When topical agents are insufficient or not tolerated, or in cases of moderate to severe acne, especially when the chest, back and shoulders are involved, systemic antibiotics are often considered the next line of treatment (Table 3).20,21 However, regular use of a combination of topical antibiotics and benzoyl peroxide may be similarly effective, as shown in a randomized controlled trial of five antimicrobial regimens.22
Cystic acne . The most severe form of acne, cystic acne develops as the result of an actual infection in the area of the outbreak. Cystic acne often runs in families. It’s often very painful and can result in disfiguring, permanent scarring.
Isotretinoin (eye-soh-tret-in-OH-in) is a prescription medicine for severe acne. This type of acne causes deep, painful cysts and nodules. These can be the size of a pencil eraser — or larger. As this acne clears, scars often appear.
It’s important to understand what’s inside those acne lesions to properly treat them. When pores fill up with sebum, dead skin cells, and bacteria, the mixture causes inflammation and results in painful pimples.
Among the most widely successful strategies they employ is prescribing different topical medications – which are frequently “coupled” in a single lotion, gel or other delivery substance – in combination with oral antibiotics to address multiple causes and effects of acne.
Use a sheet or face mask. Sheet or Face masks contain compounds that soothe your skin and kill bacteria. Use a sheet or face mask 2-3 times a week for 15-20 minutes to dry out your skin and clean out your pores. Buy face masks from your local beauty supply or drugstore, or make your own at home.
Erythromycin kills acne-causing bacteria to prevent new breakouts. This drug increases sun sensitivity, so avoid sunlight exposure or wear protective clothing if you have to spend time in the sun. Other side effects include diarrhea, vomiting, loss of appetite, vomiting and stomach pain.
The best approach to treatment requires some patience as it involves starting with milder over-the-counter medications or home remedies and then progressing to the stronger prescription drugs if satisfactory results are not realized. In general, the risk of side effects increases with the strength of what is being used as treatment. If you need to progress to the point where requiring an isotretinoin prescription is a possibility, there will be some decisions that have to be made, and these decisions should involve discussing with a dermatologist the pros, cons, and risks involved.
Spot treatments are designed to give problem pimples a mega-dose of concentrated benzoyl peroxide — in a couple of regimens, like the Proactiv Teen Kit, the spot treatment had nearly three times the benzoyl peroxide as its all-over treatment. It makes sense: If benzoyl peroxide can be irritating to the skin in high concentrations, limiting its intensity to just the pimple itself could save the rest of your healthy skin.
Some topical acne treatments (described below) may dry the skin. If this occurs, use a fragrance-free, water-based moisturising cream. Do not use ointments or oil-rich creams, as these may clog the holes of the skin (pores).
Most essential oils are great for the skin. They can be used alone or in combinations that make a potent serum. This recipe is going to make just that. Though essential oils are a bit on the expensive side, you’ll still get your money’s worth and your skin will thank you for it.
6. Gently exfoliate the skin once a week if you have oily or acne-prone skin. This will immediately remove dead skin cells from the skin surface resulting in a brighter appearance. Longer term, it will reduce the development of blackheads. It is important not to overdo this otherwise you will end up irritating the skin and making things worse better.

The reason a woman is placed on birth control to clear up acne is to reduce androgens, male sex hormones like testosterone, in her bloodstream. These androgens trigger an excess production of sebum oil that clogs pores and promotes breakouts. When you take birth control, like Yaz, Yasmin or Estrostep, you’re ingesting estrogen and progestin, a synthetic form of progesterone. Hormonal agents provide effective second-line treatment in women with acne regardless of underlying hormonal abnormalities.30 It is not necessary to demonstrate androgen excess to achieve a benefit from antiandrogen therapy. Clinical observation suggests that deep-seated nodules on the lower face and neck are especially responsive to hormonal therapy.6
Azelaic acid is an alternative that mainly works by unplugging blocked pores. So, like retinoids, it is good at clearing blackheads and whiteheads. It has some effect on reducing inflamed acne too but probably not as much as antibiotics or benzoyl peroxide. However, it may cause less skin irritation than benzoyl peroxide.

Frankincense oil is a personal favorite for me and my wife, Chelsea. Containing antibacterial and anti-inflammatory properties, it’s amazing for almost all skin types and perfect for acne-prone skin. Frankincense invites new cell growth, which can help reduce the appearance of scars. It also helps prevent or eliminate bacteria, part of what can cause acne in the first place.
Transient increases in liver enzymes can occur in people taking isotretinoin.17 These increases are usually mild and benign and will resolve upon cessation of isotretinoin. If liver transaminases are > 2.5 times the upper limit of normal, investigation of possible causes of liver dysfunction (e.g. viral hepatitis, alcoholism) is required and it is recommended that isotretinoin is stopped. Patients should be advised of the risk of drinking alcohol in excess while taking isotretinoin.

Treatment of acne scars: For those patients whose acne has gone away but left them with permanent scarring, several options are available. These include surgical procedures to elevate deep, depressed acne scars and laser resurfacing to smooth out shallow acne scars. Newer forms of laser resurfacing (“fractional resurfacing”) are less invasive and heal faster than older methods, although results are less complete and the procedures may need to be repeated three or more times. These treatments can help, but they are never completely successful at eliminating acne scars.

Oral antibiotics kill bacteria that cause acne, reducing inflammation and preventing breakouts. Oral contraceptives treat acne caused by overactive oil glands.
