Dermabrasion used to be one of the most popular acne scar treatment procedures, but the advent of new technology and strategies has made it somewhat less popular. In dermabrasion procedures, the skin is numbed, frozen, and exposed to a sharp, rotating blade that gently sands off the top layer of the skin’s surface. 

The {blocked|stopped up|back logged} pore accumulates keratin, {essential oil|olive oil|petrol} (sebum) and bacteria until it ruptures, releasing this mixture into the {pores and skin|epidermis|skin area} and {creating|triggering} inflammation. This kind of inflammation can lead to painful cyst/abscess formation and scarring.

In severe cases of acne, a visit to dermatologist or skin specialist is required to get rid of acne. Prescribed medication is needed for severe acne. Medication can be in the form of oral antibiotics, cream or lotion.

Many lotions and creams are sold at drugstores to help prevent acne and clear it up. You can try different ones to see which helps. Products with benzoyl peroxide (say: BEN-zoil peh-ROK-side) or salicylic (say: sal-uh-SIL-ick) acid in them are usually pretty helpful for treating acne. Benzoyl peroxide kills the bacteria that can lead to acne and it also can reduce swelling (puffiness) of pimples. Salicylic acid is another acne-fighting ingredient. It causes skin to dry out and peel, which can help get rid of pimples, too.

Antibiotics. These work by killing excess skin bacteria and reducing redness. For the first few months of treatment, you may use both a retinoid and an antibiotic, with the antibiotic applied in the morning and the retinoid in the evening. The antibiotics are often combined with benzoyl peroxide to reduce the likelihood of developing antibiotic resistance. Examples include clindamycin with benzoyl peroxide (Benzaclin, Duac, Acanya) and erythromycin with benzoyl peroxide (Benzamycin). Topical antibiotics alone aren’t recommended.

You’ve probably seen those tiny glass jars filled with liquid and some solid sediment at the bottom—sometimes it’s pink, sometimes it’s white, and (in the case of my favorite product) sometimes it’s flesh-tone. The solid in the jar is usually a mix of drying ingredient like camphor, sulfur, and zinc oxide, but many brands throw in a few little extras. Cult-favorite Mario Badescu Drying Lotion adds calming calamine and exfoliating salacylic acid to their formula, while my favorite, End-Zit Acne Control Drying Lotion, keeps it simple, but comes in several shades so that it conceals your breakout while it treats it.

Apply essential oils to active pimples. Neem oil and tea tree oil are thought to have antimicrobial properties that can kill acne-causing bacteria. Dab a drop of diluted version of tea tree oil or neem oil on each spot, or wet a cotton swab and wipe it over problem areas. [7]

Practitioners should discuss with the patient, and then provide in writing, the potential adverse effects, particularly teratogenicity in females. A suitable patient information brochure is provided with the medicine by the distributor. Baseline laboratory investigations should be requested. Female patients will require advice on contraceptive options, and where necessary, be prescribed contraceptives (some will already be using oral contraceptives for managing acne symptoms). Patients (and parents/caregivers for those under age 16 years) should be asked to sign a consent form, to indicate that they understand the adverse effects that are possible while taking isotretinoin and, for female patients, the importance of not becoming pregnant during the course of their treatment and a further month after it has been discontinued.

To hydrate and heal your skin, try applying masks a couple of times per week. Yogurt, honey, cinnamon, essential oils and other ingredients can be used to create soothing masks that help to hydrate skin and fight common causes of acne.

Topical or systemic antibiotics should always be used in combination with benzoyl peroxide, a topical retinoid or azelaic acid. In women, they may also be used in combination with antiandrogen therapy or oral contraceptive pill.

One can do a lot to treat acne using products available at a drugstore or cosmetic counter that do not require a prescription. However, for tougher cases of acne, one should consult a physician for treatment options.

