Combination therapy, for example with retinoids and antibiotics, is more effective than either agent used alone.13 However, the agents should be applied at separate times, unless they are known to be compatible.3 Benzoyl peroxide may oxidize a retinoid such as tretinoin if it is applied simultaneously.14 12-week randomized controlled trial involving 249 patients with mild to moderate acne showed treatment with adapalene gel 0.1% and clindamycin 1.0% to be superior to that with clindamycin 1.0% used alone.15 If inflammatory lesions are present, topical antibiotics containing benzoyl peroxide should be combined with a topical retinoid (e.g., topical antibiotic with benzoyl peroxide in the morning and retinoid at night). A review of three clinical studies with 1259 patients showed that a combination of clindamycin 1% and benzoyl peroxide 5% was more effective than either drug used alone in reducing lesions and suppressing P. acnes.16

Photodynamic therapy. “In this technique, we apply a prescription liquid to the patient’s face, chest, or back — wherever the acne is — and then apply a light or laser to activate the medicine,” Taub says. “Not only does the medication kill bacteria — which is less important, because bacteria will come back — but over a few months, it also reduces the size and activity of the oil glands.”

Oral isotretinoin (sold under a variety of different names) is used to treat cystic acne and commonly causes dry lips, skin and sun sensitivity. More seriously, it has been linked to birth defects, liver disease, severe depression and suicidal thoughts. Chinese medicine teaches that cysts and nodules associated with acne come from a build up of “phlegm” and “dampness.” Ice cream, cheese and milk are a direct cause of phlegm production. Sugar creates “dampness,” which lays the foundation for cystic acne to develop.

But the side effects of targeted spot treatments aren’t always worth it. “So many products instruct consumers to use benzoyl peroxide to spot treat red bumps and pustules. It’s best used to prevent red bumps and pustules, and applied all over the area you want to treat.” Robin Townsend, a medical aesthetician based in Cincinnati, was also quick to naysay a spot-treat-only approach: “Acne affects all of the pores. If someone is going to spot treat against my advice, I still suggest they spot treat one day and treat the whole face the next.”

Adapalene is available as a 0.1% cream and gel. It should be applied thinly, once daily.8 Adapalene is usually better tolerated than tretinoin.7 The gel is suitable for most people, although those with dry skin may prefer the cream.

OTC Medications: There are different types of over the counter (OTC) medications to get rid of acne. OTC medication doesn’t require doctor’s prescription and easily buy at pharmacy. Most OTC medication are applied to the skin. Common active ingredients found in OTC are:

While some acne types are incurable, almost all are treatable, and in the majority of cases, especially those cases of mild to moderate form, the condition goes away on its own. When the condition does go away, it may or may not return. In general, the prognosis is good. Patient compliance is very important however, especially if scarring is to be avoided. If you are experiencing this somewhat severe skin condition, you need to follow your dermatologist’s advice, or if you’ve found a medication or home remedy that appears to be working, you need to establish a treatment regimen and stick to it.

Oatmeal is much more gentle. Yet, don’t underestimate its properties. It contains saponins which is a a naural plant based cleanser that will help remove dirt and oil. It has also anti inflammatory and antioxydant properties that will help with red patches.

Considerations: Despite the frequent referencing of an initial flare of acne when starting is tretinoin, there is no science evidence to back this up. You should not expect an initial worsening of acne.3-4Exposure to sunlight should be minimized as you may be more sensitive to its rays. Ask your doctor before using other medication on the skin, especially anything containing sulfur, resorcinol, or salicylic acid, as the combination may be too irritating. Abrasive soaps and cleansers should be used with caution for the same reason. Oysters: Several studies have indicated that the mineral zinc may reduce the effects of acne. It’s best to get zinc from your food, however, as too much in supplements (more than 100 mg a day) can result in side effects. Eat more oysters, toasted wheat germ (sprinkled on salads and steamed veggies), veal liver, roast beef, roasted pumpkin and squash seeds, and dried watermelon seeds.

