Sometimes skin cells clog up these follicles, also known as pores. With the pores clogged, sebum gets trapped inside. Skin bacteria called Propionibacterium acnes (P. acnes) starts to grow inside the pores, too. The bacteria cause inflammation. When the clogged pore bursts open, all the sebum and P. acnes bacteria spill onto the surface of the skin, causing breakouts.

The diagnosis of acne vulgaris is primarily clinical.4 History and physical examination can help determine if there is an underlying cause of the acne, such as an exacerbating medication or endocrinologic abnormality causing hyperandrogenism (e.g., polycystic ovarian syndrome). Other dermatologic manifestations of androgen excess include seborrhea, hirsutism and androgenetic alopecia. Endocrinologic testing is not ordered routinely for women with regular menstrual cycles.2,3 Older women, especially those with new-onset acne and other signs of androgen excess (e.g., hirsutism, androgenic alopecia, menstrual irregularities, infertility), should be tested for androgen excess with measurements of total and free serum testosterone, dihydroepiandrosterone, and luteinizing and follicle-stimulating hormone levels.5 Pelvic ultrasonography may show the presence of polycystic ovaries.5 In prepubertal children with acne, signs of include early-onset accelerated growth, pubic or axillary hair, body odour, genital maturation and advanced bone age.

Dermatologists have more tools to treat acne and can prescribe stronger concentrations of OTC medications and oral antibiotics. They also offer light and laser therapy and chemical peels. But these treatments are probably not necessary to treat a patient’s acne, Armstrong says.

Almost all patients with this severe form of acne will require some form of systemic (oral) medication to control this condition. The goal of therapy is to minimize scarring. The choice of drug with which to begin treatment depends on the experience and preferences of the physician and patient. It is often warranted to try oral antibiotics initially (doxycycline [Doryx, Oracea, Monodox], sulfamethoxazole/trimethoprim [Bactrim, Sulfatrim], minocycline [Minocin]) for a limited period of time supplemented by a topical retinoid drug (Retin A, Differin, Tazerotene). Antibiotics probably owe much of their effectiveness to their anti-inflammatory characteristics rather than their antibacterial attributes.

The oral antiandrogen spironolactone can be added if oral contraceptives are not effective.3 Spironolactone is a 5α-reductase inhibitor when administered at higher doses.35 Spironolactone, used alone or as an adjunct at doses of 50–200 mg/d, has been shown to be effective in improving acne, but this is based on limited evidence.2 However, patients should be warned about possible side effects, including hyperkalemia, menstrual irregularities and feminization of a male fetus. Antiandrogen therapy alone may be successful, but in less than half of women;36 the acne may recur when it is discontinued. Combination therapy with topical agents or oral antibiotics provides substantially more benefit.37

Don’t touch your face. This is incredibly difficult, as people are prone to touching their faces on a regular basis. Be careful about scratching your face, resting your face on your hands, and picking at pimples. Never pop your zits or squeeze at pesky blackheads, as this only introduces more bacteria to your skin and will make your acne worse.

Exfoliating cleansers and masks: A variety of mild scrubs, exfoliants, and masks can be used. These products may contain salicylic acid in a concentration that makes it a very mild peeling agent. These products remove the outer layer of the skin and thus open pores. Products containing glycolic or alpha hydroxy acids are also gentle skin exfoliants.

The toothpaste really works. I first tried putting it on at night- and I suppose it helped some. But what works better for me is to put it on after I wash my face in the morning with a scrub. It stings more but it’s not that bad. I keep it on for 30 minutes and then I wash it off.

Doxycycline is another of the tetracyclines that is equally effective in treating acne. It comes in generic versions and also as the branded Doryx and Acticlate which are easier on the stomach. Originally FDA approved for the treatment of rosacea, Oracea is a non antibiotic dose of doxycycline that is often used as an acne treatment, as well. To avoid further clogging of your pores, all cosmetics, lotions, and sunscreens should be oil-free! To avoid dry skin, use an oil-free lotion such as Complex 15. The adverse effects of benzoyl peroxide include skin irritation, dryness and redness. Direct contact with eyes or mucous membranes will cause severe irritation. Oil glands are small in children, but puberty (and testosterone in particular) causes these glands to grow larger. They secrete oil and cellular debris in a substance called sebum. Sebum keeps the skin flexible and acts as a protective barrier against water evaporation on the skin’s surface. It also keeps hair shiny and soft.

Isotretinoin can clear severe acne when other treatments fail. With just one treatment, isotretinoin can permanently clear the skin. One treatment typically lasts 4 to 5 months. Recent studies conducted by dermatologists in 2012 show that green tea has a positive impact on acne, when applied externally. Green tea has an antioxidant known as epigallocatechin-3-gallate (EGCG). It was observed that EGCG reduced inflammation, sebum production and bacterial growth on pimple-prone skin. Place 2 teaspoons of organic tea leaves in 1/2 cup of fresh boiling water. Steep for 5 minutes; strain the leaves and let the mixture cool. Then use a cotton pad to apply it on your face. You may also put it inside a spray bottle and spray it on. Leave it on overnight.

This is a thorny issue, because Big Pharma has a firm hand in the treatment of acne and they are supporting symptomatic treatment of acne rather than treating the cause. There are surface treatment modalities that are supposed to open the skin pores: peeling agents such as benzoyl peroxide. General practitioners often treat the infection with antibiotic pills (tetracycline or erythromycin), but this is not treating the cause, only the super infection that comes from the plugged up skin pores (stasis of sebaceous gland secretions). These include skin dryness, eye dryness, muscle and bone pains, headaches, liver enzyme abnormalities, and instability of mood including depression and causing birth defects in the fetus of a pregnant woman (Ref. 3). In 2009 the manufacturer stopped distributing the drug in the US, because of too many lawsuits regarding damages from the drug.

You may hesitate to put sunscreen on your face because you’ve noticed that after a day in the sun, your complexion looks clear and breakout-free. But the inflammation from sunburn can make your acne worse or cause more dark spots. Plus, staying out in the sun without sunscreen raises your risk of skin cancer.

Exfoliate your skin. Get into the habit of exfoliating with an acne scrub every week, literally every week. Exfoliating means gently wiping away the top layer of dead skin cells that accumulate on the epidermis, clearing the way for a nice new layer of vibrant, acne-free skin. A disease whose exact cause is unknown can be difficult or impossible to prevent. In the case of acne, what contributes to an outbreak is generally known, meaning there are things you can do to significantly reduce the risk of having a problem. Cleanliness is the number one preventive measure as it will greatly reduce accumulations of old skin cells and excess oil, which lead to clogged pores. A clogged pore does not always cause an acne problem but it definitely is a leading contributor. I would not recommend using the garlic remedy if you have sensitive skin like me. I had a large pimple on my nose which then caused me to find out how to get rid of it so I then put garlic on it and moments later i removed it from the pimple and noticed a large burn on my skin which then didn’t go away for another month leaving me with a scar on the spot where the pimple was. Although the acne did go away I am still left with a large red mark where it appeared on my nose.

[31] A 2015 large retrospective study of healthy women aged 18-45 years confirms potassium monitoring is unnecessary for these patients while taking spironolactone. [48] Pregnancy must be avoided while taking spironolactone because of the risk of feminization of the male fetus, and spironolactone is not recommended for males because of the potential for gynecomastia. [31, 46] While a black box warning regarding possible cancer risk was placed on spironolactone many years ago after rats fed high doses of the medication developed both benign and malignant tumors, several large retrospective and longitudinal studies have found no association with cancer. [49]
