Blackheads are a mild form of acne that appear as unsightly, open pores that appear darker than the skin surrounding them. They get their dark appearance from a skin pigment called melanin, which oxidizes and turns black when it’s exposed to the air. What it is: You may have heard of tretinoin in reference to “Retin-A” wrinkle treatments. Tretinoin is retinoic acid and vitamin A acid combined, and comes in various strengths in creams, gels, and liquids for topical use on the skin. Shah often recommends over-the-counter retinols or prescription retinoids to her acne-prone patients. “I find that compared to other treatments they are beneficial for not just treating acne but also preventing new acne from forming as they help prevent that initial stage of the follicle getting clogged,” she says. “They can also help with some of the post acne [problems] such as hyperpigmentation.” But keep in mind if you have sensitive skin (or eczema or rosacea), a prescription retinoid might be too strong an option. However, your dermatologist can recommend an over-the-counter retinol with a low concentration (0.1 to 0.25 percent), which might be better tolerated. Retinol also isn’t a quick fix. It takes time to see results, and it’s something you’ll have to keep using to maintain its benefits. Shah also mentions that retinol plays well with other acne treatments on the list. “Retinol can be combined with other over-the-counter or prescription medications such as benzoyl peroxide, topical antibiotics, and oral medications. The right combination depends on the severity of the acne and your skin type.”

Doxycycline is another of the tetracyclines that is equally effective in treating acne. It comes in generic versions and also as the branded Doryx and Acticlate which are easier on the stomach. Originally FDA approved for the treatment of rosacea, Oracea is a non antibiotic dose of doxycycline that is often used as an acne treatment, as well. Taken orally, it can be used as solo therapy or in combination with a topical acne treatment regimen. More severe cases of acne might need higher doses of doxycycline, but since Oracea is not an antibiotic, many patients can be “down-graded” to Oracea improvement and it is suitable for longterm use as it does not cause antibiotic resistance.

Inflammatory products trigger the melanocytes, the cells that make the pigment (melanin) that gives color to your skin, to increase the synthesis of melanin. The increase in melanin leads to some pigment transfer to the surrounding skin cells (also called keratinocytes which are the cells found in the epidermal layer of the skin and are responsible for making keratin.)

What triggers this process isn’t clear. Hormonal changes are associated with the excess production of oil – thus partially accounting for acne flare-ups in teens and pregnant women – and heredity can be a factor, but research has shown that acne is not caused by dirty skin or by eating chocolate, pizza or greasy foods.

Lesions occur primarily on the face, neck, upper back and chest.4 When assessing the severity of the acne, one needs to consider the distribution (back, chest, upper arms), type and number of lesions (comedones, papules, pustules, nodules) and the presence or absence of scarring (Table 1).2,3

Tretinoin is available as a 0.05% cream, and should be applied thinly, once daily. Application should be “built up” to avoid adverse effects: on the first night, apply for five minutes before washing off; on the second night, apply for ten minutes; on subsequent nights, increase the application time by 30 minutes until a two-hour application is achieved, at which point the cream can be left on overnight.7, 8

Simple alcohols destroy the skin’s barrier, called the acid mantle, which is what keeps your skin’s pH in its happy place (and your skin blemish-free). “A lot of products that are marketed to treat acne do not maintain — or even consider the importance of — the skin’s acid mantle,” explains Natarsha Bimson, a Los Angeles-based aesthetician. “It’s a huge issue.” In one study, even a small, 3 percent alcohol solution applied to skin cells over the course of two days increased cell death by 26 percent — in some acne treatments, like Kate Somerville’s EradiKate, alcohol is one of the first ingredients listed, meaning it’s a largely alcohol-based formula.