Isotretinoin is available by prescription only and its use needs to be carefully monitored since this medication has been known to produce severe and even potentially dangerous side effects on occasion. Dermatologists will normally refuse to prescribe isotretinoin for women who are pregnant, as it has been known to be a cause of birth defects. On the positive side, isotretinoin is quite effective in reducing sebum production, a major contributor to acne, in normalizing follicular desquamation or the thinning and peeling of the skin, in reducing bacterial activity, and in acting as an anti-inflammatory agent.

While some acne types are incurable, almost all are treatable, and in the majority of cases, especially those cases of mild to moderate form, the condition goes away on its own. When the condition does go away, it may or may not return. In general, the prognosis is good. Patient compliance is very important however, especially if scarring is to be avoided. If you are experiencing this somewhat severe skin condition, you need to follow your dermatologist’s advice, or if you’ve found a medication or home remedy that appears to be working, you need to establish a treatment regimen and stick to it.

Treatment with isotretinoin often results in prolonged clearance of acne, which can be permanent for some patients. A course of treatment generally lasts about 4 to 5 months. It can run shorter or longer. 

We don’t have personal experience with the product, but consumer reviews suggest it works well to reduce redness and swelling. Based on the ingredients list it takes a different approach than most treatments, so it may be worth trying if other over the counter products have failed.

Almost all patients with this severe form of acne will require some form of systemic (oral) medication to control this condition. The goal of therapy is to minimize scarring. The choice of drug with which to begin treatment depends on the experience and preferences of the physician and patient. It is often warranted to try oral antibiotics initially (doxycycline [Doryx, Oracea, Monodox], sulfamethoxazole/trimethoprim [Bactrim, Sulfatrim], minocycline [Minocin]) for a limited period of time supplemented by a topical retinoid drug (Retin A, Differin, Tazerotene). Antibiotics probably owe much of their effectiveness to their anti-inflammatory characteristics rather than their antibacterial attributes.

Acne treatment that you apply to the skin: Most acne treatments are applied to the skin. Your dermatologist may call this topical treatment. There are many topical acne treatments. Some topicals help kill the bacteria. Others work on reducing the oil. The topical medicine may contain a retinoid, prescription-strength benzoyl peroxide, antibiotic, or even salicylic acid. Your dermatologist will determine what you need.

About 1-3% of people are allergic to benzoyl peroxide and experience an inflammation of the skin or severe crusting.3This should not be confused with the “hardening effect” of benzoyl peroxide. Steroids are simply injected into the raised hypertrophic scar. The steroids will reduce the inflammation process and as a result the scar is reduced. Yet, to get better results, those steroid injections should be administered as early as possible, meaning it is often not as effective on hypertrophic scars older than 2 years.

If a pore gets clogged up, closes, and bulges out from the skin, that’s a whitehead. If a pore clogs up but stays open, the top surface can get dark and you’re left with a blackhead. Sometimes the walls of the pore are broken, allowing sebum, bacteria, and dead skin cells to get under the skin. This causes a small, red infection called a pimple. Clogged-up pores that open up deep in the skin can lead to bigger infections known as cysts.

Oil glands are small in children, but puberty (and testosterone in particular) causes these glands to grow larger. They secrete oil and cellular debris in a substance called sebum. Sebum keeps the skin flexible and acts as a protective barrier against water evaporation on the skin’s surface. It also keeps hair shiny and soft.

Prescribed Medications: Your healthcare professional may deem it necessary to prescribe oral antibiotics to help fight bacteria in the skin or a prescription only topical cream. In some cases a combination of both may be the solution.

Acne appears when a pore in our skin clogs. This clog begins with dead skin cells. Normally, dead skin cells rise to surface of the pore, and the body sheds the cells. When the body starts to make lots of sebum (see-bum), oil that keeps our skin from drying out, the dead skin cells can stick together inside the pore. Instead of rising to the surface, the cells become trapped inside the pore.

