One way to treat acne is to stop the proliferation of the bacteria. In this science fair project, you will test a variety of over-the-counter acne medications to determine which is the most effective at preventing bacterial proliferation. Although Propionibacterium acnes (P. acnes ) is the bacteria which most frequently contributes to acne, in this science fair project you’ll be using the bacterium Escherichia coli (E. coli). E. coli is safer and easier to work with. You’ll use the Kirby-Bauer disk-diffusion method to measure the effectiveness of each acne medication. With this technique, bacteria is uniformly spread across an agar plate, then sterile disks, each coated with a compound (in this case, medication) to be tested, are placed on the surface of the agar. The compounds diffuse out from the disk into the agar. The concentration of the compound is highest right next to the disk and gradually reduces as you move out in concentric circles from the disk. If a compound is effective against the bacteria at a certain concentration, no colonies will grow in the area of the plate where that compound is equal to or higher than the effective concentration. The areas without bacterial growth are called zones of inhibition and are characterized by a clear circle around the disk in an otherwise contiguous lawn of bacteria. The larger the zone of inhibition is, as measured by the diameter of the clear circle, the more effective the compound.

Acne, on the other hand, is a skin affliction caused by sebum, an oily secretion in pores. It most often occurs on the face, neck, chest, and back. Contrary to popular belief, chocolate and greasy foods don’t contribute to acne, which is instead caused more by hormone production and genetics.

Combination therapy, for example with retinoids and antibiotics, is more effective than either agent used alone.13 However, the agents should be applied at separate times, unless they are known to be compatible.3 Benzoyl peroxide may oxidize a retinoid such as tretinoin if it is applied simultaneously.14 A 12-week randomized controlled trial involving 249 patients with mild to moderate acne showed treatment with adapalene gel 0.1% and clindamycin 1.0% to be superior to that with clindamycin 1.0% used alone.15 If inflammatory lesions are present, topical antibiotics containing benzoyl peroxide should be combined with a topical retinoid (e.g., topical antibiotic with benzoyl peroxide in the morning and retinoid at night). A review of three clinical studies with 1259 patients showed that a combination of clindamycin 1% and benzoyl peroxide 5% was more effective than either drug used alone in reducing lesions and suppressing P. acnes.16

Considerations: Regardless of the type of antibiotic prescribed, only about one half of patients respond. When antibiotics do produce results, these results are moderate at best.5-8Oral antibiotics should be used for only a short period of time, up to 6 months. However, even within this short time frame, antibiotics have been implicated in the proliferation of resistant colonies of bacteria. Some antibiotics cause increased sensitivity to sunlight. Exposure to the sun could cause a rash, itchiness, or redness, and you may be burnt more easily, so you’ll want to wear protective clothing and sunscreen.2Side effects may include upset stomach, diarrhea, nausea, headache, vomiting, dizziness, or light-headedness as your body becomes accustomed to it.2Minocycline is less prescribed because it works no better than any other antibiotic and comes with safety concerns, including the possibility of irreversible skin pigmentation.8-10

Antibiotics. Antibiotics may be used on top of the skin (topical) or taken orally (systemic). Antibiotics work by clearing the skin of acne-causing bacteria and reducing inflammation. There are several topical products available in creams, gels, solutions, pads, and lotions. Topical antibiotics are limited in their ability to penetrate the skin and clear more deep-seated acne, whereas systemic antibiotics circulate throughout the body and into sebaceous glands. However, systemic antibiotics often cause more side effects than topicals, but they can be used for more severe kinds of acne. Usually, topical antibiotics aren’t recommended alone as an acne treatment, as they can increase the risk for antibiotic resistance in skin bacteria. However, using benzoyl peroxide with a topical antibiotic may reduce the chances of developing antibiotic resistance.

Use a toner after cleansing. After you wash your face, exfoliate, or apply a face mask, apply a toner to the entirety of your face. Toners work to tighten pores making it less likely that dirt and oil will become trapped in them. Buy acne toners at a local drugstore, or use witch hazel or apple cider vinegar dabbed on with a cotton ball. Don’t rinse toners after application – allow them to stay on your skin.

Shower after sweating. After going for a run or basketball practice, remember to hit the showers. Sweat that’s not washed off your skin after exercise effectively creates a summer playground for bacteria that cause acne. Additionally, this sweat can also clog pores, causing acne to rear its ugly little head.

Most of the time, the glands make the right amount of sebum and the pores are fine. But sometimes a pore gets clogged up with too much sebum, dead skin cells, and germs called bacteria. This can cause acne.

Use products with benzoyl peroxide. You can use benzoyl peroxide in the form of either a soap or lotion over your acne-prone areas. This product works to clean off dead skin and help your skin to regenerate new, clean cells faster. Look for products that contain 3% or less of benzoyl peroxide to avoid irritating your skin.

