Isotretinoin (eye-soh-tret-in-OH-in) is a prescription medicine for severe acne. This type of acne causes deep, painful cysts and nodules. These can be the size of a pencil eraser — or larger. As this acne clears, scars often appear.

“You unfortunately cannot determine the strength of a product strictly by the percentage of its active ingredients because how well a product works depends on how well its inactive ingredients help it penetrate the skin,” explains Dr. Green. “In other words, a 2 percent benzoyl peroxide may be more effective than another brand’s 5 percent benzoyl peroxide because there are other ingredients helping out.”

Buying a generic acne wash won’t necessarily improve your complexion; for a cleanser to be most effective, you have to pay attention to your skin’s needs and pick the ingredients accordingly. Benzoyl peroxide. For mild acne, you may try, or your doctor may recommend, treatment with a nonprescription drug that contains benzoyl peroxide. It’s believed that this compound works by destroying the bacteria associated with acne. It usually takes at least four weeks to work and it must be used continuously to keep acne at bay. Like many over-the-counter and prescription products, it does not affect sebum production or the way the skin follicle cells are shed, and when you stop using it, the acne comes back. It is available in many forms: creams, lotions, washes, and gels. Benzoyl peroxide can cause dry skin and can bleach fabrics, so take care when applying it. Consider wearing an old T-shirt to bed if you are applying it to your back or chest overnight.

Depending on acne severity, Benzoyl Peroxide is used in varied concentrations: 2.5%, 5%, and 10%. If you are sensitive to salicylic acid or find commercial products ineffective for your back acne (bacne), try a medicated skin powder. This will usually work well and won’t over-dry your skin. Speak to your pharmacist about brand options.

Using sterile devices, these professionals can pull out the hardened material or fluid pus found within a comedo or pimple. After extraction, dermatologists will often apply medicine to the open pore to encourage healing. Even if a dermatologist is able to successfully extract the contents of an acne lesion, there is a high chance of reoccurrence. Erythromycin and azithromycin are second-line therapies called macrolides. Azithromycin is dosed 3 times per week to 4 times per month to treat acne. Macrolides can cause GI upset and, in severe cases, cardiac conduction abnormalities and even hepatotoxicity.

Once the scar tissue responsible for tethering the skin downward is broken up, the skin can snap back to the surface and some blood usually fills underneath and clot, which helps form tissue to level skin with the surface: the hole and scar is gone 🙂

If you have severe acne which does not respond to the treatments usually prescribed by GPs, you may need to see a hospital specialist. In particular, if your doctor believes you would be helped by isotretinoin tablets. You may also need hospital treatment for acne scars. Options available for scarring include laser resurfacing of the skin, mechanical or chemical peeling of the skin, breaking the scar tissue down with a sterile needle (subcision) and injection of collagen filler.

Blackheads are, essentially, open comedones. “Comedone refers to plugging of the follicular opening,” explains NYC dermatologist Elizabeth Hale, M.D., referring to hair follicles that technically cover your entire face and body (hi, peach fuzz). “Every hair follicle appears in a sebaceous gland.” So a blackhead is the mixture of dead cells, bacteria, and grime that builds up and hardens in the follicular opening—but it’s open to the world, which is why blackheads are so easy (read: tempting) to push out.

A randomized controlled trial with 128 women showed a reduction in acne lesions of 63% with 35 μg ethinylestradiol and 3 mg drospirenone and a 59% reduction with 35 μg ethinylestradiol and 2 mg cyproterone acetate.34 Antiandrogen therapy is usually needed for at least three to six months to see significant improvement.

Because the cost of a deep peel for full face can be pretty high ($2000-$5000), many affordable home kits are now available. There are even professional peel kits with high concentration of TCA that are available.

Benzoyl peroxide is an antibacterial ingredient, and it’s very effective at killing the P. acnes bacteria that causes breakouts. But benzoyl isn’t without its downsides. The leave-on creams and cleansing treatments can dry out sensitive skin types and bleach clothing if you aren’t careful. Board-certified dermatologist Eric Meinhardt, M.D., previously told SELF that it’s best to stick to formulations that have no more than 2 percent of benzoyl peroxide listed on the active ingredients chart; stronger concentrations are harder on your skin without being any tougher on bacteria.

