Acne appears when a pore in our skin clogs. This clog begins with dead skin cells. Normally, dead skin cells rise to surface of the pore, and the body sheds the cells. When the body starts to make lots of sebum (see-bum), oil that keeps our skin from drying out, the dead skin cells can stick together inside the pore. Instead of rising to the surface, the cells become trapped inside the pore.

It’s been almost five years since I stopped taking isotretinoin, and the mountain range of cysts spread across my face is long gone. I don’t worry what people see when they look at me, and looking in the mirror no longer means fixating on flaws. Some people will tell you that sitting out in the sun helps acne. But this isn’t true. A suntan can make acne look less severe by hiding pimples, but it won’t help them go away. The severity of acne vulgaris (Gr. ἀκµή, “point” + L. vulgaris, “common”)[23] can be classified as mild, moderate, or severe as this helps to determine an appropriate treatment regimen.[19] Mild acne is classically defined by the presence of clogged skin follicles (known as comedones) limited to the face with occasional inflammatory lesions.[19] Moderate severity acne is said to occur when a higher number of inflammatory papules and pustules occur on the face compared to mild cases of acne and are found on the trunk of the body.[19] Severe acne is said to occur when nodules (the painful ‘bumps’ lying under the skin) are the characteristic facial lesions and involvement of the trunk is extensive.[19][24]

Never use grease creams and cosmetics. Buy only ‘non-comedogenic’ or water-based formulas. These formulas had been thoroughly tested and proven that they do not clog the pores, which is the main cause of acne.

Response to oral antibiotics is usually seen after at least six weeks of therapy.4 If control is maintained for several months, the antibiotic may be discontinued gradually and only the topical therapy continued. Systemic antibiotics should not be used to treat mild acne because of the risk of increasing resistance.23,24 The additional use of nonantibiotic topical agents in combination with oral antibiotics should be considered.4 Topical retinoids with oral antibiotics may give a faster response and be more effective than either drug used alone.3

Get a facial. These are available at most spas, and involve the use of several different cleansers, masks, and extraction tools to reduce acne on your face. If you don’t feel comfortable with having an esthetician work on your face, visit your dermatologist for a more medically-oriented facial.

Avoid exposure to sunlight or artificial UV rays (sunlamps or tanning beds). Accutane can make your skin more sensitive to sunlight and sunburn may result. Accutane may impair your vision, especially at night. Be careful if you drive or do anything that requires you to see clearly.

Dark spots. Spots that appear where blemishes have healed usually disappear over time. But it can help to apply skin-lightening products. Your dermatologist may also suggest a concealer makeup to cover the spots and make them less apparent.

To put it another way, if the nodule heals and goes away, you may still have a problem since these cysts can at times proliferate, or whatever has caused them to develop can still cause more to develop.

Topical or systemic antibiotics should always be used in combination with benzoyl peroxide, a topical retinoid or azelaic acid. In women, they may also be used in combination with antiandrogen therapy or oral contraceptive pill.

Studies so far have focused mostly on the foods that make acne worse. Here are the five that come up most often as culprits in increasing breakouts. Avoid these for about a week, and see if you notice a difference.

Spironolactone may also be used in the treatment of acne vulgaris. [45] Spironolactone binds the androgen receptor and reduces androgen production. Adverse effects include dizziness, breast tenderness, and dysmenorrhea. [31] Dysmenorrhea may be lessened by coadministration with an oral contraceptive. In two 2017 retrospective studies, spironolactone has been shown to be effective in reducing inflammatory lesions in multiple areas of the body with minimal adverse effects. [46, 47] Currently, more high-powered randomized controlled trials are needed to assess the efficacy of spironolactone monotherapy in treating acne, but spironolactone should be considered in recalcitrant acne, in women who do not tolerate or have contraindications to oral contraceptives, and to prevent antibiotic resistance. [31] A 2015 large retrospective study of healthy women aged 18-45 years confirms potassium monitoring is unnecessary for these patients while taking spironolactone. [48] Pregnancy must be avoided while taking spironolactone because of the risk of feminization of the male fetus, and spironolactone is not recommended for males because of the potential for gynecomastia. [31, 46] While a black box warning regarding possible cancer risk was placed on spironolactone many years ago after rats fed high doses of the medication developed both benign and malignant tumors, several large retrospective and longitudinal studies have found no association with cancer. [49]

Ortho Tri-Cyclen works to balance the hormones, which prevents the sebaceous glands from producing too much oil. Since oil clogs the pores and increases the risk of acne, this helps prevent acne breakouts. Women who smoke, and those over 35 years of age, should not take this drug due to the risk of blood clots.

Antibiotics. Antibiotics may be used on top of the skin (topical) or taken orally (systemic). Antibiotics work by clearing the skin of acne-causing bacteria and reducing inflammation. There are several topical products available in creams, gels, solutions, pads, and lotions. Topical antibiotics are limited in their ability to penetrate the skin and clear more deep-seated acne, whereas systemic antibiotics circulate throughout the body and into sebaceous glands. However, systemic antibiotics often cause more side effects than topicals, but they can be used for more severe kinds of acne. Usually, topical antibiotics aren’t recommended alone as an acne treatment, as they can increase the risk for antibiotic resistance in skin bacteria. However, using benzoyl peroxide with a topical antibiotic may reduce the chances of developing antibiotic resistance.

Antibiotic resistance in C acnes (formerly P acnes) is common and is a significant threat to acne treatment. [29] Antimicrobials should be combined with a topical retinoids for greater clearing of lesions and to increase the potential for shortened antibiotic treatment. They should be used with benzoyl peroxide to reduce the likelihood of resistance. [2] Concurrent use of oral and topical antibiotics should be avoided and should not be used as monotherapy. If acne relapses, use the same antibiotic if it was previously effective. It may also be helpful to use benzoyl peroxide for 5-7 days between antibiotic courses to reduce resistance in organisms on the skin. [2]

Of all forms of laser and light therapy, the most evidence is with photodynamic therapy (PDT) in treating acne.