Treating this skin condition is important not only for the sake of appearance but also to relieve the pain caused by these nodules. At the very least, they tend to be very sensitive and can easily become irritated when touched. Another reason for treating these nodules early on is that they tend to be long lasting, often lasting for months at a time. If a pore gets clogged up, closes, and bulges out from the skin, that’s a whitehead. If a pore clogs up but stays open, the top surface can get dark and you’re left with a blackhead. For individuals suffering from the cystic form of acne, a controlled clinical trial has found that Guggul supplements (also known as guggulsterone) outperformed 500 milligrams of tetracycline by a small margin. A review of three clinical studies with 1259 patients showed that a combination of clindamycin 1% and benzoyl peroxide 5% was more effective than either drug used alone in reducing lesions and suppressing P. acnes.16

Antibiotic resistance in C acnes (formerly P acnes) is common and is a significant threat to acne treatment. They should be used with benzoyl peroxide to reduce the likelihood of resistance. [2] Concurrent use of oral and topical antibiotics should be avoided and should not be used as monotherapy. If acne relapses, use the same antibiotic if it was previously effective. Oral antibiotics: Doctors may start treatment with tetracycline (Sumycin) or one of the related “cyclines,” such as doxycycline (Vibramycin, Oracea, Adoxa, Atridox, and others) and minocycline (Dynacin, Minocin). Other oral antibiotics that are useful for treating acne are cefadroxil (Duricef), amoxicillin (Amoxil, DisperMox, Trimox), and the sulfa drugs.

The best treatments inhibit sebum production, limit bacterial growth, or encourage shedding of skin cells to unclog pores. Because many therapies can have side effects, any patient with acne should proceed with caution when trying a new treatment. People with any type of acne that lowers their self-esteem or makes them unhappy, those with acne that is leaving scars or people with severe, persistent cases of acne, need the care of a dermatologist.

When starting treatment with topical retinoids, Dr. Schlosser advises that the therapy should only be applied three times a week for the skin to get accustomed to it. Over time, the frequency of the medication should be gradually increased with the goal of using a topical retinoid every night.

Risk factors for the development of acne, other than genetics, have not been conclusively identified. Possible secondary contributors include hormones, infections, diet and stress. Studies investigating the impact of smoking on the incidence and severity of acne have been inconclusive.[2][37][38] Sunlight and cleanliness are not associated with acne.[14]

Topical erythromycin and clindamycin are generally well-tolerated and have been shown to reduce inflammatory lesions by 46% to 70% in several randomized controlled trials.2 Monotherapy with topical antibiotics should not be used routinely beause P. acnes may become resistant within one month after daily treatment has begun.6 Some argue that this resistance is not relevant because the antibiotics (e.g., clindamycin, tetracyclines, erythromycin) also have intrinsic anti-inflammatory and antimicrobial effects.11 However, antibiotic-resistant Staphylococcus epidermidis and Staphylococcus aureus may also develop with monotherapy; resistance can be avoided when a topical antibiotic is combined with benzoyl peroxide.12

If you’ve found that your skin is too raw and sensitive to handle any treatment at all, don’t despair: there’s still hope. Thi Nguyen, program development specialist in dermatology at pharmaceutical research organization Biorasi, told us: “Patients with sensitive skin are not obligated to use topical products. Retinoids are medications which reduce inflammation, normalize the follicle cell life cycle, and reduce sebum production.[44][81] They are structurally related to vitamin A.[81] The retinoids appear to influence the cell life cycle in the follicle lining. This helps prevent the accumulation of skin cells within the hair follicle that can create a blockage. They are a first-line acne treatment,[1] especially for people with dark-colored skin, and are known to lead to faster improvement of postinflammatory hyperpigmentation.[35]

Certain hormones called androgens are an additional factor in causing acne. Androgens are male hormones that are present in both men and women but are present at higher levels in men. Androgens do two things: First, they enlarge the sebaceous glands in the skin. Second, they cause these glands to increase sebum (oil) production. The increased sebum production exacerbates plug formation and serves as more “food” for the bacteria. Androgens surge at puberty, which is why teens develop armpit and pubic hair and why boys develop facial hair and deeper voices. Even though isotretinoin does not remain in the body after therapy is stopped, improvement is often long-lasting. It is safe to take two or three courses of the drug if unresponsive acne makes a comeback. It is, however, best to wait at least several months and to try methods before using isotretinoin again.