Cold temperature helps reduce redness and swelling so get a frozen ice pack or ice cubes right out of the freezer. Wrap in a washcloth or plastic wrap to apply on the affected area. Treat the pimple with ice for a few minutes but no more than 10 minutes at least three times a day and the process will reduce the swelling or inflammation. It will also ease the itching. You can start the treatment even before the pimple gets swollen or once you feel a pimple is beginning to form.

“Stress leads to increased cortisol levels,” says dermatologist Ava Shamban. Like testosterone, cortisol is a hormone that can increase oiliness as well as inflammation. Not a good combo when you want to avoid being red in the face. Once upon a time, our bodies produced cortisol only when we were in danger, but nowadays, it’s much more likely to flood your system in response to a looming deadline than in response to a predator in the underbrush. Isopropyl alcohol, SD alcohol, and denatured alcohol are everywhere in acne treatment because they trick you into thinking they’re working: Splash some on and any oil on your face instantly vaporizes. Granted, there are other less nefarious reasons for alcohol too, such as helping vitamin C penetrate the skin. But ultimately, we weren’t fans of their inclusion in acne treatment ingredient lists.

Benzoyl peroxide. It’s an antibacterial that eliminates P. acnes on skin. Look for this ingredient if you have lots of pustules, since they’re an indication that you’re dealing with higher levels of bacteria on your skin. This ingredient won’t be as effective when used on blackheads.

When your acid mantle is damaged, you’re actually more susceptible to breakouts, enlarged pores, and inflammation. To make matters worse, evaporating all the oil on your face can actually set your sebaceous glands into overdrive, leaving your skin oilier than ever. If any product included a simple alcohol high up in its ingredients list, we nixed its whole kit.

If you have been taking topical retinoids while you were pregnant do not immediately assume the worst. Findings from a study of a large number of pregnancies where the mother was exposed to topical retinoids, along with data from studies looking into the absorption of retinoids after use of topical cream on the face, suggests that you have not exposed your child to any extra risks. However, as retinoids are known to cause birth defects, it is important that you discuss this issue with your doctor.

10. Check your diet. Diet has a small part to play in certain individuals suffering with acne. Not everyone’s acne is driven by diet and usually the problem comes down to genetic and hormonal factors, however, the following methods can be considered:

Cleansing – using cleansers specifically developed for acne-prone skin can help. Try washing the affected areas twice per day. Don’t overdo it. Too much cleansing can cause other skin problems, such as dryness or skin irritations. Try to keep hair clean and off the face and neck, as oil from the hair can make acne worse.

These powerful rays penetrate follicles to kill off acne-causing bacteria. For severe cases, photodynamic therapy adds a topical solution called Levulan to blue light therapy. Note that these treatments can cause temporary redness and may not be covered by insurance. Dermatologists’ fees start at about $250 per session for blue light therapy and $800 for photodynamic therapy.

If you’re experience hormonal acne every single month—and it’s don’t-want-to-leave-the-house bad—it’s worth visiting your doctor to rule out polycystic ovarian syndrome, a hormonal imbalance that can lead to infertility, anxiety, and depression. “This type of treatment is focused on teenagers, who usually have a period of a year to four years when they’re breaking out because of changing hormone levels and increased oil production, and in some cases, genetics,” says Amy Taub, MD, founder and medical director of Advanced Dermatology in Lincolnshire, IL. Of all forms of laser and light therapy, the most evidence is with photodynamic therapy (PDT) in treating acne. [29] PDT involves a photosensitizer applied to the skin, time allowed for it to be absorbed by the pilosebaceous units, and then application of a light source to activate the photosensitizer, generating reactive oxygen species to damage the glands and reduce Cutibacterium acnes (formerly Propionibacterium acnes). A 2016 Cochrane review was unable to draw firm conclusions regarding the benefit of light therapy in acne, owing to the variation in wavelength, dose, photosensitizer, comparator interventions, and treatment frequency that were studied. [63] However, despite the interstudy variation, many individual trials show PDT to be effective in reducing lesions in mild-to-severe acne, with 5-aminolevulinic acid and methyl aminolevulinate photosensitizers and red light source as promising treatment components. [64, 65]

Sulfur has been used in treating acne for thousands of years, and in the 1950s, it became available in a foam product designed for direct application to the skin. Can sulfur work on acne? Sulfur dries out the skin, consequently drying out and shrinking pimples. It tends to be gentler than Benzoyl Peroxide treatments, and is less likely to dehydrate the skin, which would result in further irritation. 

