Other concerns include inflammatory bowel disease and the risk of depression and suicide in patients taking isotretinoin. Recent evidence seems to indicate that these problems are exceedingly rare. Government oversight has resulted in a highly publicized and very burdensome national registration system for those taking the drug. This has reinforced concerns in many patients and their families have that isotretinoin is dangerous. In fact, large-scale studies so far have shown no convincing evidence of increased risk for those taking isotretinoin compared with the general population. Just cutting out the damaging foods listed above will likely lead to clearer skin—especially if you were regularly consuming them before. But what if you’re already eating healthy? Folliculitis: Pimples can occur on other parts of the body, such as the abdomen, buttocks, or legs. These represent not acne but inflamed follicles. If these don’t go away on their own, doctors can prescribe oral or external antibiotics, generally not the same ones used for acne.

I would be careful using a dehumidifier – sometimes the opposite is needed (a humidifier). “Things are so much better today because there are so many more options for treating acne,” said Sarah Taylor, MD, a dermatologist with Wake Forest Baptist Health. “While OTC products are pretty much the same as they have been for years – just different concentrations of benzoyl peroxide and salicylic acid in various forms such as cleansers, gels and creams – the prescription world has really changed in the past 10 years or so. We’re much better equipped to deal with all different types of acne.”

When it comes to skin health, you are what you eat. Consuming too many greasy foods like pizza will make your skin overproduce oils that can lead to clogged pores. Processed foods will also put unnatural chemicals and free radicals in the body that leaves skin dull, puffy or irritated.

Isotretinoin has a high risk of inducing birth defects if taken by pregnant women. Women of childbearing age who take isotretinoin need two negative pregnancy tests (blood or urine) before starting the drug, monthly tests while they take it, and another after they are done. Those who are sexually active must use two forms of contraception, one of which is usually the oral contraceptive pill. Isotretinoin leaves the body completely when treatment is done; women must be sure to avoid pregnancy for one month after therapy is stopped. There is, however, no risk to childbearing after that time.

Adverse psychological issues have been associated with isotretinoin, particularly depression and suicidality, but causality has not been established. Depression may be present before treatment or can occur for unrelated reasons.17 A brief psychological assessment for depression and suicidal tendency should be performed prior to prescribing isotretinoin, and then briefly whenever the patient is seen during and after treatment.

Investigators have also focused on a low-glycemic diet to avoid stress from high-carbohydrate diets and to reduce insulin levels. A study found that 12 weeks of a high-protein, low–glycemic load diet decreased the total amount of acne lesions, and this may lead to dermatologists recommending the South Beach Diet to patients with acne. [19] However, guideline groups have not found benefit in acne treatment or control with dietary restrictions. [29]

Omega-3-fatty acid supplementation is useful for inflammatory acne in about 2/3 of the cases as this study showed. Here is a patient from this study who benefitted from omega-3 supplementation. Thebaseline image is seen with inflammatory acne lesions on his cheek. Only 12 weeks after taking 3 Grams of omega-3 supplementation daily his face looked much improved.

All the dermatologists we talked to agreed that there is no one-size-fits-all solution to acne. Every patient responds to treatments differently, and sometimes it can get worse before it gets better. But with the help of your dermatologist, you can find an acne treatment regimen that works for you. And, yes, we do stress how helpful it is to work with a derm to get it right.

The general rule is 4 to 8 weeks although it can take as long as three months, and symptoms may get worse before they get better. It is easy for patients to get disheartened and give up so it is important to persevere and keep any routines going, even if there is no immediate change for the better.

The most popular over-the-counter products, such as Proactiv, contain benzoyl peroxide but at lower concentrations than most prescription-strength products. Proactiv, a system of cleansing products in which benzoyl peroxide 2.5% is the active ingredient, is claimed to enhance compliance by providing a cosmetically elegant product that also minimizes irritation. The makers of Proactiv also market Gentle Formula, which replaces benzoyl peroxide with salicylic acid for people with allergy or intolerance. There have been few studies assessing the efficacy of the Proactiv system. In one open-label study of 23 patients with mild to moderate acne, inflammatory lesions were reduced by 39% in patients using a combination of butenifine (an allylamine) and benzoyl peroxide compared with 34% in those using Proactiv.18

Once acne is resolved, how do dermatologists remove acne scars? A peel might be recommended to remove the appearance of dark spots or scars, such as a glycolic peel. Peels and other acne treatments can increase photo-sensitivity, so you’ll need to protect your skin from the sun.

