Some people who use this drug are more sensitive to sun after they eat celery or celeriac or after they take St. John’s wort. “Things are so much better today because there are so many more options for treating acne,” said Sarah Taylor, MD, a dermatologist with Wake Forest Baptist Health. “While OTC products are pretty much the same as they have been for years – just different concentrations of benzoyl peroxide and salicylic acid in various forms such as cleansers, gels and creams – the prescription world has really changed in the past 10 years or so. What it is: Originally under the brand name “Accutane” but now available only in generic form, isotretinoin is a derivative of vitamin A taken in pill form for 15-20 weeks. Doctors normally prescribe it for people with “severe nodular acne” that does not respond to other treatments. Nodules are inflammatory lesions with a diameter of 5mm or more. It may take a team effort to defeat acne once and for all. In 2010, researchers surprised some people when they reported that diet could indeed, affect acne outbreaks. That year, an article in the scientific journal Skin Therapy Letter reported the results of a 27-study analysis—21 observational studies and 6 clinical trials. A report presented by New York University and published in The Academy of Nutrition and Dietetics states that eating high glycemic foods like bakery goods and sweet beverages that are high in sugar content causes pimples (5). Having too many dairy products is also not good for your skin, though they are high in calcium. These pimples can be corrected by following the right diet.

Mild acne is a very common thing and goes away in time, normally without any scarring. If you start to develop red spots, pustules or rashes, it is advisable to start treatment as early as you can, to prevent the acne worsening. Treatment will eventually clear your acne it if is used correctly, however you may always have a breakout due to outside influences.

Benzoyl peroxide is available as a gel, cream or cleanser, and ranges in strength from 2.5% – 10%. The 2.5% strength is sufficient for most people, and higher concentrations are associated with greater adverse effects. Also, waiting to treat acne until it becomes severe can lead to extensive scarring, so it’s best to treat it sooner than later. (5)

And cruising through the aisles at the drugstore, you’ll find it as the active ingredient on the majority of products labeled “acne wash” or “spot treatment.” Salicylic acid is a beta-hydroxy acid that works by dissolving excess oil and gently exfoliating away dead skin cells. Salicylic also has anti-inflammatory properties to help with inflamed cystic breakouts that can occur when blockages deep in the hair follicles rupture beneath the skin. It’s best to apply this ingredient as a toner, moisturizer, or leave-on spot treatment instead of a face wash to give it time to do its work. And keep in mind, salicylic acid can dry out the skin if over-applied, so maybe choose only one product with the ingredient to use every day.

Dermatologists prescribe isotretinoin for severe acne. Severe acne is often very painful. When the painful cysts and nodules of severe acne clear, they leave permanent scars. Many people who have severe acne feel depressed. Some feel anxious. Low self-esteem often develops. Grades can plummet, and job performance can suffer.

If your acne is severe, painful, or refusing to get lost, you may just be beyond what an over-the-counter treatment can do. Ask your doctor before using other medication on the skin, especially anything containing sulfur, resorcinol, or salicylic acid, as the combination may be too irritating. Abrasive soaps and cleansers should be used with caution for the same reason.

Inflammatory bowel disease (IBD) is another concern. There have been reports of patients developing IBD while taking isotretinoin. Again, there is not enough scientific evidence to determine whether isotretinoin is actually the cause. More research is needed.

There are now three oral contraceptives that are specifically approved by the FDA for the treatment of acne in women: Yaz, Estrostep, and Ortho Tri-Cyclen. Only pills that combine the female hormone estrogen with the synthetic version of the male hormone progesterone, progestin, can stabilize hormonal fluctuations in a way that can treat acne.

Although acne is common in teenagers, it isn’t just a teenage affiction.3,9 Adult acne may be more common than previously thought.9 About one-third of people seeking treatment for acne are 25 or older.9

The recommended starting dose of isotretinoin is 0.5 mg/kg/day. Daily doses can be titrated up or down, between 0.1 and 1 mg/kg/day, depending on response to treatment and presence of adverse effects. The total dose of isotretinoin over the course of treatment should be between 120 – 150 mg/kg of body weight.14 The of treatment is dictated by the daily dose. Courses usually last between three to four months but may continue for one year or longer. Total doses of >150 mg/kg are associated with increased adverse effects.16 The dose and duration of the course may be adjusted on an individual basis; some patients will wish to reduce the dose or stop isotretinoin when the active acne has settled.

Wear sunscreen everyday. The sun is very damaging to your skin and many acne medications make your skin more sensitive to the sun.[5] Use a broad spectrum sunscreen with an SPF of 30 or higher. Many moisturizers already have sunscreen in them. Just make sure that it protects against both UVA and UVB rays. Apply your sunscreen after you have washed your face and applied any topical acne medication.

For more severe forms of acne, you may need prescription medication. Dr Wong explains: “This can range from antibiotics which you apply as a cream or a gel, or your doctor could prescribe antibiotic tablets, for courses of three to six months. “There are also stronger tablets, which people can take and would be done under the supervision of a dermatologist.”

What it does: Dapsone targets the inflammatory component of acne by helping to inhibit the body’s inflammatory response. Treatment is normally no different than that for the milder acne forms. Over-the-counter products and even some home remedies can be effective, but since this type is relatively severe, it may be necessary to visit a dermatologist if the products or home remedies you are using are not effective. Many, though not all, over-the-counter products contain three important agents used in fighting acne:

During pregnancy, especially during the first trimester, estrogen and progesterone levels increase therefore increasing the skin’s production of sebum. Once the spots have cleared, acne commonly flares up again if you stop treatment. Therefore, after the spots have gone or are much reduced, it is common to carry on with a maintenance treatment to prevent acne from flaring up again. It is common to need maintenance treatment for 4-5 years to keep acne away. This is typically until the late teens or early 20s. In a small number of cases, acne persists into the 30s, or even later. For these people it is possible to continue to treat the skin to keep it under control.