When we asked what those types of other ingredients are, Dr. Green said there were no clear answers there either — skin is too subjective. “I think the best answer is to use one that feels good and rubs into your skin well without over-drying it,” he says. The more comfortable it is to apply, the more likely you are to keep up the regimen.

Salicylic acid. On the skin, salicylic acid helps to correct the abnormal shedding of cells. For milder acne, salicylic acid helps unclog pores to resolve and prevent lesions. It does not have any effect on sebum production and does not kill bacteria. It must be used continuously, just like benzoyl peroxide, because its effects stop when you stop using it — pores clog up again and the acne returns. Salicylic acid is available in many acne products, including lotions, creams, and pads.

My skin was like that too when I was 14. You could always exfoliate.. I usually use lemon, water and sugar to make a mask and then I leave it on my skin for about 15 minutes. That really helped with the big pimples. 🙂

But the side effects of targeted spot treatments aren’t always worth it. “So many products instruct consumers to use benzoyl peroxide to spot treat red bumps and pustules. I don’t recommend it,” says Dr. Green. “Benzoyl peroxide, when placed on red spots, can actually cause more irritation and inflammation to the area. It’s best used to prevent red bumps and pustules, and applied all over the area you want to treat.” Robin Townsend, a medical aesthetician based in Cincinnati, was also quick to naysay a spot-treat-only approach: “Acne affects all of the pores. If someone is going to spot treat against my advice, I still suggest they spot treat one day and treat the whole face the next.”

Depression: A connection between taking isotretinoin and developing symptoms of depression/anxiety has been suggested. Dermatologists take reports of depression seriously. Results from some studies, however, show that sometimes patients treated with isotretinoin have fewer symptoms of depression and anxiety. These patients also have improved quality of life.

Frequently used topical retinoids include adapalene, isotretinoin, retinol, tazarotene, and tretinoin.[46] They often cause an initial flare-up of acne and facial flushing, and can cause significant skin irritation. Generally speaking, retinoids increase the skin’s sensitivity to sunlight and are therefore recommended for use at night.[1] Tretinoin is the least expensive of the topical retinoids and is the most irritating to the skin, whereas adapalene is the least irritating to the skin but costs significantly more.[1][82] Tazarotene is the most effective and expensive topical retinoid, but is not as well-tolerated.[1][82] Retinol is a form of vitamin A that has similar but milder effects, and is used in many over-the-counter moisturizers and other topical products.

Usually when you get a pimple, it heals into healthy, blemish-free skin. It’s as if there was never a pimple in the first place. Unfortunately, that’s not always the case: Most of us will wear the scar of one or two breakouts in some random corner of our face. And some people forever wear the aftermath of cystic acne, long after it’s under control.

Treatment for acne vulgaris should aim to reduce severity and recurrences of skin lesions as well as to improve appearance. The approach depends on the severity of the acne, treatment preferences and age of the patient, and adherence and response to previous therapy (Table 2).3,6 Various acne treatments target different steps in the pathogenesis of acne, from counteracting androgens and decreasing sebum production to preventing follicular occlusion, reducing P. acnes proliferation and decreasing inflammation.

Hyperkalemia is a potential dangerous adverse effect from spironolactone; however, a 2015 large retrospective study of healthy women aged 18-45 years confirms potassium monitoring is unnecessary for these patients while taking spironolactone. [48] Plovanich et al found the rate of hyperkalemia in patients taking spironolactone is equivalent to the minimal baseline rate of hyperkalemia in this population, and, of the hyperkalemia cases, none was clinically significant.

Inflammatory bowel disease (IBD) is another concern. There have been reports of patients developing IBD while taking isotretinoin. Again, there is not enough scientific evidence to determine whether isotretinoin is actually the cause. More research is needed.

Oral antibiotics are recommended for no longer than three months as antibiotic courses exceeding this duration are associated with the development of antibiotic resistance and show no clear benefit over shorter courses.[85] Furthermore, if long-term oral antibiotics beyond three months are thought to be necessary, it is recommended that benzoyl peroxide and/or a retinoid be used at the same time to limit the risk of P. acnes developing antibiotic resistance.[85] Dapsone is not a first-line topical antibiotic due to higher cost and lack of clear superiority over other antibiotics.[1] Topical dapsone is not recommended for use with benzoyl peroxide due to yellow-orange skin discoloration with this combination.[10]

Pimples commonly occur during puberty, and both young boys and girls face it. When the body undergoes physical changes and gets ready for reproduction, the sebaceous glands become overactive. The sebum build-up also blocks pores and causes pimples (1).

