Isotretinoin (eye-soh-tret-in-OH-in) is a prescription medicine for severe acne. This type of acne causes deep, painful cysts and nodules. These can be the size of a pencil eraser — or larger. As this acne clears, scars often appear.

There is no clear evidence that acne vulgaris is related to poor hygiene or that frequent face washing lessens acne. Patients should be instructed to wash their face gently with warm water and mild soap twice daily; rough scrubbing can cause new lesions because of follicular rupture. The only antibacterial soaps that may be effective are those containing benzoyl peroxide.19

The earliest pathologic change is the formation of a plug (a microcomedone), which is driven primarily by excessive growth, reproduction, and accumulation of skin cells in the hair follicle.[1] In normal skin, the skin cells that have died come up to the surface and exit the pore of the hair follicle.[10] However, increased production of oily sebum in those with acne causes the dead skin cells to stick together.[10] The accumulation of dead skin cell debris and oily sebum blocks the pore of the hair follicle, thus forming the microcomedone.[10] This is further exacerbated by the biofilm created by P. acnes within the hair follicle.[44] If the microcomedone is superficial within the hair follicle, the skin pigment melanin is exposed to air, resulting in its oxidation and dark appearance (known as a blackhead or open comedo).[1][10][19] In contrast, if the microcomedone occurs deep within the hair follicle, this causes the formation of a whitehead (known as a closed comedo).[1][10]

Luckily, there’s a dermatologist who’s garnered quite the acne-obsessed cult following: Sandra Lee, MD, a board-certified dermatologist in Southern California—aka Dr. Pimple Popper. Her YouTube channel is crazy popular, recently logging its billionth view. In her videos, not only does she educate people on what acne is, but she features footage of all sorts of skin issues erupting, including blackheads, pustules, cysts, and more. (There’s a “graphic and shocking” warning that precedes her videos—fair warning!)

The best approach to treatment requires some patience as it involves starting with milder over-the-counter medications or home remedies and then progressing to the stronger prescription drugs if satisfactory results are not realized. In general, the risk of side effects increases with the strength of what is being used as treatment. If you need to progress to the point where requiring an isotretinoin prescription is a possibility, there will be some decisions that have to be made, and these decisions should involve discussing with a dermatologist the pros, cons, and risks involved.

Dermatologists take these reports seriously. That’s why dermatologists are conducting research studies to find out whether isotretinoin is directly responsible. Proving or disproving this is not as easy as it sounds. One reason is research shows that people who have severe acne often become depressed. This is true for people who have never taken isotretinoin.

Access to subsidised oral isotretinoin for severe acne, was widened on 1 March, 2009, to allow vocationally registered General Practitioners and Nurse Practitioners working in an appropriate field to prescribe the medicine, fully subsidised, subject to Special Authority criteria.15

Acne responds well to melalecua, more commonly known as tea tree oil. It’s used the world over as an antiseptic and to treat wounds. Like coconut oil, honey and cultured milk products, it fights bacteria and fungi.

It can also overstimulate growth in the human embryo, and cause birth defects if taken during the first month or pregnancy. And a significant problem with Retin-A is that it can stimulate so much growth in the skin that new skin blemishes occur.

Coconut oil is all the rage, with uses ranging from hair conditioning to cooking. But some swear by it as a natural acne treatment. To use coconut oil as an acne treatment, you can include it as part of a healthy diet. The fatty acids like lauric acid caprylic acid are metabolized into antibacterial agents in the body. Or, you can apply a very small amount and rub directly onto your skin after cleansing for an extra hydrating boost.

Acne treatment that works throughout the body: Medicine that works throughout the body may be necessary when you have red, swollen types of acne. This type of treatment is usually necessary to treat acne cysts and nodules. Your dermatologist may prescribe one or more of these:

Birth control pill (females only): A birth control pill can be an effective part of an acne treatment plan. When treating severe acne, a birth control pill may be used along with an antibiotic or a medicine called spironolactone (spy-ren-no-lac-tone) to get the acne under control.