Oil glands are small in children, but puberty (and testosterone in particular) causes these glands to grow larger. They secrete oil and cellular debris in a substance called sebum. Sebum keeps the skin flexible and acts as a protective barrier against water evaporation on the skin’s surface. It also keeps hair shiny and soft.

I usually break out with dairy/milk consumption. I recently heard about A2 milk which lacks the A1 protein that is suspected to cause inflammation. Cystic acne is the most severe of these three types. It is characterized by the appearance of cysts or cysts and nodules. Of the three types of acne vulgaris, this is the type that is most apt to lead to scarring. It is normally the only type where nodules are involved. By way of definition, a nodule is an enlarged papule, but it goes deeper into the integument than does a papule and it is chiefly this characteristic that distinguishes nodular acne from the milder, inflammatory type of acne.

The earliest pathologic change is the formation of a plug (a microcomedone), which is driven primarily by excessive growth, reproduction, and accumulation of skin cells in the hair follicle.[1] In normal skin, the skin cells that have died come up to the surface and exit the pore of the hair follicle.[10] However, increased production of oily sebum in those with acne causes the dead skin cells to stick together.[10] The accumulation of dead skin cell debris and oily sebum blocks the pore of the hair follicle, thus forming the microcomedone.[10] This is further exacerbated by the biofilm created by P. acnes within the hair follicle.[44] If the microcomedone is superficial within the hair follicle, the skin pigment melanin is exposed to air, resulting in its oxidation and dark appearance (known as a blackhead or open comedo).[1][10][19] In contrast, if the microcomedone occurs deep within the hair follicle, this causes the formation of a whitehead (known as a closed comedo).[1][10]

Scrubbing your face raw with grainy cleansers and exfoliating products can do more harm than good. When done too often, it can make redness, inflammation, and irritation worse. “Exfoliating a pimple can pull away healthy skin cells and create an open wound and higher risk for scarring,” says Jessica Weiser, MD, from New York Dermatology Group. “Exfoliation should be done with caution, and not more than 2-3 times a week maximum.”

Topical retinoids are used in anti-aging efforts, and can speed up the rate of cell regeneration, aiding your skin in sloughing off the dead skin cells at a regulated rate. In some patients, retinoids can also serve as an anti-inflammatory agent, but they have been known to cause irritation when first used.

Topical antibiotics are mainly used for their role against C acnes (formerly P acnes). They may also have anti-inflammatory properties. Topical antibiotics are not comedolytic, and bacterial resistance may develop to any of these agents. Commonly prescribed topical antibiotics for acne vulgaris include clindamycin, erythromycin, or, more recently, dapsone. Topical dapsone is a new sulfone antibiotic with anti-inflammatory properties that has been shown to be effective for mild-to-moderate acne, and it has a convenient once-daily application schedule. [34] It is available as 5% twice-daily and 7.5% once-daily formulations. [34] The current American Academy of Dermatology guidelines preceded the FDA approval of the 7.5% formulation. Although no research has compared the efficacy of the 5% formulation with the 7.5% formulation, both have been separately shown to be efficacious and safe. The 7.5% formulation has the additional compliance factor of once-daily application. [29, 34]

Olive oil soothes the inflammation in and around the pimple. It also moisturizes the skin, thus bringing a balance in the amount of sebum that is produced. Acne scars will soon start fading away with regular usage of this oil (44).

Beware of “miracle cures.” There are no overnight cures for acne. If you hear someone on TV or the radio promising a fast, guaranteed treatment for acne, it’s just hype to get you to buy their product.

In 2007, a study published in the American Journal of Clinical Nutrition found that glycemic load can greatly affects acne. 43 males with acne, aged 15 to 25, were separated into two groups. For twelve weeks, one group ate a diet that was 25 percent protein and 45 percent low-glycemic carbohydrates. Omega-3-fatty acid supplementation is useful for inflammatory acne in about 2/3 of the cases as this study showed. Here is a patient from this study who benefitted from omega-3 supplementation. I usually break out with dairy/milk consumption. I recently heard about A2 milk which lacks the A1 protein that is suspected to cause inflammation. I drank a liter over a few days and was relieved that my acne didn’t flare up. Just wondering if anyone else has heard of this and what they’ve experienced.

If you have severe acne which does not respond to the treatments usually prescribed by GPs, you may need to see a hospital specialist. In particular, if your doctor believes you would be helped by isotretinoin tablets. You may also need hospital treatment for acne scars. Options available for scarring include laser resurfacing of the skin, mechanical or chemical peeling of the skin, breaking the scar tissue down with a sterile needle (subcision) and injection of collagen filler.

Turn the water temperature down before you rinse conditioner out of your hair. Warm water will open up pores, while cool water will close them. Having open pores right when your conditioner is slurrying down your hair is not a good recipe for a pimple-free back.