Tea tree oil, from the Melaleuca alternifolia tree, has anti-bacterial properties when applied topically to skin. One study showed that 5% tea tree oil was as effective as 5% benzoyl peroxide topically for reducing the number of inflamed acne lesions. Unfortunately, some people experience burning, stinging and redness when they use it. One warning – only use it topically, never by mouth.

Directions Clean the skin thoroughly before applying this product. Cover the entire affected area with a thin layer one to three times daily. Because excessive drying of the skin may occur, start with one application daily, then gradually increase to two or three times daily if needed or directed by a doctor. If bothersome dryness or peeling occurs, reduce application to once a day or every other day. If going outside, apply sunscreen after using this product. If irritation or sensitivity develops, stop use of both products and ask a doctor.

Try microdermabrasion. This is a process by which your skin is “sanded” away to produce new skin growth. Getting microdermabrasion treatments once a week over several months is the most effective route to take, as each treatment only affects the outermost layer of skin.

But the side effects of targeted spot treatments aren’t always worth it. “So many products instruct consumers to use benzoyl peroxide to spot treat red bumps and pustules. I don’t recommend it,” says Dr. Green. “Benzoyl peroxide, when placed on red spots, can actually cause more irritation and inflammation to the area. It’s best used to prevent red bumps and pustules, and applied all over the area you want to treat.” Robin Townsend, a medical aesthetician based in Cincinnati, was also quick to naysay a spot-treat-only approach: “Acne affects all of the pores. If someone is going to spot treat against my advice, I still suggest they spot treat one day and treat the whole face the next.”

Herbal therapies such as tea tree oil, and topical and oral ayurvedic compounds seem to be well tolerated; however, there are limited data about their efficacy and safety in treating acne.3 One clinical trial showed that topical tea tree oil was effective but had a slower onset of action than traditional topical agents.43 The Cochrane Collaboration is undertaking a systematic review of the effects of treatments in the management of acne that are currently considered complementary or alternative.

Intralesional steroid injections have been found to be beneficial for large inflammatory lesions. Comedone removal does not affect the course of the disease, but it does improve the patient’s appearance. [29] Additionally, some patients may benefit from superficial peels that use glycolic or salicylic acid, although more research needs to be conducted to establish best practice for these modalities.

I know firsthand how brutal these techniques were—and how counterproductive. In 2002, I interrogated an al-Qaeda associate named Abu Zubaydah. Blackheads are a mild form of acne that appear as unsightly, open pores that appear darker than the skin surrounding them. They get their dark appearance from a skin pigment called melanin, which oxidizes and turns black when it’s exposed to the air. Blackheads aren’t caused by dirt, but by sebum (oil) and dead skin cells blocking the pore. Almost all patients with this severe form of acne will require some form of systemic (oral) medication to control this condition. The goal of therapy is to minimize scarring. The choice of drug with which to begin treatment depends on the experience and preferences of the physician and patient. Even though isotretinoin does not remain in the body after therapy is stopped, improvement is often long-lasting. It is safe to take two or three courses of the drug if unresponsive acne makes a comeback. It is, however, best to wait at least several months and to try other methods before using isotretinoin again.

Some topical acne treatments (described below) may dry the skin. If this occurs, use a fragrance-free, water-based moisturising cream. Do not use ointments or oil-rich creams, as these may clog the holes of the skin (pores).

Over-washing is no solution, and excessive scrubbing can make your acne worse.1 Try to limit washing your face to twice a day, and be gentle.1 Dry, red skin can make acne appear worse.1 Remember, everyone’s skin is different.

Apart from being a commonly used antimicrobial agent, lemon oil also protects the skin from free radical damage (11, 12). This essential oil can be used anywhere on the body, except the private parts.

Patients with mild acne can be treated with topical therapies; however, those with moderate to severe acne will require systemic therapy. Oral antibiotic treatment, hormonal therapies and isotreretinoin are the mainstay systemic therapies for acne.