Antibiotics. These work by killing excess skin bacteria and reducing redness. For the first few months of treatment, you may use both a retinoid and an antibiotic, with the antibiotic applied in the morning and the retinoid in the evening. The antibiotics are often combined with benzoyl peroxide to reduce the likelihood of developing antibiotic resistance. Examples include clindamycin with benzoyl peroxide (Benzaclin, Duac, Acanya) and erythromycin with benzoyl peroxide (Benzamycin). Topical antibiotics alone aren’t recommended.

“Stress leads to increased cortisol levels,” says dermatologist Ava Shamban. Like testosterone, cortisol is a hormone that can increase oiliness as well as inflammation. Not a good combo when you want to avoid being red in the face. Once upon a time, our bodies produced cortisol only when we were in danger, but nowadays, it’s much more likely to flood your system in response to a looming deadline than in response to a predator in the underbrush. So when your acne starts acting up, take a minute to figure out whether there’s anything you’re freaking out over, then unload all of that skin-wrecking stress.

They also require some follow-up care such as sun protection, a particular cleansing regimen or antibiotics to prevent infection as the deep peels get rid of a big part of your skin, which is your natural protection to harmful bacteria.

Some topical acne treatments (described below) may dry the skin. If this occurs, use a fragrance-free, water-based moisturising cream. Do not use ointments or oil-rich creams, as these may clog the holes of the skin (pores).

“Some teenagers are very confident and self-assured even if their face looks terrible, so they’re easy to deal with,” Taylor said. “But then there are kids who become very depressed and withdrawn and may isolate themselves. With them I try to be hopeful and optimistic, upbeat and positive, to tell them that I know it’s hard having this condition and to show some sympathy. Or empathy, really, because I had acne, too, when I was a teen.”

If you have scarring, your dermatologist may suggest surgery to help heal lesions and remove scarring. A laser can reshape scar tissue and reduce redness. Dermabrasion is a type of surgery that can remove surface scars and reduce the depth of deep scars. Another option is to transfer fat from one part of the body to the face. In some cases, a single treatment can help scarring. But for lasting results, several sessions of this acne treatment are often needed. There are also topical treatments for acne scarring.

It’s not a secret that you really need to take care of your skin and treat pimples carefully because if you don’t treat them well than it can lead to skin damages, ugly scars etc. There are several treatments available in the market but its always better to go for natural treatment to avoid any side effects. Intralesional steroid injections have been found to be beneficial for large inflammatory lesions. Comedone removal does not affect the course of the disease, but it does improve the patient’s appearance. [29] Additionally, some patients may benefit from superficial peels that use glycolic or salicylic acid, although more research needs to be conducted to establish best practice for these modalities.

While it’s hard to pinpoint specific foods that cause acne, “a high glycemic diet causes spikes in blood sugar levels that will stimulate the release of certain growth factors that in turn lead to increased oil production and acne,” says Dr. Goldenberg. In a nutshell: Too much sugar and processed grains can mess with your skin.

Isotretinoin has been shown to affect all four pathogenic processes involved in acne formation and results in sebaceous gland apoptosis (cell death) and altered gene expression.16–18 Isotretinoin is highly effective for clearing acne and reducing recurrence. A single course of isotretinoin will result in significant improvement or complete remission of acne in nearly all patients. Long-term efficacy depends on individual patient factors and the duration and dose of treatment.18 If isotretinoin is ineffective, investigation of a potential endocrine cause for the acne, such as polycystic ovary syndrome, should be considered.19

The first three types listed are all types of the most common form of acne – acne vulgaris. The non-inflammatory kind is the mildest of these types and is usually characterized by the appearance of whiteheads or blackheads. The inflammatory type is a moderate form that is characterized by the formation of papules, pustules, and macules.

All cases of acne are not created equal, and neither are all acne treatments. Although all acne has its roots in the same process — hormonal fluctuations that stimulate oil production — not all acne is equally severe and not all cases of acne will respond to the same types of treatments.

Add zinc-rich foods such as kefir, yogurt, lamb, pumpkin seeds and chicken. According to a recent study published in BioMed Research International, there is a correlation between low zinc levels and the severity of acne. (18)

Dermal injections have become a popular way to treat and alleviate shallow acne scars. There are many dermatologists and cosmetic surgeons performing these injections, and there’s been a significant increase in the number and quality of filler substances used to help decrease the appearance of scars. Dermal fillers may also stimulate the production of collagen, to help smooth the skin even further. While these injections have immediate effect, they also are only temporary, and last around three to 12 months after injection. 