Cinnamon and Honey Mask: Mix two tablespoons of raw honey, one teaspoon of coconut oil and 1/2 teaspoon of cinnamon. Smooth over face. Keep away from eyes, as the cinnamon can be an irritant. Relax for 5–10 minutes and gently remove with damp cloth. Honey and cinnamon used together helps to fight acne because of its anti-inflammatory, antioxidant and antibacterial properties.

As isotretinoin waged a scorched-earth strategy on my face that left me feeling like my skin was practically falling off, I was also weirdly excited by the changes I saw taking place. If I could molt my skin like a snake and discard my bacteria-ridden, blemished skin, it gave me hope that a fresh, healthy layer would finally surface.

DIY scrubs to fight acne and keep skin fresh are easy to make and economical. First, you need something that is gritty. Sea salt, brown sugar and ground oatmeal are good choices. In addition, you need a base. Coconut oil, kefir and honey are all good choices. These bases help to fight bacteria, fungi and Candida overgrowth on the skin while the textured ingredients help to unplug pores and remove dead skin.

Although it is not contagious or infectious, acne is classed as a disease which affects the skin. The pores in your skin are right next to the oil glands which lie under the skin. It is the job of the glands to produce an oil substance called sebum. The glands are connected to the pores by the canal called a follicle. The follicle is the method whereby the oil carries the dead skin cells to the surface of the skin. It is also the place where a single strand of hair grows. When the follicle gets clogged with oil, it does not bring the dead skin cells to the surface, and a pimple forms.

Drugs: Some medications may cause or worsen acne, such as those containing iodides, bromides, or oral or injected steroids (either the medically prescribed prednisone [Deltasone, Orasone, Prednicen-M, Liquid Pred] or the steroids that bodybuilders or athletes sometimes take). Other drugs that can cause or aggravate acne are anticonvulsant medications and lithium (Eskalith, Lithobid). Most cases of acne, however, are not drug related.

Sometimes, birth control pills are used along with a drug called spironolactone to treat acne in adult females. This medication lowers levels of the hormone androgen in the body. Androgen stimulates the skin’s oil glands. Side effects include irregular menstruation, breast tenderness, headache, and fatigue. Spironolactone is not an appropriate form of acne treatment for all people.

Toothpaste can help dry out a pimple, but it’s not a real solution for acne and can damage your skin. Stick to products made specifically for acne. Alternatively, there are some natural solutions you could try. For example, many people have great success clearing up acne by washing the affected area with honey (particularly manuka honey) daily or using it as a mask. You can also use it as a spot treatment.

On this subject, Renée is most clear: “Whenever a blemish appears, the goal is to get it to heal quickly. Most people have a myriad of zit-zapping spot treatments in their bathroom and will apply these generously the moment a blemish pops up in hopes of drying it out to get it to go away more rapidly. But most spot treatments actually make the blemish stick around longer when used in this way. It will also lead to a prolonged recovery time with scarring.” The reason is that when used too early, the drying lotion will dry out the skin on top of the infection, actually trapping the infection from coming out. “Have you ever tried squeeze a blemish that has been dried out with a spot treatment? I’m sure you’ve experienced sheets of skin peeling off which will result in more scarring. Not good,” Renée says. “The best solution is to leave it alone to get rid of it faster.” Frustrating, but probably true.

Tea tree oil is an effective remedy for pimples. But if you are wondering why you should add oil to your oil-clogged face, then hear this: Tea tree oil is unique. Unlike the natural oil produced by your skin, it acts like a solvent and penetrates through the extra sebum, unblocks the pores and eliminates the dead cells. It also has strong antibacterial properties.

Typical features of acne include increased secretion of oily sebum by the skin, microcomedones, comedones, papules, nodules (large papules), pustules, and often results in scarring.[28][29] The appearance of acne varies with skin color. It may result in psychological and social problems.[19]

But know that the cheaper price point isn’t free of drawbacks. The Neutrogena kit misses the mark with its Skin Polishing Acne Cleanser. It’s the only product in the kit with salicylic acid, and you end up washing it all off anyway. A recent “formula improvement” in the acne control lotion has left customers leaving less-than-pleased comments on the Neutrogena website, too.

If acne isn’t responding to glycolic or salicylic treatments, Dr. Birchall recommends adding retinol to your nightly routine. Unlike gentler exfoliation methods, this topical form of vitamin A helps prevents pores from getting clogged. With strong scientific evidence behind it, retinol or retinoids are among the best treatments for persistent acne. The adverse effects of benzoyl peroxide include skin irritation, dryness and redness. Direct contact with eyes or other mucous membranes will cause severe irritation. Most of the adverse effects can be minimised by reducing the time the product is on the skin before being washed off, or by reducing the concentration used. Advise the patient that if their skin peels or becomes very dry, an oil-free moisturiser can be used. Benzoyl peroxide will bleach linen, clothing and towels.