Isotretinoin affects all causative mechanisms of acne — it changes abnormal follicular keratinization, decreases sebum production by 70%, decreases P. acnes colonization and is anti-inflammatory.38 Indications for isotretinoin include scarring disease, severe nodulocystic acne and less than 50% improvement with oral antibiotics or hormonal therapies after four months.38 Isotretinoin therapy must be monitored carefully because adverse effects include potent teratogenicity, hypertriglyceridemia and pancreatitis, hepatoxicity, blood dyscrasias, hyperostosis, premature epiphyseal closure and night blindness. An association with severe skin reactions, such as erythema multiforme, Stevens–Johnson syndrome and toxic epidermal necrolysis, has been reported.39 Although a causal relationship has not been shown, patients must be warned about depression, suicidal thoughts and psychosis, and monitored closely.40

Washing your face everyday using face scrubs and face washes twice a day really help. I’ve seen a massive difference. I love using ‘Neem’ (tee tree herbal face wash) and it works quickly. About less than a month or 2 weeks. (for me)

See a dermatologist. You may have tried all of the over-the-counter products and nothing has worked. A dermatologist can assess your skin and develop a plan to treat your acne. There are many options and medications for acne treatment nowadays. You can control your acne, prevent future breakouts, and reduce your chances of scarring and dark spots.[7]

Doxycycline 50 – 100 mg, daily, for four to six months, is the first-line antibiotic choice (N.B. 50 mg tablets are not fully subsidised).6 If effective, the dose can be tapered after four months to alternate day treatment. If the standard dose of doxycycline is ineffective, increase to 100 mg, twice daily, provided it is tolerated. Doxycycline is contraindicated in children aged under 12 years and women who are pregnant.

Use prescription drugs. Certain medications can be prescribed by your dermatologist to help with particularly bad cases of acne, but should be used with caution. As with all medications, there may be unwanted side-effects in a small portion of the population.

Simple way to get rid of acne is using home remedies. Tea tree oil or the straight oil is an alternative of benzoyl peroxide cream and prevent from mild acne. 2. Keep a diary. Recording lifestyle factors such as frequency of breakouts, menstrual cycles, diet (sugar, dairy and GI foods) stress levels etc may help you and your medical professional identify triggers. 


When topical agents are insufficient or not tolerated, or in cases of moderate to severe acne, especially when the chest, back and shoulders are involved, systemic antibiotics are often considered the next line of treatment (Table 3).20,21 However, regular use of a combination of topical antibiotics and benzoyl peroxide may be similarly effective, as shown in a randomized controlled trial of five antimicrobial regimens.22

Washes can be used once or twice daily, and should be applied for thirty seconds before rinsing thoroughly. Some patients may tolerate twice daily applications.

Keep your away from your face. Think about all of the things you touch throughout your day. When you touch your face, you spread germs and bacteria to your face. This can lead to flare-ups and breakouts.[6]

Sometimes shoulder acne is just a reaction to tight-fitting clothes or shoulder pads, but often shoulder acne is a sign of a much more serious form of this common skin condition. Shoulder acne can be especially severe because the pores on the shoulders are large and pimples can grow especially large.

Drink enough water: The old adage that water is good for your health is true. In addition to that, water is also fantastic for your skin. By drinking two litres of water per day as recommended by doctors, you end up flushing away toxins from your body. An unintended effect of limiting access to isotretinoin was an increase in health disparities due to the limited access to publicly funded and private Dermatologists in New Zealand.25 The result was that a person living in the least deprived socioeconomic area was 2.5 times more likely to be prescribed isotretinoin than a person living in the most deprived area, and that Māori and Pacific peoples were five times less likely to be prescribed isotretinoin than people in other ethnic groups, largely New Zealand Europeans.25 This is despite the fact that there is no significant evidence of an association between the incidence of acne and deprivation level or ethnicity.25, 26 One of the primary reasons for widening access to isotretinoin was to reduce this disparity.

Teenagers first develop acne in response to their body’s production of androgens. Both men and women have androgens present in their body, and while this flux occurs, skin can breakout into pimples and cysts. This is because androgens help produce sebum, which is the oily substance your skin secretes to keep it moist and hydrated. Oil on your skin is actually a good thing, because it keeps your skin from drying out and cracking—no desert skin here.

If you are spot prone anyway, I would like to recommend Sudocrem. Forget the “put a tiny bit just on the spot”, you can spread it all over your face if you want, it won’t harm you, but it does reduce and clear spots very well if you leave it overnight. Unfortunately after a while, it doesn’t seem to work as well as the first few times you use it, but it still does help. Some patients only require one course of oral isotretinoin for complete acne remission, while others require additional courses of isotretinoin therapy. A study found 38% of the patients had no acne during 3-year follow up, and, among the remaining patients, 17% were controlled with further topical therapy, 25% with topical and oral antibiotics, and 20% with second course of isotretinoin. If you have acne, chances are pretty good that you also tend to get blackheads — they’re caused by oil and debris that has darkened within clogged pores. To battle them without causing irritation, start by degunking with an exfoliator with 2 percent or less salicylic acid or glycolic and lactic Acids every morning.
