Hormonal changes during puberty increase sebum production.5 Predisposition to hyperkeratinisation leads to occlusion of hair follicles and sebaceous ducts, forming open or closed comedones.5 P. acnes bacteria in the follicle breaks down sebum into fatty acids and peptides and may rupture the follicle wall.5 This causes an inflammatory response and the formation of papules and pustules. Deeper inflammation leads to nodules and cysts.2 Scarring may then occur as deeper lesions heal.

“Acne occurs frequently after the teenage years and at significantly higher rates in women compared with men,” says Doris Day, a clinical associate professor of dermatology at NYU Langone Medical Center and the author of 100 Questions & Answers About Acne (Jones & Bartlett Learning). In fact, Day points out, “as many as 50 percent of women will suffer from acne at some point in their adult lives,” with more than half of women in their 20s and 35 percent of women in their 30s experiencing some form of acne.
Not surprisingly, the answer to this question is yes. Vodka works as an astringent because of its alcohol content. When applied on the face, it cleanses it and kills the bacteria. It is also said to shrink pores. Unfortunately not many people have tried this remedy. However, there are a few who vouch for it. If you do plan to try this rather offbeat home remedy, be prepared as vodka might sting on the sensitive, inflamed skin.

Isotretinoin is associated with a range of serious adverse effects (Table 1). Some, particularly cheilitis, are so common that they can be considered indicators of adherence.17 Isotretinoin may cause an initial worsening of acne, but severe flares are uncommon. It is important to discuss these adverse effects with patients prior to initiation, to optimise adherence. Most adverse effects are dose-related and starting with a low dose may reduce the incidence and severity.
An earlier study published in 2007 showed similar results—Australian researchers found that young men between the ages of 15 and 25 with mild-to-moderate acne experienced dramatic improvement when they switched from eating the typical American diet (with white bread and highly processed breakfast cereals) to a healthier diet of whole grains, lean meat, and fruits and vegetables.
Wash problem areas with a gentle cleanser. Twice a day, use your hands to wash your face with a mild soap and warm water. If you tend to develop acne around your hairline, shampoo your hair every day. And be gentle if you’re shaving affected skin.
In severe cases of acne, a visit to dermatologist or skin specialist is required to get rid of acne. Prescribed medication is needed for severe acne. Medication can be in the form of oral antibiotics, cream or lotion.
“What then happens is that they start trying different products, abandoning them very quickly if they do not see results in a day or two. They also add one product to another. Sometimes the products can cause irritation of the skin and add further insult to the owner,” she says.
With acne, the inflammation will develop to the extent that the follicle is ruptured and a pustule with fluid is formed. As a result, pimples and acne cause inflammation that can result to severe damage to the skin.

Recurrent acne may be treated with a further course of isotretinoin, and in some cases long-term, low-dose treatment is appropriate, however, discussion with a Dermatologist is recommended if this is being considered.18

Medical research is constantly looking for ways that new drugs can be made to treat acne. As with most bacterial infections, the problem is that over a period of time, bacteria become drug resistant and certain antibiotics will no longer work. There is research work going on to see how effective the use of probiotics is in the struggle with acne.

Egg whites provide a cheap, easy and quick way to remove pimples. They also eliminate scars. Egg whites contain vitamins and amino acids that help to kill pimples and re-build your skin’s cells. Separate 3 egg whites from their yolks. Whisk them properly and allow them to settle for 3 minutes. Apply the whisked egg whites to the pimples using your fingers. Wait for it to dry then repeat this 4 times. Then leave the final mask for 20 minutes, rinse off and apply a suitable skin moisturizer.

The Pore Targeting Treatment gel and Complexion Perfecting Hydrator moisturizer slip on nicely, with the former powered by skin-loving glycerin and the latter by a whole slew of delicious ingredients, including licorice root extract, sodium hyaluronate, bisabolol, and allantoin. We are iffy on the Skin Smoothing Exfoliator face wash, though, and not only because the microbead granules can be harsh on active breakouts. “Acne products that are washes will tend not to work as well as leave-on or rub-in products,” explains Dr. Green. “Think about it. How well can a product work when rinsed off a few seconds after you put it on?”

For 6 –12 months after an acne breakout, stay out of the sun as much as possible to avoid making dark spots and scars worse. When you are in the sun, use an all-natural sunscreen to protect skin. Topical clindamycin and erythromycin are antibiotics that are also anti-inflammatory drugs and are effective against a number of bacteria. They should always be combined with benzoyl peroxide or a topical retinoid and applied directly to the skin. Oral erythromycin is also available, but you may become resistant to its effects, limiting its usefulness.

What’s more, Zeichner brings up the fact that now more than ever women are starting birth control pills at a young age, with many staying on the drug for decades. This might be fine while it lasts, as the hormones in the pill help to regulate oil production and keep acne under control, but Zeichner explains that it can turn into an issue after discontinuation, because the body is then forced to go through an “adjustment period” to the normal hormonal environment that the oil glands haven’t experienced in years. Needless to say, it could be worth taking into consideration next time you’re due to discuss birth control options with your OBGYN.

The recommended starting dose of isotretinoin is 0.5 mg/kg/day. Daily doses can be titrated up or down, between 0.1 and 1 mg/kg/day, depending on response to treatment and presence of adverse effects. The total dose of isotretinoin over the course of treatment should be between 120 – 150 mg/kg of body weight.14 The length of treatment is dictated by the daily dose. Courses usually last between three to four months but may continue one year or longer. Total doses of >150 mg/kg are associated with increased adverse effects.16 The dose and duration of the course may be adjusted on an individual basis; some patients will wish to reduce the dose or stop isotretinoin when the active acne has settled.