The genetics of acne may be out of your control—thanks mom and dad!—but there is plenty you can do in the fight for clearer skin. Whether you have the occasional pimple, regular breakouts, or more serious acne, Dr. Lee has advice for you.

Be patient. It can take weeks to see results from any medication. Your acne may look worse before it gets better. Don’t be surprised if you get redness, burning, or dry skin from your acne medicines. If it’s serious, call your doctor.

And that irritation of the skin usually triggers a reaction to repair the skin by producing still more oil. The overly vigorous washing of your skin will kill some skin cells that will land on top of new pores, trapping the newly produced oil. The net result is even more acne than you started with. The old advice on washing your skin doesn’t make your skin any better, but it’s a great marketing tool for the makers of soap and wash cloths.

3 Has a potential for abuse less than those in schedules 1 and 2. Has a currently accepted medical use in treatment in the United States. Abuse may lead to moderate or low physical dependence or high psychological dependence.

The diagnosis of acne vulgaris is primarily clinical.4 History and physical examination can help determine if there is an underlying cause of the acne, such as an exacerbating medication or endocrinologic abnormality causing hyperandrogenism (e.g., polycystic ovarian syndrome). Other dermatologic manifestations of androgen excess include seborrhea, hirsutism and androgenetic alopecia. Endocrinologic testing is not ordered routinely for women with regular menstrual cycles.2,3 Older women, especially those with new-onset acne and other signs of androgen excess (e.g., hirsutism, androgenic alopecia, menstrual irregularities, infertility), should be tested for androgen excess with measurements of total and free serum testosterone, dihydroepiandrosterone, and luteinizing and follicle-stimulating hormone levels.5 Pelvic ultrasonography may show the presence of polycystic ovaries.5 In prepubertal children with acne, signs of hyperandrogenism include early-onset accelerated growth, pubic or axillary hair, body odour, genital maturation and advanced bone age.

While rare, those with this condition can also often suffer from severe cases of acne.  Those with congenital Adrenal Hyperplasia experience an accumulation of adrenal steroid precursors and disrupt the metabolic rate of androgens. This increase in androgens may result in excess sebum, which catalyzes the formation of acne.

Many different treatments exist for acne. These include alpha hydroxy acid, anti-androgen medications, antibiotics, antiseborrheic medications, azelaic acid, benzoyl peroxide, hormonal treatments, keratolytic soaps, nicotinamide, retinoids, and salicylic acid.[73] They are believed to work in at least four different ways, including the following: reducing inflammation, hormonal manipulation, killing P. acnes, and normalizing skin cell shedding and sebum production in the pore to prevent blockage.[74] Common treatments include topical therapies such as antibiotics, benzoyl peroxide, and retinoids, and systemic therapies including antibiotics, hormonal agents, and oral retinoids.[19][75]

Like hair products, and skin care products contain ingredients that can cause acne. Common offenders include lanolin, mineral oil, aluminum, retinyl acetate, alcohol, oxybenzone, triclosan, parabens, polyethylene, BHA and BHT, and formaldehyde-based preservatives. Read ingredient labels to avoid putting these types of chemicals on your sensitive skin.

Selecting appropriate treatment in pregnant women can be challenging because many acne therapies are teratogenic; all topical and especially oral retinoids should be avoided.38 Oral therapies such as tetracyclines and antiandrogens are also contraindicated in pregnancy. Topical and oral treatment with erythromycin may be considered.