Topical antibiotics deemed safe during pregnancy include clindamycin, erythromycin, and metronidazole (all category B), due to negligible systemic absorption.[46][97] Nadifloxacin and dapsone (category C) are other topical antibiotics that may be used to treat acne in pregnant women, but have received less study.[46][97] No adverse fetal events have been reported from the topical use of dapsone.[97] If retinoids are used there is a high risk of abnormalities occurring in the developing fetus; women of childbearing age are therefore required to use effective birth control if retinoids are used to treat acne.[19] Oral antibiotics deemed safe for pregnancy (all category B) include azithromycin, cephalosporins, and penicillins.[97] Tetracyclines (category D) are contraindicated during pregnancy as they are known to deposit in developing fetal teeth, resulting in yellow discoloration and thinned tooth enamel.[1][97] Their use during pregnancy has been associated with development of acute fatty liver of pregnancy and is further avoided for this reason.[97]

Comedonal acne. This is the kind of mild acne that involves blackheads and whiteheads. It forms because a component of skin oil called sebum, along with old skin cells, block the pores of the skin. Comedonal acne appears most often on the forehead, nose, and chin.

Acne often begins during puberty. It occurs when sebaceous glands in the skin are over-stimulated to produce sebum and skin cells are not shed normally. These sticky cells block the skin’s hair follicles, trapping the sebum.

Postinflammatory hyperpigmentation (PIH) is usually the result of nodular acne lesions. These lesions often leave behind an inflamed darkened mark after the original acne lesion has resolved. This inflammation stimulates specialized pigment-producing skin cells (known as melanocytes) to produce more melanin pigment which leads to the skin’s darkened appearance.[34] People with darker skin color are more frequently affected by this condition.[35] Pigmented scar is a common term used for PIH, but is misleading as it suggests the color change is permanent. If you are sensitive to salicylic acid or find commercial products ineffective for your back acne (bacne), try a medicated skin powder. This will usually work well and won’t over-dry your skin. Speak to your pharmacist about brand options.

If you really must do something about your pimple beyond washing your face and spot treating, ice or a cold compress can help reduce swelling. Wrap an ice cube or the compress in a soft tissue or cloth and apply it to your zit for 20-30 seconds at a time, a few times a day. Isotretinoin is a standard prescription that you can get from your doctor. You may have heard of the brand names Absorica®, Accutane®, Amnesteem®, Claravis®, Myorisan®, Sotret® and Zenatane™. Acne (acne vulgaris, common acne) is a disease of the hair follicles of the face, chest, and back that affects almost all teenagers during puberty — the only exception being members of a few primitive Neolithic tribes living in isolation. It is not caused by bacteria, although bacteria play a role in its development. It is not unusual for some women to develop acne in their mid- to late-20s.

Sugar: You may have already suspected that sugar is related to breakouts. Some studies now suggest that there may be a link. This doesn’t mean that if you eat a cookie you’re going to get a pimple. It comes down to how much sugar you’re eating in a day—particularly at any one time. Blackheads (Open comedones): Blackheads result when a pore is only partially blocked, allowing some of the trapped sebum (oil), bacteria, and dead skin cells to slowly drain to the surface. The black color is not caused by dirt. When the contents of the pore come in contact with air, they turn dark.

Patients who are treated with isotretinoin will need to be enrolled in the iPLEDGE (http://www.ipledgeprogram.com) program as will the physician prescriber and the pharmacy provider. This requires monthly visits and laboratory work. Females of childbearing potential must be on some form of birth control and have negative pregnancy tests during the course of therapy because isotretinoin can damage exposed fetuses. There are a variety of other potential side effects produced by the drug. Isotretinoin, however, when used in appropriate patients, is a unique drug in that it offers a safe and effective “cure” for a noninfectious disease.

If this option is not successful, then the patient is started on isotretinoin (Claravis, Amnesteem, Absorica, Myorisan, Zenatane). Patients may benefit from a lower dosage initially, followed by an increase to the standard 1 mg/kg dose after about a month. Patients should remain on this dosage for at least six months to maintain a durable improvement. Rarely, a short course of oral steroids (prednisone) may be necessary to suppress the inflammation during the initiation of therapy.

