Frequently used topical retinoids include adapalene, isotretinoin, retinol, tazarotene, and tretinoin. They often cause an initial flare-up of acne and facial flushing, and can cause significant skin irritation. Generally speaking, retinoids increase the skin’s sensitivity to sunlight and are therefore recommended for use at night. Tretinoin is the least expensive of the topical retinoids and is the most irritating to the skin, whereas adapalene is the least irritating to the skin but costs significantly more. Tazarotene is the most effective and expensive topical retinoid, but is not as well-tolerated. Retinol is a form of vitamin A that has similar but milder effects, and is used in many over-the-counter moisturizers and other topical products.
It’s most important, however, to change your diet if you want to smooth over the process of going off the pill and address your acne head on, according to both Richichi and Dr. Jaliman. “When you don’t address the cause, you go off the pill and your acne comes back with a vengeance,” Richichi says, “If you’re so concerned about getting off your medication in the first month, start changing your diet and wait four weeks before going off the pill.”
Acne is an external manifestation of an internal imbalance that leads to overproduction of sebum (skin oil) by sebaceous glands. This extra sebum together with dead skin cells creates an ideal condition for bacterial growth, which can promote inflammation. The first stage of acne usually starts as oily skin; the next stage is the appearance of comedones (whiteheads and blackheads). If the pore becomes inflamed due to the influx of white blood cells, pustules and papules appear. If the inflammatory response spreads to adjacent tissue, this represents cystic acne.

“Stress leads to increased cortisol levels,” says dermatologist Ava Shamban. Like testosterone, cortisol is a hormone that can increase oiliness as well as inflammation. “Things are so much better today because there are so many more options for treating acne,” said Sarah Taylor, MD, a dermatologist with Wake Forest Baptist Health. “While OTC products are pretty much the same as they have been for years – just different concentrations of benzoyl peroxide and salicylic acid in various forms such as cleansers, gels and creams – the prescription world has really changed in the past 10 years or so. Few high-quality studies have been performed which demonstrate that stress causes or worsens acne. While the connection between acne and stress has been debated, some research indicates that increased severity is associated with high stress levels in certain contexts such as hormonal changes seen in premenstrual syndrome.
Hemp seed oil protects the skin from free radicals while improving elasticity. Isotretinoin is an important medicine. It can clear severe acne that not respond to any other treatment. As acne clears, a patient’s quality of life often greatly improves. Dermatologists are committed to the safe and responsible use of this medicine. Patient safety is a dermatologist’s first priority when prescribing isotretinoin.
The most commonly prescribed topical retinoids for acne vulgaris include adapalene, tazarotene, and tretinoin. These retinoids should be applied once daily to clean, dry skin, but they may need to be applied less frequently if irritation occurs. Skin irritation with peeling and redness may be associated with the early use of topical retinoids and typically resolves within the first few weeks of use.  The use of mild, nonirritating cleansers and noncomedogenic moisturizers may help reduce this irritation.  Alternate-day dosing may be used if irritation persists.
Blackheads and whiteheads (comedones). Comedones are enlarged hair follicles filled with sebum. Blackheads are comedones that have pushed through the skin’s surface. Exposure to air causes the sebum to turn black. Whiteheads are comedones that have not pushed through the skin’s surface.

Benzoyl peroxide clears the pores and reduces bacterial count by creating an environment where bacteria cannot survive. It dries skin and works as a peeling agent. It commonly causes dryness and irritation, and in some can cause burning, itching, peeling and swelling. Topical use of green tea reduces sebum production and protects against acne-causing bacteria. Its high antioxidant concentration eliminates the free radicals that lead to inflammation.
An ice pick scar is formed when an inflamed blemish reaches the surface of the skin. When the blemish heals, the follicle empties, forming a column-like scar that runs from the surface to the inner layer of the skin.
More than 80% of people will develop some degree of acne between age 11 – 30 years.1 Acne is usually mild and transitory, however, it can lead to complications including scarring, dyspigmentation and psychological issues such as anxiety, depression and, rarely, suicide.2, 3
Tetracyclines, a first-line therapy, include doxycycline and minocycline and work by binding to the 30S subunit of the bacterial ribosome; they also have anti-inflammatory effects. Pharmacists can make sure they’re being dosed correctly. Doxycycline is most effective at 1.7 to 2.4 mg/kg and can be dosed once- or twice-daily. It’s important to counsel patients that gastrointestinal (GI) upset, photosensitivity, and dizziness are possible AEs.
Wash your fabrics. Any fabric that comes into contact with your skin on a regular basis – clothes, towels, pillowcases, and sheets – should be washed at least once a week to remove oil and bacteria that build up over time. Use a gentle cleanser for sensitive skin to help solve your acne problem.
Some acne treatments can be bought over the counter at pharmacies or supermarkets. These work by cleaning the skin and drying up excess oil. Mild irritation can occur with such treatments. If this happens, take a short break from treatment and restart after a few days. If the irritation is excessive, stop the medication and speak to your doctor about an alternative.

There is also a growing body of evidence for laser treatment of acne, standalone or in the context of PDT. However, similar to light therapy, the power and quality of research is limited by small patient numbers, lack of a control population, and comparison to standard therapies. Nonetheless, preliminary studies have shown improvement in acne, and even potential long-lasting effects with the 1550-nm erbium-glass laser, 1064-nm Nd:YAG laser, and PDT with laser.