Use oil-free makeup. If you wear makeup, you may be stuck in a vicious cycle of covering up acne while simultaneously causing it with your cover-up usage. Find acne-fighting oil-free mineral makeup to help prevent worsening your acne while simply trying to hide it. Power foundations are also recommended. When possible, avoid wearing make-up at all though as it clogs your pores over the course of the day.

Environment makes a difference. Sometimes humidity or dry air and disrupt your skin. If you noticed a change after a move, this may be the culprit. Unfortunately this is hard to handle without moving again – but maintaining balanced skin with good products should be the aim. Eventually it should acclimatize.

Low-dose prednisone: This is a super potent corticosteroid. Dermatologists prescribe it to treat a very serious type of severe acne called acne fulminans. It’s also used to treat pityrosporum folliculitis, an itchy, acne-like rash.

This type of acne is more common in men than women. When women do get cystic acne, they often experience outbreaks at certain intervals during their menstrual cycle, and often on the lower half of their face. Prompt treatment by a dermatologist can minimize scarring. In some cases, a doctor may inject corticosteroids directly into nodules and cysts to reduce the lesion’s size, as well as painful inflammation.

Considerations: Regardless of the type of antibiotic prescribed, only about one half of patients respond. When antibiotics do produce results, these results are moderate at best.5-8Oral antibiotics should be used for only a short period of time, up to 6 months. However, even within this short time frame, antibiotics have been implicated in the proliferation of resistant colonies of bacteria. Some antibiotics cause increased sensitivity to sunlight. People with acne-prone skin don’t just have a physical condition to contend with, they can also be affected psychologically. The presence of spots and pustules on the face are difficult to hide so it is common for sufferers to lose self-confidence and self-esteem.

Lortscher says to resist all temptation to pop a pimple when it first appears. This can be hard, because we assume that popping it means eradicating all the gunk, and that this expedites healing. Wrong: “When you pop a zit, the gunk inside can make its way to the skin’s surface and irritate your skin,” he says. “This leads to even more pimples nearby. While acne usually affects teenagers and 20somethings the most, anyone of any age can get acne, even babies. Our instinctual response — picking at it, rubbing it and canceling our social engagements — doesn’t help in the least, but fortunately, there are some treatments available that can.

Current consensus recommends a combination of topical retinoid and antimicrobial therapy as first-line therapy for almost all patients with acne. [2] The superior efficacy of this combination, compared with either monotherapy, results from complementary mechanisms of action targeting different pathogenic factors. Retinoids reduce abnormal desquamation, are comedolytic, and have some anti-inflammatory effects, whereas benzoyl peroxide is antimicrobial with some keratolytic effects and antibiotics have anti-inflammatory and antimicrobial effects. [2]

Unfortunately, many prescription and over-the-counter acne treatments come with a high risk of birth defects so avoiding them is both a necessity and responsibility we have to take very seriously. Below is information that can help keep you and your unborn baby safe.

Before prescribing isotretinoin, the dermatologist looks at the test results. If the results are okay, the dermatologist will discuss possible side effects of isotretinoin. After learning about isotretinoin, a patient must decide whether to take this medicine.

The exact cause of acne is unknown, but doctors believe it results from several related factors, including bacteria and clogged pores, genetics and hormonal changes.8 Stress doesn’t cause acne directly, but research suggests that for people who have acne, stress can make it worse.8

Don’t touch your face. This is incredibly difficult, as people are prone to touching their faces on a regular basis. Be careful about scratching your face, resting your face on your hands, and picking at pimples. Never pop your zits or squeeze at pesky blackheads, as this only introduces more bacteria to your skin and will make your acne worse.

All cases of acne are not created equal, and neither are all acne treatments. Although all acne has its roots in the same process — hormonal fluctuations that stimulate oil production — not all acne is equally severe and not all cases of acne will respond to the same types of treatments.

Food: Parents often tell teens to avoid pizza, greasy and fried foods, and junk food. While these foods may not be good for overall health, they don’t cause acne or make it worse. Although some recent studies have implicated a high-carbohydrate diet, milk, and pure chocolate in aggravating acne, these findings are very far from established.