Unfortunately, there’s no overnight cure for acne. In order to properly get rid of acne without damaging your skin, you must practice a daily cleansing routine. Use a gentle acne treatment that effectively fights breakouts without causing damage to the skin.

Acne vulgaris is the world’s most common skin disease. Anyone of any age can get it, but teens and young adults are most likely to suffer acne outbreaks. In fact, 80 percent of people ages 11-30 have acne outbreaks at least occasionally.

The good point is that it is permanent, unlike the other dermal fillers, but it requires a more specialised technique that must be perfomed with care by a skilled professional. A mistake is harder to correct.

A review published in the Archives of Dermatological Research found evidence that sleep deprivation, stress and other aspects of “modern life” are linked to adult female acne. The researchers point out that “Modern life presents many stresses including urban noises, socioeconomic pressures and light stimuli. Women are especially affected by stress during daily routine. Women also have a higher risk of developing psychiatric disorders such as depression and anxiety. Sleep restriction is added to these factors, with several negative consequences on health, including on hormonal secretion and the immune system.” (7)

Many different oral contraceptives have been shown to be effective in treating acne. Oral contraceptives approved by the U.S. Food and Drug Administration (FDA) for the treatment of hormonal acne contain ethinyl estradiol plus either the progestin norgestimate, norethindrone acetate or drospirenone.

General Practitioners are now significant prescribers of isotretinoin. From July, 2011 – June, 2012 there were a total of 46 531 dispensed prescriptions for isotretinoin, of which 58% originated from a General Practitioner.24

Drugs: Some medications may cause or worsen acne, such as those containing iodides, bromides, or oral or injected steroids (either the medically prescribed prednisone [Deltasone, Orasone, Prednicen-M, Liquid Pred] or the steroids that bodybuilders or athletes sometimes take). Other drugs that can cause or aggravate acne are anticonvulsant medications and lithium (Eskalith, Lithobid). Most cases of acne, however, are not drug related.

Genetics is thought to be the primary cause of acne in 80% of cases.[2] The role of diet and cigarette smoking is unclear, and neither cleanliness nor exposure to sunlight appear to play a part.[2][13][14] During puberty, in both sexes, acne is often brought on by an increase in hormones such as testosterone.[5] A frequent factor is excessive growth of the bacterium Propionibacterium acnes, which is normally present on the skin.[5]

Toothpaste dries out the pimples and reduces their healing time. Its antibacterial properties play a vital role in eliminating the bacteria that cause pimples (6). This can be used for pimples on the face and also on the body, like the legs and arms.

In severe cases, oral isotretinoin may be considered. This medication can be very effective but can also cause serious side effects including severe birth defects. Strict protocols must be followed. Monthly appointments with the treating doctor must be kept throughout the treatment period to monitor for any side effects. In females of child-bearing age, protocol includes two forms of birth control. The treatment period is usually five months.

Adapalene is available as a 0.1% cream and gel. It should be applied thinly, once daily.8 Adapalene is usually better tolerated than tretinoin.7 The gel is suitable for most people, although those with dry skin may prefer the cream.

The microinjury triggers the skin repair process by producing new collagen and elastin in the skin’s dermis, thereby improving the appearance of scars and rejuvenating the skin. This is what we call a collagen induction therapy.

Herbal, organic, and “natural” medications. There are many herbal, organic, and natural products marketed to treat or prevent acne. The effectiveness of these agents isn’t proven and they are unlikely to have much benefit.

Noninflammatory acne occurs when tiny hair follicles, typically on your face, back, or neck, become clogged with oil or debris. Our skin’s natural oils are secreted through the hair follicle from tiny sebaceous glands; when these glands become overactive, comedones, aka blackheads and whiteheads, form. The good news is, noninflammatory acne is easier to treat than inflammatory acne. Try these three tactics:

In 2005, the FDA established an online tracking database, called iPledge, and now requires all patients to sign onto the database to continue receiving their prescriptions. The iPledge system requires women to submit two negative pregnancy tests before they can receive an initial prescription for isotretinoin. Women must also undergo a monthly pregnancy test before each refill. Men are also tracked because of isotretinoin’s depression risk.

