Limit washing your face (and body if you have body acne) to two times per day or after heavy sweating. Over-washing and over-drying your skin will actually stimulate sebaceous glands (it is a skin self-protection mechanism) to produce more oil.

Benzoyl peroxide is available as a gel, cream or cleanser, and ranges in strength from 2.5% – 10%. The 2.5% strength is sufficient for most people, and higher concentrations are associated with greater adverse effects. The choice between gel, cream and wash should be based on patient preference and skin-type, with gels being more suited to oily skin. A rinse-off cleanser is more suitable if irritation occurs and is more convenient for treating acne affecting the trunk.

A disease whose exact cause is unknown can be difficult or impossible to prevent. In the case of acne, what contributes to an outbreak is generally known, meaning there are things you can do to significantly reduce the risk of having a problem. The diagnosis of acne vulgaris is primarily clinical.4 History and physical examination can help determine if there is an underlying cause of the acne, such as an exacerbating medication or endocrinologic abnormality causing hyperandrogenism (e.g., polycystic ovarian syndrome). Other dermatologic manifestations of androgen excess include seborrhea, hirsutism and androgenetic alopecia. Endocrinologic testing is not ordered routinely for women with regular menstrual cycles.2,3 Older women, especially those with new-onset acne and other signs of androgen excess (e.g., hirsutism, androgenic alopecia, menstrual irregularities, infertility), should be tested for androgen excess with measurements of total and free serum testosterone, dihydroepiandrosterone, and luteinizing and follicle-stimulating hormone levels.5 Pelvic ultrasonography may show the presence of polycystic ovaries.5 In prepubertal children with acne, signs of hyperandrogenism include early-onset accelerated growth, pubic or axillary hair, body odour, genital maturation and advanced bone age.

Patients should be advised to wash their face gently with warm water and mild soap or cleanser, twice daily. An un-medicated face-wash is sufficient, although products containing benzoyl peroxide or salicylic acid can be effective. Rough scrubbing should be avoided as it causes follicular rupture, increasing the inflammatory response. Patients with sensitive skin, e.g. atopic dermatitis, should avoid soap, and anti-acne cleansers may cause irritation and contact dermatitis.

The sebaceous glands or oil glands are where the acne form. These glands are more developed on the face, neck, scalp, and chest. Also, the face is more exposed to pollution and bacteria compared to the rest of the body. Both these factors work together to make acne mainly affect the face.

Topical erythromycin and clindamycin are generally well-tolerated and have been shown to reduce inflammatory lesions by 46% to 70% in several randomized controlled trials.2 Monotherapy with topical antibiotics should not be used routinely beause P. acnes may become resistant within one month after daily treatment has begun.6 Some argue that this resistance is not relevant because the antibiotics (e.g., clindamycin, tetracyclines, erythromycin) also have intrinsic anti-inflammatory and antimicrobial effects.11 However, antibiotic-resistant Staphylococcus epidermidis and Staphylococcus aureus may also develop with monotherapy; resistance can be avoided when a topical antibiotic is combined with benzoyl peroxide.12

Although there is no one single cure that works in all patients with acne, dermatologists can recommend patient-specific treatment regimens to control acne and minimize future breakouts. In women, hormonal therapies are commonly used to treat acne safely and effectively.

Exfoliating cleansers and masks: A variety of mild scrubs, exfoliants, and masks can be used. These products may contain salicylic acid in a concentration that makes it a very mild peeling agent. These products remove the outer layer of the skin and thus open pores. Products containing glycolic or alpha hydroxy acids are also gentle skin exfoliants.

One last word of advice: apply sunscreen daily. Dr. MacGregor couldn’t stress enough the important role sunscreen plays in reducing marks and scars. “I always suggest using a broad spectrum sunscreen because the spots only get worse if the person is constantly getting sunlight on the skin,” she said.

2. Use an exfoliating acid to de-gunk your pores. When you think about an exfoliant, you probably think of a cleanser with beads or granules. These are called physical exfoliants, and while they’re great for sloughing away dead skin, they can be irritating to acne that’s already present on the skin. And, even worse, they inadvertently spread bacteria across your face, causing more breakouts. Acid exfoliators (also referred to as chemical exfoliants) sound strong and scary, but in reality, they’re phenomenal for your complexion. They work like little Pac-Man on your skin, helping to devour dirt, debris, and dead skin cells that clog your pores. The all-star chemical exfoliators for acne are glycolic acid and fruit-based enzymes, such as papaya and pumpkin. Just be careful not to over-exfoliate, which can dry out the skin and lead to the overproduction of oil and, therefore, more acne; two or three times a week should suffice.

Use a clay mask every night. Bentonite clay is an excellent anti-inflammatory that also draws out impurities from your skin. Using a mask every night for 20minutes will help calm any redness and swelling, and help to prevent new pimples from developing (it also mops up oil and helps reduce blackheads). Isotretinoin is associated with a range of serious adverse effects (Table 1). Some, particularly cheilitis, are so common that they can be considered indicators of adherence.17 Isotretinoin may cause an initial worsening of acne, but severe flares are uncommon. It is important to discuss these adverse effects with patients prior to initiation, to optimise adherence. Most adverse effects are dose-related and starting with a low dose may reduce the incidence and severity.