As if that wasn’t enough bad news, Joshua Zeichner, the director of cosmetic and clinical research in dermatology at Mount Sinai Hospital in New York City, says the number of woman who will experience acne at some point during adulthood is rising. “It is unclear exactly why this is happening, however it may be due to a combination of stress, dietary factors and changing hormones,” he said.

Use sunscreen every time you’re in the sun. Read the ingredients list on the back of your sunscreen, and if you’re acne-prone, look for lighter chemical ingredients like avobenzone, oxybenzone, methoxycinnamate, octocylene, and zinc oxide.

The Food and Drug Administration warns that some popular OTC acne products can cause a serious reaction, including throat tightness and swelling of the face, lips or tongue. This type of reaction is quite rare, so don’t confuse it with the redness, irritation or itchiness that may occur where you’ve applied such products.

There are no home remedies that are likely to have any effect on cystic acne. It is unwise to postpone treatment waiting for some nonprescription medication to produce improvement while scarring becomes worse.

If you squeeze, pop, or pick your acne—with fingernails, pins, or anything else—your skin will take longer to clear up and you may be left with acne scars.1 In general, try not to touch your skin too much as this can cause flare-ups.1

The good news is that whatever parts of your face are having a little high-school reunion, your body is probably in the clear. Oil glands on the chest and back typically become less active with age, so if you’re still suffering from a bumpy chest or bacne, a change of wardrobe may be in order. “The times I see increased body acne in adults are more commonly in women who work out and who wear spandex or tighter-fitting clothing,” Day says.

If you try a doctor-prescribed acne regimen for at least a month and you’re still experiencing severe breakouts, there’s one treatment that’s very effective — but it has significant side effects. Isotretinoin (commonly referred to as Accutane, a brand that was taken off the market in 2009) can help with almost all types of acne, though it may be slightly less likely to clear up hormonal acne, says Zeichner. A type of oral retinoid, isotretinoin works by “significantly reducing oil production from your sebaceous glands, which subsequently reduces the amount of P. acnes bacteria on the skin,” says Zeichner. It also has an anti-inflammatory effect. (If you’re interested in finding out more, ask your doctor about the isotretinoin brands that are available now, such as Claravis, Amnesteem, Myorisan, Zenatane, and Absorica.) Zeichner says that all patients who finish a course of isotretinoin (typically lasting five months, but many physicians recommend a longer treatment plan) will be “significantly better than when they started.” For the majority of patients, that can mean 100 percent clear skin. “Approximately 20 percent of patients will need a second course, 5 percent will need a third course, and one percent will need more than three,” says Zeichner. But even if you end up in the not-completely-clear camp, the post-treatment breakouts you experience will likely be “much more manageable with traditional treatments, like topicals and oral antibiotics,” says Zeichner.

Do not wash more than normal. Twice a day is normal for most people. Use a mild soap and lukewarm water. (Very hot or cold water may worsen acne.) Do not scrub hard when washing acne-affected skin. Do not use abrasive soaps, cleansing granules, astringents, or exfoliating agents. Use a soft washcloth and fingers instead. Excess washing and scrubbing may cause more inflammation and possibly make acne worse.

I tried all of these things and they just made my fussy sensitive skin worse. Retin-A dried my skin out to the point that it was flaking off and what was underneath was raw or even worse- open sores. Even with all of that drama, I still had horrible acne!

Diet therapy has been suggested. Fulton et al performed a study on chocolate, having teenage patients with acne consume 1 bar of chocolate each day. Some of the patients improved and some worsened, but the vast majority were unchanged. [67] This study helped decrease the emphasis on diet as a causal factor in acne vulgaris. However, investigators always returned to the diet question. Data suggest that the westernization of certain Native American populations and the related consumption of unhealthy “junk” foods (eg, potato chips, soft drinks) has had a negative impact on general and skin health, resulting in acne flares. Skim milk, compared with full-fat milk, has been found to have a positive association with acne, especially in teenagers and young adults. [68, 69] Causation was not able to be drawn from the case control studies, but the mechanism involving hormonal constituents of the skim milk has been postulated. [68] . Whey protein is a commonly used supplement that has been suggested to worsen acne and should be discontinued if flaring of the condition is associated with its use.

