Teenagers first develop acne in response to their body’s production of androgens. Pseudofolliculitis: This is sometimes called “razor bumps” or “razor rash.” When cut too close to the skin, growing hairs twist into the skin and produce tender bumps. The other type of breakouts are inflammatory acne, which occur when an already clogged hair follicle experiences an overgrowth of P. Acnes bacteria, which causes an immune reaction, leading to inflammation and infection. It is a myth that people get acne because they don’t wash enough. Gentle washing is important, but too much washing can irritate the whiteheads and blackheads, causing them to be more infected and making more pimples.

Cystic acne is the most severe of these three types. It is characterized by the appearance of cysts or cysts and nodules. Of the three types of acne vulgaris, this is the type that is most apt to lead to scarring. It is normally the only type where nodules are involved. By way of definition, a nodule is an enlarged papule, but it goes deeper into the integument than does a papule and it is chiefly this characteristic that distinguishes nodular acne from the milder, inflammatory type of acne.

Salicylic acid vs benzoyl peroxide has long been a hot topic in the treatment of acne. Salicylic acid is a Beta Hydroxy Acid and has proven to be one of the most effective acne treatments, and a safer option than benzoyl peroxide. Almost all patients with this severe form of acne will require some form of systemic (oral) medication to control this condition. The goal of therapy is to minimize scarring. It is often warranted to try oral antibiotics initially (doxycycline [Doryx, Oracea, Monodox], sulfamethoxazole/trimethoprim [Bactrim, Sulfatrim], minocycline [Minocin]) for a limited period of time supplemented by a topical retinoid drug (Retin A, Differin, Tazerotene). Antibiotics probably owe much of their effectiveness to their anti-inflammatory characteristics rather than their antibacterial attributes.

Zinc is a mineral your body needs in small amounts that not everyone gets enough of, especially vegetarians. That’s because zinc is most abundant in red meat, organ meats and seafood, although whole grain foods and nuts are decent sources. One of the characteristics of nodular acne is that its size or appearance does not always give an actual indication of its severity. The one thing that does characterize it is its appearance as a big bump or pimple – for lack of a better description. Nevertheless, this rather simplistic-sounding description doesn’t mean that if you have a large bump or pimple, it is a nodule. It can also be a cyst. Diet therapy has been suggested. Fulton et al performed a study on chocolate, having teenage patients with acne consume 1 bar of chocolate each day. Some of the patients improved and some worsened, but the vast majority were unchanged. [67] This study helped decrease the emphasis on diet as a causal factor in acne vulgaris. However, investigators always returned to the diet question. Data suggest that the westernization of certain Native American populations and the related consumption of unhealthy “junk” foods (eg, potato chips, soft drinks) has had a negative impact on general and skin health, resulting in acne flares. Skim milk, compared with full-fat milk, has been found to have a positive association with acne, especially in teenagers and young adults. [68, 69] Causation was not able to be drawn from the case control studies, but the mechanism involving hormonal constituents of the skim milk has been postulated. [68] . Whey protein is a commonly used supplement that has been suggested to worsen acne and should be discontinued if flaring of the condition is associated with its use.

Grade IV: This is the most severe of all acne grades, and individuals with severe acne will exhibit a large number of papules, pustule, nodules, and pus-filled cysts. Severe acne is often observed on varied areas of the body, including the back and chest. This type of acne is the most likely to cause permanent skin damage in the form of scarring and disfiguration if treated improperly.

Benzoyl peroxide products are also effective against C acnes (formerly P acnes), and bacterial resistance to benzoyl peroxide has not been reported. [35] Benzoyl peroxide products are available over the counter and by prescription in a variety of topical forms, including soaps, washes, lotions, creams, and gels. Benzoyl peroxide products may be used once or twice a day. These agents may occasionally cause a true allergic contact dermatitis. More often, an irritant contact dermatitis develops, especially if used with tretinoin or when accompanied by aggressive washing methods. [35] If intensive erythema and pruritus develop, a patch test with benzoyl peroxide is indicated to rule out allergic contact dermatitis.

