess acne severity; since its creation in 1979, the scale has undergone several revisions. The Leeds acne grading technique counts acne lesions on the face, back, and chest and categorizes them as or non-inflammatory. Leeds scores range from 0 (least severe) to 10 (most severe) though modified scales have a maximum score of 12. The Pillsbury acne grading scale simply classifies the severity of the acne from 1 (least severe) to 4 (most severe).

2. Turn up the heat. When you have painful, cystic pimples beneath the surface of your skin that haven’t come to a head, use heated compresses to draw the infection to the surface. Encouraging your body to sweat — whether in a sauna, steam room or in an exercise class or at the gym — is a great way to help your body rid the skin of toxins and bacteria.
A small, cylindrical container with a roller cap or dropper like this one (prefer glass bottles as essential oils can interact with plastic) . It helps if the bottle is opaque. This is to preserve the potency of the oils.
Retinoids: Retin-A (tretinoin) has been in use for years, and preparations have become milder and gentler while still maintaining its effectiveness. Newer retinoids include adapalene (Differin) and tazarotene (Tazorac). These medications are especially helpful for unclogging pores. Side effects may include irritation and a mild increase in sensitivity to the sun. With proper sun protection, however, they can be used even during sunny periods. The combination medication known as Epiduo gel — which contains the retinoid, adapalene, along with the antibacterial, benzoyl peroxide — is applied once a day.
Benzoyl peroxide is an antibacterial ingredient, and it’s very effective at killing the P. acnes bacteria that causes breakouts. But benzoyl isn’t without its downsides. The leave-on creams and cleansing treatments can dry out sensitive skin types and bleach clothing if you aren’t careful. Board-certified dermatologist Eric Meinhardt, M.D., previously told SELF that it’s best to stick to formulations that have no more than 2 percent of benzoyl peroxide listed on the active ingredients chart; stronger concentrations are harder on your skin without being any tougher on bacteria.
I tried all of these things and they just made my fussy sensitive skin worse. Retin-A dried my skin out to the point that it was flaking off and what was underneath was raw or even worse- open sores. Even with all of that drama, I still had horrible acne!

Trapped sebum is ideal for a germ (bacterium) called Propionibacterium acnes to live and multiply. Small numbers of this bacterium normally live on the skin and do no harm. However, if a large number develop in the trapped sebum, the immune system may react and cause inflammation. If inflammation develops, it causes the surrounding skin to become red and the spots become larger and filled with pus (pustules). In some cases the pustules become even larger and form into small nodules and cysts.
Zinc is a mineral your body needs in small amounts that not everyone gets enough of, especially vegetarians. That’s because zinc is most abundant in red meat, organ meats and seafood, although whole grain foods and nuts are decent sources. Some research shows that acne sufferers have lower zinc levels than those without the disease and that taking zinc supplements may help. Unfortunately, zinc supplements can interfere with the absorption of other minerals like copper, so it’s important to take zinc under a doctor’s care. The other way to get the benefits is to eat more foods rich in zinc.