Isotretinoin is a powerful drug that’s used to treat the most severe cases of acne. Your doctor may recommend this drug if you have severe acne that doesn’t get better with other medications, including antibiotics.

The sebaceous glands or oil glands are where the acne form. These glands are more on the face, neck, scalp, and chest. Also, the face is more exposed to pollution and bacteria compared to the rest of the body. Both these factors work together to make acne mainly affect the face.

Antibiotics: Antibiotics can be effective in treating most inflammatory acne (papules and pustules). They work by decreasing inflammation caused by bacteria and other irritating chemicals present in the sebaceous follicle.

It’s best to consult a primary-care physician or dermatologist if an individual is unable to adequately control his or her acne. The goal of treatment should be the prevention of scarring (not a flawless complexion) so that after the condition spontaneously resolves there is no lasting sign of the affliction. Here are some of the options available:

A range of OTC preparations is available from pharmacies and drugstores, usually in the form of gels, pads, creams, lotions, and soaps. Most of these are topical treatments, for applying directly onto the skin.

P. acnes also provokes skin inflammation by altering the fatty composition of oily sebum.[44] Oxidation of the lipid squalene by P. acnes is of particular importance. Squalene oxidation activates NF-κB (a protein complex) and consequently increases IL-1α levels.[44] Additionally, squalene oxidation leads to increased activity of the 5-lipoxygenase enzyme responsible for conversion of arachidonic acid to leukotriene B4 (LTB4).[44] LTB4 promotes skin inflammation by acting on the peroxisome proliferator-activated receptor alpha (PPARα) protein.[44] PPARα increases activity of activator protein 1 (AP-1) and NF-κB, thereby leading to the recruitment of inflammatory T cells.[44] The inflammatory properties of P. acnes can be further explained by the bacterium’s ability to convert sebum triglycerides to pro-inflammatory free fatty acids via secretion of the enzyme lipase.[44] These free fatty acids spur production of cathelicidin, HBD1, and HBD2, thus leading to further inflammation.[44]

CBS News reports that Accutane has some pretty serious side effects, possibly even death. One man reported severe inflammatory bowel disorder that required removal of his colon. It’s been noted that it can cause miscarriage, birth defects, increased internal skull pressure, bone mineral density problems, depression, psychosis, suicide, aggressive or violent behaviors, acute pancreatitis, cardiovascular issues, deafness, hepatitis, bowel disease, excessive bone growth, night blindness and sight loss. Spot treatments are designed to give problem pimples a mega-dose of concentrated benzoyl peroxide — in a couple of regimens, like the Proactiv Teen Kit, the spot treatment had nearly three times the benzoyl peroxide as its all-over treatment. It makes sense: If benzoyl peroxide can be irritating to the skin in high concentrations, limiting its intensity to just the pimple itself could save the rest of your healthy skin.

Combined oral contraceptives containing less androgenic progestogens, such as drospirenone + ethinyloestradiol (Yaz and Yasmin, not subsidised) or desogetrel + ethinyloestradiol (Mercilon and Marvelon, partly subsidised) may also be suitable.10 However, these are more expensive to the patient than fully subsidised options, and have a similar risk of venous thromboembolism as cyproterone-containing contraceptives.

Researchers have found a relationship between stress and acne flare-ups. In response to stress our bodies produce more androgens (male hormone). These hormones stimulate the oil glands and hair follicles in the skin, which can lead to acne.

For most women, hormonal changes, either around the monthly cycle or during a menopausal shift, are the culprit. But dietary imbalances and stress also cause flare-ups. “Acne in adults is like a whistle blow. Often it’s a sign that something else not quite right is going on,” says Michael Murray, ND, a naturopath and coauthor of The Encyclopedia of Natural Medicine.

One can do a lot to treat acne using products available at a drugstore or cosmetic counter that do not require a prescription. However, for tougher cases of acne, one should consult a physician for treatment options.