2 Use only your bare hands to wash, and wash for 10 seconds or less. Washcloths and hand-held cleansing devices are unnecessarily irritating. Consider the act of washing as simply prepping your skin for medication, nothing more. Washing itself does not clear breakouts since dirt does not cause acne, so there is absolutely no need to scrub.

People with severe acne, treatment-resistant acne or older adults with persistent acne may require oral isotretinoin.5 Isotretinoin is associated with many adverse effects, is a major teratogen and requires monitoring throughout treatment, therefore may not be a suitable option for everyone.14 If isotretinoin is unsuitable or is not tolerated, a higher dose of antibiotic, e.g. doxycycline 100 mg, twice daily, if tolerated and not already trialled, and discussion with, or referral to, a dermatologist is recommended.2, 4

Alternative and integrative medicine approaches used in the treatment of acne include fish oil, brewer’s yeast, probiotics, oral zinc and topical tea tree oil. More research is needed to establish the potential effectiveness and long-term safety of these and other integrative approaches, such as biofeedback and traditional Chinese medicine. Talk with your doctor about the pros and cons of specific treatments before you try them.

1. Wash your face regularly — with a targeted face wash. Find a facial cleanser with ingredients that treat acne without stripping your skin of its natural moisture and oils. Ideally, this is a formula that doesn’t contain a detergent and is non-foaming; good ingredients to look for are salicylic acid, charcoal, or sulfur. Acne is an inflammatory disorder of pilosebaceous units and is prevalent in adolescence. The characteristic lesions are open (black) and closed (white) comedones, inflammatory papules, pustules, nodules and cysts, which may lead to scarring and pigmentary changes (Figures 1 to 4). The pathogenesis of acne is multifactorial and includes abnormal follicular keratinization, increased production of sebum secondary to hyperandrogenism, proliferation of Propionibacterium acnes and inflammation.2,3

Mild acne is predominately indicated by the presence of non-inflammatory lesions (i.e. comedones – see “The vocabulary of acne”, over page). Some inflammatory lesions (pustules or papules) may be present, but generally less than 10 – 15.

Out of  all of the drawbacks to the drug, the biggest hurdle for me was the monthly blood test. I have low blood pressure that causes me to routinely pass out after having my blood drawn, and just the sight of blood makes me queasy.  

Tea tree oil, also known as melaleuca, is one of my favorite and most recommended remedies for acne because it has amazing microbial properties that help fight the bacteria causes acne. A study published in the Australasian Journal of Dermatology revealed that tea tree oil provides positive results for mild acne with no serious adverse effects. The study asked participants to apply tea tree oil to the affected area of the face twice a day for three months with assessments conducted at four, eight and 12 weeks of use. If you really must do something about your pimple beyond washing your face and spot treating, ice or a cold compress can help reduce swelling. Wrap an ice cube or the compress in a soft tissue or cloth and apply it to your zit for 20-30 seconds at a time, a few times a day. In case of an emergency (like, prom), you can also see a dermatologist for a cortisone injection, which can help shrink the cyst down quickly in a day or two.

Sometimes skin cells clog up these follicles, also known as pores. With the pores clogged, sebum gets trapped inside. Skin bacteria called Propionibacterium acnes (P. acnes) starts to grow inside the pores, too. The bacteria cause inflammation. When the clogged pore bursts open, all the sebum and P. acnes bacteria spill onto the surface of the skin, causing breakouts.

For mild acne, Dr. Wong recommends: “Face washes and leave-on creams that contain Benzoyl Peroxide or Salicylic Acid can be very effective. Prepare a mix of one part apple cider vinegar and one part filtered drinking water in a spray bottle. After showering and before bed, spray the vinegar solution on your skin and let dry. This process will naturally lower your skin’s pH.[6]

Acne removal: Your dermatologist may perform a procedure called “drainage and extraction” to remove a large acne cyst. This procedure helps when the cyst does not respond to medicine. It also helps ease the pain and the chance that the cyst will leave a scar. If you absolutely have to get rid of a cyst quickly, your dermatologist may inject the cyst with medicine.