Benzoyl peroxide clears the pores and reduces bacterial count by creating an environment where bacteria cannot survive. It dries skin and works as a peeling agent. It commonly causes dryness and irritation, and in some can cause burning, itching, peeling and swelling. Topical use of green tea reduces sebum production and protects against acne-causing bacteria. Certain ingredients found in products such as cosmetics, sunscreen, and moisturizers are more likely to clog pores. Renée mentions that many of her clients with acne-prone skin are paranoid about using moisturizer, thinking that it will immediately clog up their pores and not let the skin “breathe.” “For starters, wearing moisturizer does not cause breakouts,” Renée clarifies. “Acne happens when the cells that line the inner pores fail to fall off properly, clogging the pore. This happens whether you moisturize or not. Tea tree oil is renowned for being good for your skin. In addition to this, it also contains anti-bacterial and anti-fungal properties, which makes it highly beneficial for acne treatment. Our family has this issue. My Mom had them, I have them, some of my children and grandchildren have them. In our family there is one major culprit…chocolate. Isotretinoin. Isotretinoin is known under the brand name Accutane and a number of generic names. It is an oral product used mostly for treating severe acne. However, it’s especially dangerous before and during pregnancy as the drug is known to affect a fetus and cause serious birth defects.

“Other good over-the-counter options are benzoyl peroxide-containing agents,” says Dr. Engelman. “I like La Roche-Posay Effaclar Duo. Benzoyl peroxide is anti-microbial, attacking the bacteria that is associated with acne. The La Roche Posay product also contains Lipohydroxy acid (LHA), which serves as an exfoliator to smooth roughness and even out skin texture.”

Because we do not have enough evidence to tell whether this medicine can cause depression and other mental health conditions, dermatologists carefully monitor their patients for warning signs of depression and other mental health conditions.

Mild acne is a very common thing and goes away in time, normally without any scarring. If you start to develop red spots, pustules or rashes, it is advisable to start treatment as early as you can, to prevent the acne worsening. Treatment will eventually clear your acne it if is used correctly, however you may always have a breakout due to outside influences.

Complementary therapies have been investigated for treating people with acne.[108] Low-quality evidence suggests topical application of tea tree oil or bee venom may reduce the total number of skin lesions in those with acne.[108] Tea tree oil is thought to be approximately as effective as benzoyl peroxide or salicylic acid, but has been associated with allergic contact dermatitis.[1] Proposed mechanisms for tea tree oil’s anti-acne effects include antibacterial action against P. acnes, and anti-inflammatory properties.[63] Numerous other plant-derived therapies have been observed to have positive effects against acne (e.g., basil oil and oligosaccharides from seaweed); however, few studies have been performed, and most have been of lower methodological quality.[109] There is a lack of high-quality evidence for the use of acupuncture, herbal medicine, or cupping therapy for acne.[108]

Dermatologists prescribe isotretinoin for severe acne. Severe acne is often very painful. When the painful cysts and nodules of severe acne clear, they leave permanent scars. Many people who have severe acne feel depressed. Some feel anxious. Low self-esteem often develops. Grades can plummet, and job performance can suffer.

More than 80% of adolescents suffer from acne. Even adults and younger children are affected by these annoying breakouts. A simple pimple may take three to seven days to go away by itself. Sometimes, they last longer and indicate a more complex underlying cause that needs to be treated.

The number of times you wash your face is important. Over-washing is no solution. Excessive scrubbing won’t make it go away.1 Try to limit washing your face to twice a day.1 Any more than this can leave your skin dry and irritated.

Try over-the-counter acne products to dry excess oil and promote peeling. Look for products containing benzoyl peroxide as the active ingredient. You might also try products containing salicylic acid, glycolic acid or alpha hydroxy acids, which may help with mild and moderate acne. It may take a few weeks before you see any improvement.

Differin (adapalene) is also used as a cream, gel, or lotion applied to the skin that has anti-inflammatory properties. As with Tazorac, the gel form is best for acne. Sometimes Differin is combined with benzoyl peroxide to prevent infection of surrounding skin as cysts and nodules open. The product works by keeping pores from being covered with new skin, and is best for people who need to keep cyst-prone skin clear.

The best approach to treatment requires some patience as it involves starting with milder over-the-counter medications or home remedies and then progressing to the stronger prescription drugs if satisfactory results are not realized. In general, the risk of side effects increases with the strength of what is being used as treatment. If you need to progress to the point where requiring an isotretinoin prescription is a possibility, there will be some decisions that have to be made, and these decisions should involve discussing with a dermatologist the pros, cons, and risks involved.

Contrary to the marketing promises of “blemish banishers” and “zit zappers,” immediate results are not the trademark of acne treatments — a frustrating truth to anyone suffering through a breakout. And while pimples are personal (your stress-induced spots will look and act differently than your best friend’s breakout), the best acne treatments will include a regimen of products to hit all of acne’s root causes.

If not treated, acne can cause scarring. Scarring is more likely when the skin is inflamed, swollen, reddish and painful, such as cystic acne and nodules. This form of acne penetrates deep into the skin, causing damage. Delaying treatment can cause scarring too.