Buying a generic acne wash won’t necessarily improve your complexion; for a cleanser to be most effective, you have to pay attention to your skin’s needs and pick the ingredients accordingly. If your skin tends to be oily, choose products with salicylic acid, benzoyl peroxide, or glycolic acid. Isotretinoin has a high risk of inducing birth defects if taken by pregnant women. Women of childbearing age who take isotretinoin need two negative pregnancy tests (blood or urine) before starting the drug, monthly tests while they take it, and another after they are done. Those who are sexually active must use two forms of contraception, one of which is usually the oral contraceptive pill. Isotretinoin leaves the body completely when treatment is done; women must be sure to avoid pregnancy for one month after therapy is stopped. There is, however, no risk to childbearing after that time.

Folliculitis: Pimples can occur on other parts of the body, such as the abdomen, buttocks, or legs. These represent not acne but inflamed follicles. If these don’t go away on their own, doctors can prescribe oral or external antibiotics, generally not the same ones used for acne.

In severe cases, oral isotretinoin may be considered. This medication can be very effective but can also cause serious side effects including severe birth defects. Strict protocols must be followed. Monthly appointments with the treating doctor must be kept throughout the treatment period to monitor for any side effects. In females of child-bearing age, protocol includes two forms of birth control. The treatment period is usually five months.

If you have large, deep, solid and painful lumps or cysts under your skin, you may have severe acne. The breakouts cover large areas of the face or body and last longer than in moderate acne, often not going away for months or years. 

Avoid using strong soaps or products that strip all oil and moisture from your skin. Your skin’s natural reaction is to try to compensate by producing more oil, which will worsen your acne in the long run!

Sometimes birth control alone isn’t enough to really make a difference in hormonal acne. That’s when your doctor might recommend adding in an androgen blocker such as Spironalactone. Spiro (as it’s called) minimizes the amount of androgen hormones in circulation by blocking the receptors that bind with testosterone. When these pills are taken at the same time as an oral contraceptive, 90 percent of women see an improvement in breakouts, according to Linkner. The drug is sometimes prescribed to women with PCOS (polycystic ovarian syndrome) to relieve androgen-related symptoms like excessive hair growth, hypertension, oily skin, and acne.

Use a soft washcloth (change it every time you wash) or cotton pads, but be gentle, and do not rub or injure your skin in any way. Micro-abrasions open the door, so to speak, for bacteria and may cause more breakouts.

If you are spot prone anyway, I would like to recommend Sudocrem. Forget the “put a tiny bit just on the spot”, you can spread it all over your face if you want, it won’t harm you, but it does reduce and clear spots very well if you leave it overnight. Unfortunately after a while, it doesn’t seem to work as well as the first few times you use it, but it still does help. It’s fairly cheap and can be bought pretty much anywhere that sells cosmetics etc.

Some large cysts do not respond to medication and may need to be drained or removed. Your dermatologist is the only person who should drain or remove these cysts. You should never try to drain or remove your acne by squeezing or picking. This can lead to infection, worsen your acne, and cause scarring.

Some patients do not clear after one course of treatment, and a second course of treatment may be an option. Studies show this helps some patients see clear skin. You should wait at least 8 weeks between treatments. The skin often continues to clear for a while after patients stop taking the medicine.

A few weeks ago, I made a drastic change to my diet to see if it would affect my acne. I replaced all my beverages with water or natural green-tea (no sugar!), I cut out ALL junk food and sugary foods, I went gluten-free and dairy-free, I included more fruits and veggies, and I ate low-glycemic foods. I was also taking zinc supplements, so you should research the effects of zinc for acne if you are looking to try a new remedy. With all of these combined, my [severe, cystic] acne cleared up in 2-3 weeks. Unfortunately, I had to stop because I had exams coming up and could not maintain the strict lifestyle. I truly believe acne can be cured by diet; the over-the-counter products can definitely help to reduce acne, but I do not think it will get rid of it forever. To everyone who is still struggling, including myself for the time being, please give this a try.

Sleep – Less sleep can cause your skin to produce more sebum, and this can cause acne. Sleep deprivation can also increase the stress levels and worsen your acne. Sleeping for at least six to eight hours every day is recommended

The effectiveness of an acne facial mask depends on what ingredients the mask contains. Sometimes they can make matters worse by preventing acne treatments known to work from doing their job. Lots of people enjoy the masks because they feel good on the skin. So if you want to use a face mask, look for products specially formulated with acne treatments such as benzoyl peroxide, salicylic acid, or sulfur, and only use them once in a while to avoid worsening acne.