Treatment is normally no different than that for the milder acne forms. Over-the-counter products and even some home remedies can be effective, but since this type is relatively severe, it may be necessary to visit a dermatologist if the products or home remedies you are using are not effective. Many, though not all, over-the-counter products contain three important agents used in fighting acne:

The relationship between diet and acne is unclear, as there is no high-quality evidence that establishes any definitive link between them.[51] High-glycemic-load diets have been found to have different degrees of effect on acne severity.[7][52][53] Multiple randomized controlled trials and nonrandomized studies have found a lower-glycemic-load diet to be effective in reducing acne.[52] There is weak observational evidence suggesting that dairy milk consumption is positively associated with a higher frequency and severity of acne.[50][51][52][54][55] Milk contains whey protein and hormones such as bovine IGF-1 and precursors of dihydrotestosterone.[52] These components are hypothesized to promote the effects of insulin and IGF-1 and thereby increase the production of androgen hormones, sebum, and promote the formation of comedones.[52] Effects from other potentially contributing dietary factors, such as consumption of chocolate or salt, are not supported by the evidence.[51][54] Chocolate does contain varying amounts of sugar, which can lead to a high glycemic load, and it can be made with or without milk. Few studies have examined the relationship between obesity and acne.[2] Vitamin B12 may trigger skin outbreaks similar to acne (acneiform eruptions), or worsen existing acne, when taken in doses exceeding the recommended daily intake.[56] Eating greasy foods does not increase acne nor make it worse.[57]

Antibiotics, such as clindamycin solution (Topicil™, ClindaTech™) or erythromycin solution (Stiemycin™) and gel (Eryacne™), which are best used with benzoyl peroxide or azelaic acid to reduce the chance of antibiotic resistance

A study conducted by the Department of Dermatology at the University of Freiburg in Germany reports that using frankincense and five other plant extracts for antimicrobial effects on bacteria and yeast relating to the skin proved effective. The study concluded that their antimicrobial effects were powerful enough to be used as a topical treatment of some skin disorders, including acne and eczema. (19) 

Pimples are a normal skin condition that affect many people. Pimples are an inflammation of the skin in which the sebaceous glands (oil glands) become infected with bacteria, swell up, and fill with pus.

Salicylic acid. This ingredient helps prevent pores from becoming plugged. OTC salicylic acid products are available in strengths from 0.5 to 5 percent. Possible side effects include mild stinging and skin irritation.

The topical retinoids adapalene and tretinoin are prescription only and fully subsidised (one tube at a time) for use in patients with acne. Topical isotretinoin gel is available by prescription, but is not subsidised.

Hormonal changes during puberty increase sebum production.5 Predisposition to hyperkeratinisation leads to occlusion of hair follicles and sebaceous ducts, forming open or closed comedones.5 P. acnes bacteria in the follicle breaks down sebum into fatty acids and peptides and may rupture the follicle wall.5 This causes an inflammatory response and the formation of papules and pustules. Deeper inflammation leads to nodules and cysts.2 Scarring may then occur as deeper lesions heal.

Depending on your case, those treatments will help you totally get rid of them or drastically minimize their appearance. There is not  one best way to get rid of acne scars. Dermatologists might need to combine several treatments for your case.

Shari Marchbein, board-certified dermatologist and clinical assistant professor at New York University School of Medicine, explains that there are currently four FDA-approved birth control bills for acne, as well as a drug she suggests called Spironolactone, which helps to block hormones from stimulating the oil glands. Day adds that it’s one of the most reliable medications she uses, though if you’re thinking about starting a family, definitely opt for a different treatment, as it’s been known to cause birth defects in women of child-bearing age that aren’t also on birth control.

There are various topical antibiotic preparations. They reduce the number of bacteria and reduce inflammation. However, they have little effect on unplugging blocked pores. So, they are usually good at treating inflamed acne but blackheads and whiteheads may remain. You need a prescription to obtain a topical antibiotic. They may cause mild irritation but generally cause fewer side-effects than the other topical preparations. Topical antibiotics are usually prescribed in combination with other medicines (see below). Using them alone can increase the risk that the germ will become used to the antibiotic and make the treatment less effective.

