The oral antiandrogen spironolactone can be added if oral contraceptives are not effective.3 Spironolactone is a 5α-reductase inhibitor when administered at higher doses.35 Spironolactone, used alone or as an adjunct at doses of 50–200 mg/d, has been shown to be effective in improving acne, but this is based on limited evidence.2 However, patients should be warned about possible side effects, including hyperkalemia, menstrual irregularities and feminization of a male fetus.
