Pustules or Nodules: Sometimes, the tissues around the clogged pores get infected and cause inflammation. This inflamed portion is filled with pus, and it is hard. These are nothing but nodules or pustules.
Clinical trials have shown that estrogen-containing oral contraceptives can be helpful;31–33 the various formulations are thought to decrease levels of free testosterone by increasing sex-hormone-binding globulin and are considered equally effective. The choice of combined oral contraceptive should be based on a patient’s tolerance and potential side effects. It is not known whether other estrogen-containing contraceptives (e.g., vaginal rings, transdermal patches) are effective. Contraceptives containing only progesterone may worsen acne.4
“Eating chocolate or a lot of junk food doesn’t by itself seem to cause acne, but not having a balanced diet and eating too many refined carbs can cause problems,” says Albert Lefkovits, MD, director of the Park Avenue Center for Advanced Medical and Cosmetic Dermatology in New York City. In a 2007 study, Australian researchers found that people who followed a low-glycemic index (GI) diet (which is low in refined carbohydrates like those found in white bread) had a 22% decrease in acne lesions, compared with a control group that ate more high-GI foods. Scientists suspect that raised insulin levels from the carbs may trigger a release of hormones that inflame follicles and increase oil production. (Check out this handy list of low glycemic foods.)
“A woman’s hormones are kind of like a beautifully orchestrated symphony,” Richichi says. “In the right balance, a woman’s skin will glow.” She recommends nipping acne in the bud with a low-glycemic diet and cutting out refined carbohydrates, gluten and dairy — all of which spike up your insulin levels and, in turn, your androgen levels (see why here). “To me, acne is a symptom and birth control is just a Band-Aid, because birth control pills don’t address the underlying cause,” she says.
In severe cases, oral isotretinoin may be considered. This medication can be very effective but can also cause serious side effects including severe birth defects. Strict protocols must be followed. Monthly appointments with the treating doctor must be kept throughout the treatment period to monitor for any side effects. In females of child-bearing age, protocol includes two forms of birth control. The treatment period is usually five months.
Antibiotics used to treat acne can cause upset stomach, dizziness and discoloration of the skin. And, of course, overuse of antibiotics ultimately leads to antibiotic resistance, which has now become a global health issue.

A BHA product often cited includes salicylic acid, it must be between a pH between 3 and 4 to work. A BHA works to slough (to get rid of) off dead skin cells and encourage new skin growth. Less irritating than its chemical cousin benzoyl peroxide, tea tree oil has a long history of fighting mild to moderate acne outbreaks. The oil, which comes from the leaves of a tree native to Australia, has antiseptic properties that help reduce acne-causing bacteria on the skin and quell inflammation in skin cells. “We’ve seen it work against a wide range of organisms, including 27 of the 32 strains of acne-causing bacteria,” says Murray. tree oil in a wide variety of soaps, skin washes, and topical solutions. Look for a minimum concentration of 5% of the oil (up to 15% for more severe acne). Discuss the severity of the patient’s acne with them. Many people will perceive their acne to be worse than the clinical appearance suggests, and this should be taken into account in the treatment approach. The converse may also be true, e.g. a parent may perceive the acne to be worse than the patient believes or the physical findings suggest.
Taking baseline photographs of acne severity and lesion distribution is useful for assessing response to treatment, and encouraging medicine adherence.3 Most electronic patient management systems allow images to be added to the patient record.
“A true acne scar is when your body takes that healing process one step further and is not able to actually form the collagen the normal way. All the enzymes that are in the area of the acne lesion eat away at things like collagen and elastin,” explained Dr. Bowe. “In the case that the inflammation is so great, the collagen and elastic tissue is not able to regenerate completely, or it does in a sort of disorganized or pathologic and not healthy fashion, and you’re left with a scar.”