Isotretinoin is a teratogen, so pregnancy must be avoided. Contraception counseling is mandatory, and two negative pregnancy test results are required prior to the initiation of therapy in women of childbearing potential. The baseline laboratory examination should also include cholesterol and triglyceride assessment, hepatic transaminase levels, and a CBC count. Pregnancy tests and a lipid panel and liver enzyme examination should be repeated monthly during treatment while dosing is changing. Once a level dose is used and the lipids, liver enzymes, and CBC count are normal, these tests may be discontinued. [31] Other adverse effects include dry skin, lips, and eyes; muscle aches; and headaches. Patients experiencing severe headaches, decreased night vision, or adverse psychiatric events should stop taking isotretinoin immediately. [31]

It can be used to treat hormonal acne in women (only) by reducing the production of androgens (male hormones) in a woman’s body, which can then reduce oil production in the skin. If you’re wondering how to get rid of acne overnight, keep in mind that aldactone can take up to three months to start taking effect.

Lortscher says to resist all temptation to pop a pimple when it first appears. This can be hard, because we assume that popping it means eradicating all the gunk, and that this expedites healing. A papule is a tiny pimple (less than .5 centimeters wide). It usually has a dome shape. Papules are sometimes called “pinheads.” Papules do not contain pus. A large number of papules may indicate moderate to severe acne.

Scientists initially hypothesized that acne represented a disease of the skin’s hair follicle, and occurred due to blockage of the pore by sebum. During the 1880s, bacteria were observed by microscopy in skin samples affected by acne and were regarded as the causal agents of comedones, sebum production, and ultimately acne.[121] During the mid-twentieth century, dermatologists realized that no single hypothesized factor (sebum, bacteria, or excess keratin) could completely explain the disease.[121] This led to the current understanding that acne could be explained by a sequence of related events, beginning with blockage of the skin follicle by excessive dead skin cells, followed by bacterial invasion of the hair follicle pore, changes in sebum production, and inflammation.[121]

If you have a healthy diet but you still have acne and inflammation, get tested for food allergies. Even mild allergies can cause breakouts or leave skin feeling too dry, oily, red or dull. Eliminating sugar, drinking plenty of water and getting your omega-3 foods can make a difference. Too much sugar can cause insulin spikes, which can create inflammation in the skin and clog up the pores. Water, conversely, hydrates, and it seems we cannot get enough of it. Make sure to get at least half your bodyweight in ounces every day.

Salicylic acid. For the same reasons, many experts also recommend against using topical treatments containing salicylic acid while pregnant. This is an ingredient found in almost all over-the-counter acne products with Proactiv®, NatureCure®, Clearsil® and other known brands. As for those who are not pregnant and/or do not plan on having a baby within the immediate future (up to 6 months), use of these products may have limited efficacy in treating acne but is not harmful (with the exception of possibly drying, irritating or prematurely aging your skin). Sun restrictions may also be necessary.

Differin (adapalene) is also used as a cream, gel, or lotion applied to the skin that has anti-inflammatory properties. As with Tazorac, the gel form is best for acne. Sometimes Differin is combined with benzoyl peroxide to prevent infection of surrounding skin as cysts and nodules open. The product works by keeping pores from being covered with new skin, and is best for people who need to keep cyst-prone skin clear.

Picking is a major skin sin. It can spread bacteria, cause additional breakouts, and even lead to permanent scarring. So try to keep your hands off! But if you absolutely can’t control yourself, here’s how to cope with the aftermath: Dab the area with witch hazel to disinfect it, then apply an antibiotic salve, like Neosporin, which will continue to fight bacteria and help the spot heal.

Inflammatory Acne: Inflammatory acne is red bumps and pustules, not whiteheads, blackheads and comedones. It does not necessarily start as them, either. It arises on its own. That’s good news because the British Medical Journal reports that acne affects more than 80 percent of teenagers and continues in adult life in 3 percent of men and 12 percent of women. Benzoyl peroxide. For mild acne, you may try, or your doctor may recommend, treatment with a nonprescription drug that contains benzoyl peroxide. Thick or greasy make-up may, possibly, make acne worse. However, most make-up does not affect acne. You can use make-up to cover some mild spots. Non-comedogenic or oil-free products are most helpful for acne-prone skin types.