The medical term for pimples or acne is “acne vulgaris”. For years it has been postulated that hormones and medication can cause acne. According to Ref.1 there are

steps that work together in causing acne. The hair follicle and sebaceous gland work as one unit. Male hormones, called androgens play an important role in the development of acne, both in males and females. Testosterone in males is not only produced in testicles, but also in the skin itself. It gets converted by an enzyme, 5-alpha-reductase, into the much more active metabolite dihydrotestosterone. In individuals with hypersensitive receptors in the sebaceous gland this will cause blockage in the sebaceous gland duct and at the same time stimulate the sebaceous gland oil production leading to the formation of a keratotic plug. White heads and black heads are formed this way. Contributing factors are inflammatory substances that are caused by insulin release stimulated by sugar, wheat and starch intake. This stimulates IGF-1 receptors in the skin, which causes growth of the subcutaneous skin layers, which is pushing up from the layer below the skin, kinking the sebaceous gland duct and causing acne pustules (pimples) to form. A skin bacterium, called Propionibacterium acnes (P. acnes), is getting trapped in the pimple causing a local skin infection, which in turn can cause acne cysts and furuncles, particularly in males where there is a family history of acne. High cortisol levels from stress can also be a contributing factor in causing acne. Today’s teenagers are exposed to a lot of stresses from exams, competitive sports and peer pressures.

In 2007, a study published in the American Journal of Clinical Nutrition found that glycemic load can greatly affects acne. 43 males with acne, aged 15 to 25, were separated into two groups. For twelve weeks, one group ate a diet that was 25 percent protein and 45 percent low-glycemic carbohydrates. The other group ate carbs without any control of glycemic index, resulting in a higher glycemic diet. At the end of the study, the acne had decreased in the low-glycemic group by almost twice the rate of the high-glycemic group! Considerations: Unlike oral dapsone which is sometimes prescribed for leprosy and other skin infections, topical dapsone does not come with potentially dangerous blood related side effects.4-5Side effects of topical dapsone are normally mild in nature and can include dryness, rash, and burning.6

These side effects contribute to why isotretinoin is often a last resort for curing severe acne. Extremely dry skin is one of the most common effects of the drug, but since it became FDA-approved to treat acne in 1982 it has also been linked to inflammatory bowel disease, depression, and increased rates of suicide. While studies have not proven that isotretinoin causes these conditions, they remain among the risks of taking the medication. Other serious but rare side effects include joint and muscle pain, eyesight trouble, and liver damage. Due to the drug’s impact on the liver, patients are advised not to drink and their liver enzymes are monitored throughout treatment. Much to my high-school friends’ delight, this made me the designated driver the majority of our senior year. If I had much more than a beer, it would show in my blood work and I was admonished by my doctor and my mom—something no teen wants to experience.

Since everyone gets acne at some time, the right time to treat it is when it becomes bothersome or when the potential for scarring develops. Recent studies conducted by dermatologists in 2012 show that green tea has a positive impact on acne, when applied externally. Green tea has an antioxidant known as epigallocatechin-3-gallate (EGCG). It was observed that EGCG reduced inflammation, sebum production and bacterial growth on pimple-prone skin. Place 2 teaspoons of organic tea leaves in 1/2 cup of fresh boiling water. Steep for 5 minutes; strain the leaves and let the mixture cool. Then use a cotton pad to apply it on your face. You may also put it inside a spray bottle and spray it on. Leave it on overnight.

Even if you get acne, you don’t want to make it worse. That’s why it’s important to keep your hands off your pimples. Try not to touch, squeeze, or pick at a pimple. When you play around with pimples, you can cause even more inflammation by poking at them or opening them up. Plus, the oil from your hands can’t help! The worst part, though, is that picking at pimples may lead to scars on your face.

Antibiotics are moderately effective for acne and are frequently used for acne treatment. They are often prescribed for months or years, because acne is a chronic skin condition. However, many physicians are concerned about the use of antibiotics for acne, mainly because of reports of increasing rates of bacterial resistance to antibiotics.

Get a laser treatment. That’s right – use lasers to kill off your acne. Many dermatologists now offer a treatment in which they use lasers to fire strong blasts of light to kill overactive oil-producing glands beneath your skin. This process can be painful, but has been shown to cut down acne 50% on average.

People with skin prone to acne are usually sensitive to testosterone, a natural hormone present in both males and females. In such people, the testosterone triggers oversecretion of sebum, which leads to blockage of pores. Sometimes, dead skin cells get stuck to the sebum and make matters worse (4).

Some doctors suspect that sodium has consequences for skin, because the iodine frequently found in table salt and some seafood may worsen acne breakouts. Stick to low-sodium versions of packaged foods, try to keep your overall salt consumption below 1,500 mg a day, and stay away from these 10 healthy foods that are secretly loaded with salt.

First-line therapy is chosen based on the site and extent. Benzoyl peroxide kills P. acnes and mildly comedolytic ointments prevent the formation of comedones and blemishes.