Isotretinoin. Isotretinoin is known under the brand name Accutane and a number of generic names. It is an oral product used mostly for treating severe acne. However, it’s especially dangerous before and during pregnancy as the drug is known to affect a fetus and cause serious birth defects.

Steer clear of detergent soaps like Ivory and abrasive soaps like Lava, as well as anything that contains fiber (polyethylene beads are OK). Your shoulder acne will clear up faster if you avoid anything that causes irritation to your skin, such as alcohol-based skin rubs, capsaicin creams for muscle pain, cologne, and clothes treated with fabric softener or dried in a drier with static control sheets.

Most home remedies work for well with superficial to minimal scars. This that if you are more interested in knowing how to get rid of deep acne scars or if you have very deep ice pick, box car, rolling or hypertrophic scars, it may be best to see a doctor and undergo a treatment in the clinic or stick with the treatments above.

Using the same technique, dip a disk in the sterile water. Apply one disk of sterile water to each agar plate. This will serve as a negative control. Sterile water should not create zones of inhibition; if zones do appear, you have a cross-contamination problem.

A dermatologist can prescribe oral antibiotics as a quick fix for acne, but it’s not typically the first choice. There has also been an important methodological improvement in isotretinoin prescribing. Recording of patient NHI numbers on prescriptions has improved markedly since access to isotretinoin was widened to include General Practitioner prescribing. From July, 2011 – June, 2012 NHI numbers were recorded in 91% of all isotretinoin dispensings compared to only 60% prior.25 Recording of NHI numbers allows analysis of medicine use to occur.

More than 80% of adolescents suffer from acne. Even adults and younger children are affected by these annoying breakouts. A simple pimple may take three to seven days to go away by itself. Sometimes, they last longer and indicate a more complex underlying cause that needs to be treated.

To avoid further clogging of your pores, all cosmetics, lotions, and sunscreens should be oil-free! To avoid dry skin, use an oil-free lotion such as Complex 15. There are multiple androgenic, anabolic growth hormones present in cow’s milk. Consumption of milk and dairy products leads to worsening of acne, as there is no such thing as hormone-free cow’s milk. There are surface treatment modalities that are supposed to open the skin pores: peeling agents such as benzoyl peroxide. General practitioners often treat the infection with antibiotic pills (tetracycline or erythromycin), but this is not treating the cause, only the super infection that comes from the plugged up skin pores (stasis of sebaceous gland secretions). Another approach is topical application of antibiotic and peeling agent in combination (1% clindamycin and 5% benzoyl peroxide gel), which is applied twice daily (Ref.2). Resistant cases, usually the ones who have a family history of severe acne, have been treated by a skin specialist who has a special license to treat with isotretinoin (Accutane), a vitamin A derivative, which works in many cases, but which can have serious side effects. These include skin dryness, eye dryness, muscle and bone pains, headaches, liver enzyme abnormalities, and instability of mood including depression and causing birth defects in the fetus of a pregnant woman (Ref. 3). In 2009 the manufacturer stopped distributing the drug in the US, because of too many lawsuits regarding damages from the drug.

If you are sensitive to salicylic acid or find commercial products ineffective for your back acne (bacne), try a medicated skin powder. This will usually work well and won’t over-dry your skin. Speak to your pharmacist about brand options.

Cosmetic blotting papers offer a great option for removing oil because they don’t dry out your skin. “Patients with oily skin really love blotting paper because it’s convenient and easy to use,” Armstrong says. Apply it to oily areas, such as forehead, nose, and chin. Don’t scrub your skin with the sheet of blotting paper. Instead, simply press it against the oily area long enough to absorb oil, usually 15 to 20 seconds. Some blotting papers are lightly powdered, which further reduces shine.