Apply essential oils to active pimples. Neem oil and tea tree oil are thought to have antimicrobial properties that can kill acne-causing bacteria. Dab a drop of diluted version of tea tree oil or neem oil on each spot, or wet a cotton swab and wipe it over problem areas. [7]

Tea tree oil, also known as melaleuca, is one of my favorite and most recommended remedies for acne because it has amazing microbial properties that help fight the bacteria causes acne. A study published in the Australasian Journal of Dermatology revealed that tea tree oil provides positive results for mild acne with no serious adverse effects. The study asked participants to apply tea tree oil to the affected area of the face twice a day for three months with assessments conducted at four, eight and 12 weeks of use. The acne was reduced, making tea tree oil a great choice for how to get rid of pimples. (14)

Dr. Jegasothy recommends this intensive treatment that feature the double-whammy of vitamins C and E, two antioxidants known to improve red marks left behind by acne. It also adds gentle exfoliating acids to speed cell turnover for faster results.

The problem with prescription benzoyl peroxide usually is that it is too strong to use over the whole face. Most people experience stinging, burning, itching, flaking, peeling, and redness when they use benzoyl peroxide gels that contain more than 5% benzoyl peroxide, and doctors often start off their patients at 10% or more. Expect to see such side effects the first few weeks after starting treatment. This is normal and is known as the ‘hardening effect’ of Benzyl Peroxide. However, 1% to 3% of individuals who use this product react allergically to it. This results in extreme crusting of the skin and inflammation. This is not similar to the ‘hardening effect’ of BP, so it should not to be confused with it.

Simple way to get rid of acne is using home remedies. Tea tree oil or the straight oil is an alternative of benzoyl peroxide cream and prevent from mild acne. Soak the oil with the cotton pad then dip the pad in the water and apply to affected areas three times per day. Baking soda can use as a scrub by mixing with water and paste onto to skin for 30 seconds. It is commonly used for treatment-resistant acne. Oatmeal mask acts as a smoothing and healing of skin.

Regular application of this mixture is the fastest way to get rid of acne. Honey nourishes the skin and also kills the bacteria. Cinnamon possesses antimicrobial properties.  Its wound healing properties aid the body in fast healing of the pimple (38).

Cleansing more than twice a day is too much and can just dry out skin, “which can cause [it] to produce more oil to overcompensate.” Your Clarisonic addiction may not be helping either. “It helps remove all makeup and helps your cleanser work better, but I worry about the coarse ones. It’s almost like giving yourself microdermabrasion twice a day, which can cause a breakout,” says Dr. Kazin. Dr. Schultz seconds that: “Anything that rubs skin will, to a small extent, promote acne.” That includes a grainy or gritty cleanser, too. Try these two gentle face washes instead: Phace Bioactive Detoxifying Gel Cleanser or Frank Body Creamy Face Cleanser.

However, they have little effect on unplugging blocked pores – which you can see as blackheads and whiteheads (comedones). So, if you only have mild acne with just blackheads and whiteheads, you are better off using a topical treatment that unblocks holes in the skin (pores). If you have a lot of blackheads and whiteheads as well as inflamed acne spots, you may be advised to use a topical treatment such as benzoyl peroxide in addition to taking an antibiotic tablet.

We’ve all heard the foods that allegedly cause acne—chocolate, fried foods, pizza, caffeine, nuts. But Dr. Schultz reminds us that in large, statistically significant studies, these have not been proven to cause zits, but there are always exceptions. “If you break out when you eat chocolate, don’t eat chocolate.” Same with dairy, which again, has been shown in some cases to have an effect but no concrete cause-and-effect relationship exists.

First, your doctor cleanses your skin because the skin has to be clean and oil free before getting a chemical peel. It is good to wash the face a few times with a mild oil removing soap or use an alcohol wipe

Birth control pill (females only): A birth control pill can be an effective part of an acne treatment plan. When treating severe acne, a birth control pill may be used along with an antibiotic or a medicine called spironolactone (spy-ren-no-lac-tone) to get the acne under control.