High-glycemic foods: These are foods that break down quickly in the body, triggering an insulin spike and raising blood sugar levels. I’ve had acne since I hit puberty and I had an early puberty. Yeah I hit it hard. I had really bad acne and I just hated it. My mother told me it was a phase and I’d get over it. I was so self conscious that I just hated getting my pictures taken and everything. My mom would also make me Chinese remedy to help my acne (didn’t work too well), took me to the doctors (the drugs didn’t work), many over the counter acne cleanser and what not (went through a lot), I even ordered Proactiv and I thought “Hey, for all skin…maybe it’ll work” Just made my skin more irritated and didn’t work. I just gave up and somehow I came across this site and said “Well…might as well give it one last try.” I’m a soon to be 19 year old, first year college student and I started January 7th, and so this is my 4th week into it and my skin is CLEAR! For the first time, I took a picture of myself, took it into photoshop, fixed the colour of the picture (since my camera isn’t the greatest) and for the FIRST TIME I didn’t have to air brush my skin. People who have mild acne have a few blemishes. They may have whiteheads, blackheads, papules, and/or pustules (aka pimples). Many people can treat mild acne with products that you can buy without a prescription. A product containing benzoyl peroxide or salicylic acid often clears the skin. There are two ways acne treatment systems can work for you in ways that individual acne treatment products cannot. One is by enhancing the effects of your best individual acne products. Green tea extract, for example, reduces the production of oil in the sebaceous glands underneath the skin. Acne occurs when the skin’s pores become clogged. Each pore opens to a hair follicle containing a gland that produces oil called sebum, which helps keep skin soft. These follicle-gland units are largest and most numerous on the face, upper back and chest. When the glands produce too much oil, the pores can become blocked. As a result, dirt, bacteria and dead skin cells can build up in the pores, forming the whiteheads, blackheads, pimples and other lesions that are commonly referred to as zits.

“Overwashing your face can make acne worse,” Dr. Kazin explains. But Accutane has mixed reviews for a reason. It makes the skin super dry and sensitive, which means it’s important to keep moisturizers and lip balm nearby while you’re on the treatment. Oh, and don’t even think about waxing your eyebrows (just imagine your skin ripping off). There’s another downside to Accutane: It requires a lot of paperwork and office visits. The thought from researchers is that while the exact cause of acne is still not known, there are suggestions that it may be both genetic and environmentally caused. I think that the toothpaste works the best. The icecube is very helpful though, about 30 minutes before work or school, just put an icecube on your pimple, and it will be less redin about 10 minutes. So the icecube does work, but not to clear acne for me. I have heard to use garlic though, so I will be trying that with some crushed garlic. For those who need more tips I suggest:

Chemical peels may be performed at home, with an esthetician, or with a dermatologist, and are one of the most common treatments used for getting rid of acne scars. The peel is in fact application of gentle acid on the surface of the skin. The higher the acidity level, the further the peel goes into the skin. Alcohol: limit your alcohol, it is not necessary that you omit it altogether. Be aware that drinks like tequila and vodka do not contain sugar so should not affect your skin. The first three types listed are all types of the most common form of acne – acne vulgaris. Blackheads are a mild form of acne that appear as unsightly, open pores that appear darker than the skin surrounding them. The demonologists advice was not always right, I learned from my own experience a way to get rid of all your acne in about a week, basically focusing on certain diet principles and high level of water intake but it’s more developed than that. But it depends , if your acne is hormonal or caused by diet, the results will vary.