A BHA product often cited includes salicylic acid, it must be between a pH between 3 and 4 to work. A BHA works to slough (to get rid of) off dead skin cells and encourage new skin growth. As a result, you may experience dry skin and scaliness around your acne, but this will dissipate over time as your skin begins to regenerate faster. Use this in a cleanser or spot treatment daily on the acne-affected areas of your skin.[6]

Acne is an inflammatory rash that is almost universal among individuals going through puberty in industrialized societies. It can, however, occur at all ages. Typical acne affects the skin of the face, chest, and back and rarely the neck and upper arms of teenagers and young adults.

There are also several other ingredients you can use on skin to reduce inflammation, such as chamomile oil and aloe vera. These can especially be beneficial if your skin is irritated due to using products containing salicylic acid, benzoyl peroxide, or both.

Despite the popular belief that certain greasy foods directly contribute to acne, there is actually insufficient evidence of a link between “food enemies” and pimples. While that means no food is inherently evil when it comes to causing breakouts, some are certainly better than others. To keep skin in check, Dr. Day suggests eating a diet high in antioxidants and low in processed foods. Still, she cautions that a diet overhaul alone won’t clear up skin. Here are foods that can help clear acne (and a few that make it worse!).

So, we decided to delve into the wild, weird, and often dubious world of acne treatments. From prescription-strength remedies (Retin-A! Accutane!) to kitchen-pantry saviors, such as apple cider vinegar, we’ve evaluated a host of blemish-battling solutions to separate the worthy from the whack. Some of the top dermatologists and aestheticians from coast to coast, each with a strong background in acne, gave us their thoughts on what’s brilliant (and what’s baloney).

As common as acne is, it remains a skin disease that is imperfectly understood. Some forms are incurable, some can be cured, and most simply go away on their own. Exactly what can cause it to develop is often a mystery. What are known are those things that most often can contribute to an outbreak of the disease. Most explanations for the appearance of the disease center on clogging of the skin pores. The clogging of a pore usually involves dead skin cells. Normally, these dead cells will rise to the top of the pore where they are then shed. What sometimes happen however is that sebum, the oil that lubricates the skin, can cause these dead skin cells to stick together and become trapped within the pore.

Chemical peels can be used to reduce the appearance of acne scars.[31] Mild peels include those using glycolic acid, lactic acid, salicylic acid, Jessner’s solution, or a lower concentrations (20%) of trichloroacetic acid. These peels only affect the epidermal layer of the skin and can be useful in the treatment of superficial acne scars as well as skin pigmentation changes from inflammatory acne.[31] Higher concentrations of trichloroacetic acid (30–40%) are considered to be medium-strength peels and affect skin as deep as the papillary dermis.[31] Formulations of trichloroacetic acid concentrated to 50% or more are considered to be deep chemical peels.[31] Medium-strength and deep-strength chemical peels are more effective for deeper atrophic scars, but are more likely to cause side effects such as skin pigmentation changes, infection, and small white superficial cysts known as milia.[31]

I use this on all of my acne spots. I love it. It does dry the skin out a bit, but if you moisturize daily it doesn’t make a difference. An ice pick scar is formed when an inflamed blemish reaches the surface of the skin. When the blemish heals, the follicle empties, forming a column-like scar that runs from the surface to the inner layer of the skin.

Topical antimicrobials, including benzoyl peroxide and antibiotics, are effective in treating inflammatory disease.3,4 Benzoyl peroxide is a bactericidal agent that prevents the resistance of P. acnes to antibiotic therapy9 and has moderate comedolytic and anti-inflammatory properties. It is available in various topical preparations, ranging in strength from 2.5% to 10.0%. Any strength can be used initially, although it may be more prudent to start with a lower concentration; stronger preparations are more irritating and not necessarily more effective.10 Benzoyl peroxide kills P. acnes by releasing oxygen within the follicle. It can be fast-acting, with a response as early as five days.4 The main drawback is that it is a potent bleaching agent.2 Patients should be warned that fabrics that come in contact with benzoyl including towels, bed sheets and clothing, may be bleached.

In their absence, an appearance similar to that of acne would suggest a different skin disorder.[27] Microcomedones (the precursor to blackheads and whiteheads) are not visible to the naked eye when inspecting the skin and can only be seen with a microscope.[27] There are many features that may indicate a person’s acne vulgaris is sensitive to hormonal influences. Historical and physical clues that may suggest hormone-sensitive acne include onset between ages 20 and 30; worsening the week before a woman’s menstrual cycle; acne lesions predominantly over the jawline and chin; and inflammatory/nodular acne lesions.[1]
